        제거한 하안검 지방의 이식을 이용한 눈물받이 고랑의 교정

        Lower palpebral bags and tear trough grooves are the common signs of aging. Weakness of the orbital septum, atrophy of the orbicularis oculi muscle, slackness and laxity of the lower lid skin are the main causes of these aging changes. Tear trough groove with skeletonization of the lower orbital rim can not be corrected easily by the conventional lower blepharoplasty, but the augmentation of the volume, can be an effective method. Lower lids provide a well vascularized recipient site, which is vital for proper grafting of fat to occur. We treated tear trough groove with free fat graft of surgically removed orbital fat. Surgical excision of the fat pad and septal reinforcement with free fat graft below the levator labii superioris alaeque nasi muscle were performed to correct tear trough groove. This technique was performed in 29 consecutive individuals from October, 2001 to January, 2005. Palpebral bags and tear trough groove were corrected concomitantly and obtained a youthful attractive midface with no secondary hollowness of lower lid and irregularity. The overall results obtained were satisfactory without any complications. Recurrence of tear trough deformity due to grafted fat absorption was not noted during the 7 months of the follow-up period. The authors conclude that the procedure is safe and effective in selected patients.

        2018 평창 동계올림픽 민간안전요원제도의 문제점과 개선방안에 관한 연구

        이 연구는 2018 평창 동계올림픽에 처음 도입된 민간안전요원 제도의 문제점 을 파악하여 개선방안을 모색하였다. 2018 평창 동계올림픽에 참여한 민간안전 요원들을 대상으로 실시한 조사 결과 보안활동에서 부실한 보안 교육과 자연재 해 대비의 소홀이라는 문제점이 나타났다. 민간안전요원으로 근무하면서 나타난 민간안전요원 제도의 문제점은 열악한 근무환경, 수당 문제, 노로 바이러스 발생 에 대한 초동조치 미흡, 대규모 중도 퇴사자 발생과 이에 따른 인원 보충의 부 족, 팀장들의 폭언, 욕설, 성희롱 등으로 나타났다. 2018 평창 올림픽에서 나타난 보안 사고와 민간안전요원 제도의 문제점을 해 결하기 위해서 보안 교육의 내실화, 개최 지역의 자연현상 대비, 선발과 과정에 충분한 예산과 인력 준비, 전문가 참여, 보안에 대한 사전 지식이나 경험이 있는 인력들 우선 선발 등이 필요하다. 또한 향후 국제행사에는 개최 장소의 자연과 계절 특성에 대한 철저한 대비가 필요하다. 민간안전요원 제도를 개선하기 위해 서 열악한 근무환경 개선, 「근로기준법」 준수, 질병, 감염 등에 대한 초동조치 철저, 중도 퇴사자 예방, 적절한 인원 보충, 민간안전요원들의 직무만족도 향상 방안 강구, 관리자들에 대한 철저한 교육과 감독 등의 방안 필요하다. This study investigated the problems of the private security personnel system first introduced at the 2018 PyeongChang Winter Olympics and sought to improve the situation. A survey conducted by the private security personnel participating in the 2018 PyeongChang Winter Olympic Games revealed that the security activities were insufficient in terms of security education and neglect of natural disasters. The problem with the system were appeared as a poor working environment, payment problem, lack of initial action to the occurrence of the Norovirus and supplementation to the large-scale mid-term retirement. Team leader's abuse and sexual harassment was also can be found out. To solve the problems that appeared in 2018 Pyeongchang Olympic Games and problems of private safety personnel system, strengthening of security education, preparation of the natural phenomenon in the region has to considered. Furthermore, preparation of sufficient budget, expert participation, advance knowledge or experience in security should be considered for the selection process. It is also necessary to thoroughly prepare the nature and seasonal characteristics of the place for future international events. In order to improve the private safety personnel system, it is necessary to improve the poor working environment, comply with the “Labor Standards Act”. To improve Initial actions for diseases and infections, prevent mid-term retirement, supplement for the appropriate personnel, and job satisfaction of the private safety personnel, thorough education and supervision is necessary.

        Expanded Polytetrafluoroethylene(Gore-tex?)을 이용한 안면부 미용수술의 경험

        To date, for facial soft tissue augmentation, numerous implant materials have been used, including autogenous, homogenous, and alloplastic grafts. Among these, the porous material, Gore-tex has a pore (0.5-30 microns, average 22 microns) which allows sufficient but limited soft tissue ingrowth to anchor implants, but not so much to make removal difficult or destructive to adjacent structures. In addition, this material has some advantage: non-carcinogenic, non-allergic, less capsule formation and reduced chance of infection due to high vascularity. In the past years Gore-tex has found multiple applications in facial plastic and reconstructive surgery. However this alloplastic material can be associated with potentially severe complications and side effects such as soft tissue reaction, infection, extrusion, exposure, etc. There have been also adverse criticism of Gore-tex: relative difficulties in postoperative removal, in carving implant, higher infection and extrusion rates in scarred tissue and thicker implant as well as postoperative volume reduction. In order to analyze results depending on the implantation site, the presence of scar on recipient site, thickness and type of implant, a retrospective chart review of 56 patients(69 cases) was undertaken of all patients who underwent facial soft tissue augmentation with Gore-tex graft from November 1998 to November 2001 in a wide variety of situations. All cases were categorized as either primary(normal recipient site) or secondary(scarred recipient site) cases and the thickness of each graft used in millimeters was recorded. Postoperative follow up revealed a stable implant material with no major complications relating to the graft material and no differences in developing complications according to the implantation site, the presence of scar, thickness and type of implant. All patients were highly satisfied with their results.

