          본 논문에서는 객체관계형 지리정보 시스템의 설계 및 구현에 대하여 논한다. 본 지리정보 시스템은 상용의 객체관계형 데이터베이스 시스템을 하부 구조로 사용하며, 객체지향 데이터 모델에 근거한 공간 객체모델, 공간 질의어 및 시각적 사용자 인터페이스를 제공하고 있다. 특히 본 시스템에서는 데이터베이스 상의 주색인(primary index)기법으로 적용 가능한 DOT 공간 색인 기법을 이용하여 효율적인 공간 질의 처리를 수행한다. 또한 본 시스템에서는 SDTS데이터 모델을 개념적 모델로 사용하며, SDTS의 공간 객체에 관한 개념적 모델은 지리정보 시스템 내에서 내부 데이터 모델인 객체지향 데이터 모델로 전환된다. This paper presents the design and implementaion of an object-relational geographic information system. This system has been developed on top of a commercial object-relational database management system. It provides flexible spatial data model, spatial query language, visual user interface, and efficient spatial access methods(D0T) in which traditional primary-key access methods can be applied. We report on our design choices and describe the current status of Implementation. The conceptual model of the system is based on SDTS, and is mapped to the intemal obiect-oriented data model.         • 지식기반 지리정보시스템의 개발

          지리정보 시스템에 기반한 응용 시스템 개발에 있어 필수적인 기능으로 지도작성, 데이타 관리, 해석 기능 외에 지식 표현 및 추론 기능을 들 수 있다. 본 논문에서는 지식베이스 관리 및 연역추론 기능을 갖는 지식기반 지리정보 시스템의 개발과 연역 기능을 활용한 시스템 기능 확장에 대하여 논한다. 본 시스템에서는 사용자 인터페이스(Visual Basic), 지리정보 시스템(ArcView, ArcInfo), 추론 시스템(Eclipse)을 상호 밀결합 방식으로 결합, 구현하였으며, 각 서브 시스템은 서로 중간 파일 시스템의 사용없이 데이타 및 명령어의 전송 및 공유가 가능하다. 또한 사용자는 사용자 인터페이스를 통하여 개개의 서브 시스템을 인식하지 않고 단일화된 환경하의 작업이 가능하다. 시스템의 연역 기능은 일반적으로 거론되는 지식베이스 관리, 의사결정 지원 기능 외에도 사용자 환경개선, 복합 공간 객체의 표현, 공간질의 연산자 구현 등의 시스템 기능 확장에 활용될 수 있다. 특히 본 시스템에서는 사용자 환경 개선에 초점을 맞추어 사용자가 정보의 내부 구조나 문제 영역, 명령어 사용 방법 등을 잘 알지 못하더라도 유용한 정보를 얻도록 지원, 유도하는 협력질의 응답 기능과 명령어 자동 생성 기능을 제공한다. 또한 본 논문에서는 이들 방식을 이용한 두가지 으용 시스템(여행정보 시스템, 환경관리 시스템)의 구현 예를 들어 본 시스템의 실용성과 유용성을 보인다.

          이해하기 어려운 갑상선기능검사의 해석

          With the generalized use of highly sensitive thyroid stimulating hormone (TSH) and free thyroid hormone assays, most thyroid function tests (TFTs) are straightforward to interpret and confirm the clinical impressions of thyroid diseases. However, in some patients, TFT results can be perplexing because the clinical picture is not compatible with the tests or because TSH and free T4 are discordant with each other. Optimizing the interpretation of TFTs requires a complete knowledge of thyroid hormone homeostasis, an understanding of the range of tests available to the clinician, and the ability to interpret biochemical abnormalities in the context of the patient’s clinical thyroid status. The common etiologic factors causing puzzling TFT results include intercurrent illness (sick euthyroid syndrome), drugs, alteration in normal physiology (pregnancy), hypothalamic-pituitary diseases, rare genetic disorders, and assay interference. Sick euthyroid syndrome is the most common cause of TFT abnormalities encountered in the hospital. In hypothalamicpituitary diseases, TSH levels are unreliable. Therefore, it is not uncommon to see marginally high TSH levels in central hypothyroidism. Drugs may be the culprit of TFT abnormalities through various mechanisms. Patients with inappropriate TSH levels need a differential diagnosis between TSH-secreting pituitary adenoma and resistance to thyroid hormone. Sellar magnetic resonance imaging, serum α-subunit levels, serum sex hormone-binding globulin levels, a thyrotropinreleasing hormone stimulation test, trial of somatostatin analogues, and TR-β sequencing are helpful for the diagnosis, but it may be challenging. TFTs should be interpreted based on the clinical context of the patient, not just the numbers and reference ranges of the tests, to avoid various pitfalls of TFTs and unnecessary costly evaluations and therapies.

