      • 전이 직장암 환자의 FOLFIRI 유발 설사에 위령탕 가감방 치험 1례

        Objectives : This study was aimed to describe a case of a patient with FOLFIRI-induced diarrhea, which was improved by treatment with Korean herbal medicine, modified Wiryeong-tang. Methods : The patient with metastatic rectal cancer recieved FOLFIRI plus bevacizumab as a palliative chemotherapy and showed repetitive diarrhea despite administration of loperamide, which was treated with modified Wiryeong-tang. To evaluate the effect of the treatment, the Bristol Stool Form Scale and bowel movement frequency were checked. Results : During the treatment, the stool form using Bristol Stool Form Scale was improved from diarrhea to normal stool, and the bowel movement frequency was decreased. Conclusions : The result suggest that modified Wiryeong-tang may be an effective treatment for FOLFIRI-induced diarrhea.

      • 고형암 환자의 수술 후 통증에 대한 침 치료 효과 : 체계적 문헌고찰 및 메타분석

        Objectives: Postoperative pain is one of postoperative complications in patients with solid tumor. This systematic review and meta-analysis were aimed to evaluate the efficacy of acupuncture for postoperative pain in solid tumor. Methods: We searched for randomized controlled trials (RCTs) using acupuncture for postoperative pain in adult patients with solid tumor up to September 2016. Acupuncture was defined as manual acupuncture, electroacupuncture and pharmacopuncture. The following databases were searched: PubMed, EMBASE, Cochrane Library, CNKI, CiNii, KoreaMed, Kmbase, KISS, NDSL, KISTI. The results of the studies were meta-analyzed and the risk of bias was assessed. Results: Five studies were included in this review. When acupuncture was compared with usual care, Prince-Henry pain scale score was significantly lower in acupuncture group (MD=-0.44, 95% CI: -0.62 to -0.26, P<0.001, $I^2=87%$) and 10 points pain score (including Numeric Rating Scale and Visual Analog Scale) was lower in acupuncture group but not significantly (MD=-1.00, 95% CI: -2.00 to -0.00, p=0.05). When acupuncture was compared with sham acupuncture, 10 points pain score was significantly lower in acupuncture group (MD=-0.39, 95% CI: -0.65 to -0.14, p=0.002, $I^2=0%$). Any serious adverse events were not reported. Conclusion: This review shows that acupuncture may be considered for postoperative pain in patients with solid tumor without serious adverse events. However, only a few studies were included in this study, further investigation is needed in this area.

      • 전이암 환자에서 단일기관 영양검색 도구의 예후 가치

        Objectives : We investigated whether a single center nutrition screening tool (Kyunghee Neo Nutrition Risk Screening, KNNRS) can predict survival in patients with metastatic cancer. Methods : We retrospectively reviewed data of inpatients with metastatic cancer from April 2016 to August 2019. Data on demographic and clinical parameters were collected from electronic medical records, and overall survival was estimated using the Kaplan-Meier method. Stepwise Cox regression analysis was used to determine factors associated with survival. Patients with a KNNRS score of 0 to 3 were classified as "no-risk", 4 to 10 as "low-risk", and 11 to 20 as "high-risk". Results : Total 105 patients were included in the study. According to nutritional screening at baseline, 25 patients (23.8%, median age 57.0) were classified as ""no risk"" group; 80 patients (76.2%, median age 68.5) as "low risk" group; No patients as "high risk" group. Predictors of survival were Eastern Cooperative Oncology Group Performance Status score of 3 or 4 (hazard ratio [HR] = 1.93; 95% confidence interval [CI] = 1.21-3.10), hemoglobin less than 10 g/dL (HR = 1.97; 95% CI = 1.25-3.10) and C-reactive protein more than 1.0 mg/dL (HR = 1.95; 95% CI = 1.21-3.13). Kaplan-Meier survival analysis showed significant differences in the survival between KNNRS groups: ""no risk"" group: 6.1 ± 1.4 months (95% CI = 3.37-8.83); ""low risk"" group: 3.4 ± 0.9 months (95% CI = 1.5-5.37). Conclusions : Nutritional status according to KNNRS wasn't significant predictor of survival for patients with metastatic cancer. Improvement of KNNRS score thresholds is needed.

        전이암 환자의 예후 판단 인자로서 생체전기 임피던스 분석을 이용한 위상각의 역할

        Objectives: The aim of this study was to identify the validity of phase angle as a prognostic factor for survival in patients with metastatic cancer. Methods: Data of patients with metastatic cancer who visited the Korean medicine cancer center at Kyung Hee University Hospital in Gangdong from April 2016 to February 2018 were collected for this study. The values of phase angle (PhA) and blood hemoglobin (Hb), C-reactive protein (CRP), and serum albumin levels were also investigated by bioelectrical impedance analysis (BIA). Results: In total, 86 patients were analyzed. The cut-off value of the phase angle was determined as 5.0. Univariate and multivariate analysis revealed that phase angle (HR 3.15, 95% CI 1.79-5.54) and CRP (HR 2.15, 95% CI 1.24-3.73) predicted survival with statistical significance. The median overall survival of the low PhA group ($PhA{\leq}5.0$) was 1.8 months (95% CI 0.6-2.9 months) and 7.2 months (95% CI 5.2-9.2 months) in the high PhA group (PhA>5.0)(p<0.001). Conclusions: The phase angle could be an independent prognostic factor for patients with metastatic cancer. Further research is required to confirm these findings and their correlation with other indexes.

