확장성 심근증 환아에서의 부분 심실 절제술의 적용 -1례 보고-

심한 울혈성 심부전으로 Dopamine을 1개월간 사용해온 10세의 확장성 심근증 환아에서 심장 이식을 시행하려 하였으나, 적절한 크기의 공여 심장이 없었고, 환아의 증상이 악화되어 부분 심실 절제술과 승모판 성형술을 시행하였다. 수술후 환아의 임상 증상은 현저히 호전되었고 수술전과 수술후 3개월, 6개월 1년째의 심장 초음파 검사 소견을 비교하여 추적 관찰한 결과, 좌심실의 심박출 계수는 수술전 17 %에서 수술후 각각 29%, 35%, 36%로 개선되었고 좌심실 확장기말 직경은 수술전 72 mm에서 수술후 각각 59 mm, 61 mm, 61 mm로 변화를 보였다. 부분 좌심실 절제술과 승모판 성형술의 동반 시행은 심장 이식의 기회가 상대적으로 적은 소아의 확장성 심근증에 있어 심장의 부하를 감소시켜 증상 완화를 시킬 수 있는 외과적 방법으로 심장 이식을 시행할 때까지 유지시켜주는 고식적 가치가 크며 아울러 장기적 추적 관찰이 필요하다고 사료 된다. Heart transplantation was planned for a 10-year old boy who had dilated cardiomyopathy with severe congestive heart failure and had been on dopamine for 1month. However, partial left ventriculectomy and mitral annuloplasty were performed instead, because there was no donor heart of the adequate size and the symptoms were aggravated. The clinical symptoms were markedly improved after the surgery. Comparing the postoperative echocardiographic results with the preoperative results, there were remarkable changes in the left ventricular ejection fraction(preoperative LV EF 17% to postoperative 3 months 29%, 6 months 35%, 1 year 36%) and the left ventricular end-diastolic dimension(preoperative 72 mm to postoperative 3 months 59 mm, 6 months 61 mm, 1 year 61 mm). Partial left ventriculectomy and mitral annuloplasty reduced the cardiac loading in the dilated cardiomyopathy. Partial left ventriculectomy and mitral annuloplasty may be considered as one of the alternative surgical metho s to carry over until a heart transplantation can be performed, especially for children.

현재 인터넷 상에서 가장 많이 제공되고 사용 되는 서비스는 전자 상거래에 관련된 서비스이다. 이러한 서비스에서 각 사용자 사이의 신원을 증명해 줄 인증서는 절대적인 위치를 차지하게 된다. 따라서 전자 상거래의 성격에 비추어 그 인증서는 항상 적시에 유지 관리 되어야 하며, 그 상태가 매 순간마다 정확하게 변경되어야 할 것이다. 그리고 그 보안성도 높아야 한다. 이러한 요구사항을 만족하기 위해서 많은 연구가 진행되고 있다. 본 논문에서는 이를 현재 인터넷 환경에서 반드시 사용되어야 하는 DNS 를 이용하여 해결 하는 방법을 제안하고자 한다. 자원레코드의 한 속성을 사용하여 인증서를 배포하고, 그를 검증하여 안전하고 적시 적용이 가능하도록 한다.

한,일 TPM 활동에서 Presentation에 관한 연구

This study concerns with the presentation of Korean and Japanese TPM activity during the last 6 years Korea-Japan TPM meeting in Seoul. This paper is aimed to compare the Korean TPM activity presentation with the that of Japan. The procedures of this paper are summerized as follows. First, for the successful presentation they should know well the effective presentation methods. Second, TPM activity should be propelled step by step on the base of long prospect and schedule. Third, the presentation business ratio of manufacturing and process is almost 1 to 1 both in Korea side and Japan side. Forth, the management concept of Korean business is transfering from the type of 'problem solution' to the type of 'ideal concept plus poblem solution', but the that of Japan's from the type of 'ideal concept plus poblem solution' to the 'ideal concept type'. And Korea and Japan also consider much of business vision and innovation. Especially, Japanese business present much that employees are highly thought. Fifth, the goals of TPM activity are proposed more concrete in Japanese presentation than that of Korea. Finally, most of Korean business are on the level of 7 step of TPM activity, but the most Japanese business are above that of 10 step.