          Characteristics of Immune-Related Thyroid Adverse Events in Patients Treated with PD-1/PD-L1 Inhibitors

          Background: Thyroid immune-related adverse events (IRAEs) have been reported in patients treated with programmed cell deathprotein-1 (PD-1) and programmed cell death protein-ligand 1 (PD-L1) inhibitors. We investigated the incidence and clinical courseof PD-1/PD-L1 inhibitor-induced thyroid IRAEs, and identified predictable clinical risk factors of thyroid IRAEs, in particular, overthypothyroidism (OH). Methods: We retrospectively reviewed the medical records of 325 cancer patients receiving PD-1/PD-L1 inhibitor in a tertiary referral center. Results: A total of 50.5% (164/325) of patients experienced at least one abnormal thyroid function following PD-1/PD-L1 inhibitor. Eighty-four patients (51.2%) of them recovered to normal thyroid function during follow-up. In overall population, 25 patients(7.7%) required thyroid hormone replacement therapy due to PD-1/PD-L1 inhibitor-induced OH. Patients who progressed to OHshowed significantly higher baseline thyroid stimulating hormone level and longer duration of PD-1/PD-L1 inhibitor therapy thanthose without thyroid dysfunction or OH (both P<0.001). Median time interval to the development of OH was 3 months after thetherapy. OH was significantly associated with positive anti-thyroid peroxidase antibody at baseline and anti-thyroglobulin antibodyduring the therapy than those without thyroid dysfunction or OH (P=0.015 and P=0.005, respectively). We observed no patientswith OH who were able to stop levothyroxine replacement after the cessation of PD-1/PD-L1 inhibitor therapy. Conclusion: PD-1/PD-L1 inhibitor-induced thyroid dysfunctions are considerably reversible; however, OH is irreversible requiringlevothyroxine replacement even after stopping the therapy. Positive thyroid autoantibodies may predict the progression to OH.

          Cafes in the contemporary society have diverse roles for urban everyday life. Actually in the past too, cafe was one of the important social gathering spaces in a city. Not only drinking coffees, but cafes also have various other functions such as meeting, appointment, selling & buying, exhibition. In the past, these diverse functions were minor compared to the main activity of the cafe that is drinking coffee. In the contemporary society, everyday life has so much various urban, social, economical and cultural activities. On the basis of this change, present cafe design has been developed into much different methods and styles. One of the crucial elements is "Program Mix" in which coffee house is mixed with exhibition, book, shop as well as hospital programs. This paper aims to find the unique characteristics of this changed mixed programs and design methods in the 4 case cafe projects. There are interesting several types of mixing programs and space compositions. It is concluded that the case cafes present new role of coffee house as cultural space.

          Identification of Lactobacillus ruminus SPM0211 Isolated from Healthy Koreans and Its Antimicrobial Activity against Some Pathogens

          The intestinal microbiota are important to the host with regard to resistance they impart against bacterial infections and their involvement in mediating metabolic functions. Lactic acid producing bacteria such as Lactobacillus play an important physiological role in these matters. The aim of the present study was to isolate Lactobacillus sp. that inhibits enteric pathogens. Initially, 17 isolates from healthy Koreans were collected on Lactobacillus selective medium. Resistance of the isolates to antibiotics including rifampicin, streptomycin, clindamycin and vancomycin was measured. One of the isolate was identified as Lactobacillus ruminus on the basis of bacterial cell morphology, cultural characteristic and biochemical characteristics, 16S rRNA sequence analysis and PCR-RAPD. Antimicrobial activity of the bacterium against Vancomycin Intermediate Resistant Staphylococcus aureus (VISA) and Vancomycin-Resistant Enterococci (VRE) was measured. About 104 cells of VISA or VRE were mixed with 1, 5, and 9 mL of L. ruminus SPM 0211 and the final volume was adjusted to 10 mL with brain heart infusion (BHI) broth. The cell suspension was incubated for 3, 6, 9, and 24 h, serially diluted and then plated on BHI agar plates. As numbers of L. ruminus SPM 0211 were increased, viable cell count of VISA and VRE decreased. The strongest antimicrobial activity of SPM 0211 was observed after 9 h incubation in any mixture, almost completely inhibiting the growth of these two bacteria. The results suggest that the freshly isolated L. ruminus SPM 0211 may be used as a pro-biotic microbe that prevents the colonization of enteric pathogens and can thereby promote good gastrointestinal health.