        The objectives of this study are investigate the satisfaction index and determined the characteristics of visitors. Also present the issues and direction for development of rural-experience village. This study area is seven villages of rural experience in Cheongwongun, Chungcheongbukdo. This place consists rural cooperative tourism model project that is pursued to Cooperative Tourism Model Project by the government. The projects are forming a network of towns and themes to be exhumed. The purpose of this project inner improves the ability of residents and external to contents diversification. This polling in the February-March 2008 was conducted between visitor 330 people. The question is personal information, satisfaction and rural tourism experience of detail as part of a total of 22.

      • 농촌체험마을 계획수립을 위한 지역유형구분 분석

        녹색농촌체험마을은 종래의 농가의 경제성을 제고하기 위한, 생산 · 판매 중심에서 벗어나 자연자원과 문화체험을 기반으로 한 그린투어리즘으로 도입되었다. 그러나 사업의 시행과정에서 농촌지역의 지역적 자원의 집합체인 어메니티를 고려하지 않고 있어, 그 지역 농촌의 자료를 이용한 개발 방향의 설정이 중요한 사항으로 부각되고 있다. 이에 따라 농촌지역의 자료를 분석하여 그 결과를 응용 할 수 있는 분석기법의 정립이 필요하다 본 논문에서는 농촌 지역의 기존의 자료를 바탕으로 농촌을 유형화하여, 농촌지역의 종합계획에 필요한 객관적인 기초 자료를 제공하고, 이를 위해 다변량분석기법인 주성분분석법과 군집분석을 이용하고, 지리정보를 활용하여 지리적 고찰을 통해 최종적인 농촌지역유형 구분을 하였다. 따라서, 객관화되기 어려운 농촌의 지역자원을 농촌지역의 개발방향으로 제안할 수 있을 것으로 기대된다. 연구의 범위로, 대상지역은 충청북도 면단위로 하였고, 농촌지역의 국가통계 자료를 이용하였다. 농촌지역의 기존 자료를 바탕으로 농촌체험마을 계획수립을 위한 농촌지역유형구분 방법을 정립 하였다. The mainly concept of the green rural experience village acentric from production and sale, because it is based on the natural resources and traditional culture-experience. But at the implementation of these projects, some basic items are not considered that amenity, natural conditions, and traditional cultures of the village. So at the step of the establishment of developing, the analytical results from the data of a village is embossed to important items. However, the evaluation methodology of rural village need for the applying to localization of green tourism. In this study, the methods of the classification of rural village with existing data are studied, the results looking for applying to the making of principal viewpoint of the development. The analysis methods of classification are used the PCA(Principal Components Analysis), Clustering Analysis and combination of these, and making the revised method for localization of the rural village. In this study, we implement classification of regional pattern analysis for the planning of rural experience village for all eub/ myon in Chungcheongbuk-province.

      • 수리시설물 통합설계시스템의 구현

        The irrigation facilities are managed systematically and a facility is independent at the view of shape and function. The design of the irrigation facility is the formulated process with independent objectives. So its process is accordance to object-oriented concept. The design of irrigation facilities is classified into several steps. In these steps, the design data is made and the various problems are solved and consolidated with analysis and decision. In order to achieve our goal of constructing an efficient integrated design system, the results from the design should be able to be systematically connected and re-used. In the study, the design task of irrigation facility, the integrated design system for (IDSAF) is to be developed using the object-oriented methods, and then its applicability will be examined.

        Agricultural irrigation facilities are designed to storage, carriage, distribution and drainage water. Although those facilities may have different primary purposes, their technical functions are interrelated systematically. This Study aimed at developing the optimized object for integrated design for irrigation facilities. of reservoir and channel facilities, which are related by irrigation system design. So, total 17 elementary facility object was developed.

      • 수리시설물의 기본객체 추출과 MPC 모델을 이용한 객체 구현

        In the irrigation facilities, the irrigation system is connected systematically, and thus, it agrees to the object-oriented concept. Since it is necessary to go through comparative evaluations and to devise several alternatives plans in designing the irrigation system, it will be very efficient to use the objects that contain the design data. In this study, the object-oriented methodology has been proposed to define the objects, which will be used in the design system of irrigation facility. Furthermore, as for the essential elements of the objects, concept of element objects is formulated. By employing this concept, appropriate element objects have been derided for the irrigation facility. Necessary data model for realization of the objects is examined and selected. And then, required elements for applying the selected data model to the irrigation facility will be proposed