        전자의무기록시스템과 코스모스 블록체인에 관한 연구

        This study identified problems with the counterfeit and modulation of the electronic medical record system and sought improvement measures. The electronic medical records system collectively refers to a system that input, arrangement, and storages work data or medical care, surgery, and examination records based on the calculation of patients' medical practices. Therefore, medical institutions are able to provide consistent treatment to patients as a value for their business use and are able to use it as legal evidence in the event of a medical dispute as a value for supporting internal and external accountability. However, it is necessary to take countermeasures against electronic records as they are frequently manipulated internally and externally. Block chains are currently being introduced in various fields and have been introduced in the medical field, but the level of adoption in the electronic medical record system is insufficient. It is urgent to study and introduce block chains, such as Cosmos block chains, which are compatible with other block chains at a time when block chains are absolutely necessary to ensure the integrity and security of records by preventing forgery and modulation. 이 연구는 전자의무기록시스템의 위·변조에 대한 문제점을 해결하기 위해 도입되고 있는 블록체인의 문제점을 분석하고 개선방안을 모색하였다. 전자의무기록시스템은환자의 진료행위를 중심으로발생한 업무상의자료나 진료 및수술·검사 기록을 전산에 기반하여 입력·정리·보관하는 시스템을 통칭한다. 그렇기에 의료기관에서 업무적 활용가치로서 환자에게 일관성 있는 치료를 제공할 수 있는 근거자료가 되며 대내외적 설명 책임의무를 지원하는 가치로서는 의료분쟁 시 법적 증거로활용 가능하다. 하지만 내·외적으로 전자의무기록에 대한 위·변조가 빈번하게 이루어지면서 이에 대한 대응방안으로 블록체인 도입이 대두되고 있다. 현재 다양한 분야에서 블록체인이 도입되고 있으며 의료분야에서도 도입이 이루어지고 있으나 전자의무기록시스템에 도입된 수준은 미비하다. 위·변조를 방지하여 기록의 무결성과 보안성까지 보장해줄 수 있는 블록체인이 절대적으로 필요한 이 시점에서 다른블록체인과 호환이 가능한 코스모스 블록체인과 같은 블록체인에 대한 연구와 도입 마련이 시급하다.

        흉벽주변 근판을 이용한 급성 흉골-종격동염의 치료

        Although the incidence of mediastinal wound infection after median sternotomy is relatively low, its associated devastating complications may be life-threatening. Treatment of sterno-mediastinal infection has evolved over the past several decades. Early treatment was wet soaking of debrided wound or irrigation through a closed indwelling catheter system and its mortality was quite high. Subsequent treatments focused on the debridement followed by muscle flaps such as pectoralis major and rectus abdominis muscle flap or by omental flap closure, which were considered as the ideal modalities for acute sterno-mediastinitis. Mortality rate was reduced significantly after application of this active treatment. However, to date, there has been no definite treatment strategy for sterno- mediastinitis and the choice of operations depends largely on the experience and personal preference of the surgeon. So we introduce our experience, including technique and outcome, with transposition of pectoralis major muscle and superiorly- based rectus abdominis muscle turn over flap for deep and extensive sterno-mediastinal wounds. Five patients(3 male and 2 female) had repair of an infected median sternotomy wound from July 2001 to september 2002. Complications after wound closure occured in 2 cases, 1 case of hematoma and 1 case of seroma, respectively. No other major complications have been noted. We treated MRSA or VRE- cultured acute sterno-mediastinitis successfully with early sternal wound debridement and immediate regional muscle flap coverage.

      • 성형외과 영역에서 Nd;YAG contact laser의 다양한 유용성

        Lasers have introduced new therapeutic modalities in medicine. A wide variety of lasers provides plastic surgeon with variable treatment options that were not available previously. The 1062㎚ Nd;YAG contact laser system was used to treat 26 patients (34 cases) during period of March 1997 to July 1998. We used different setting of power rate in continuous wave mode for each purpose. The power setting of 1-2W was used for the laserbrasion because of good melting and coagulation effect. A 6-8W was selected for flap dissection and incision because of good cutting ability and excellent hemostasis. The handpiece was equipped with a frosted end contact laser scalpel made of synthetic sapphire crystal. This scalpel has a wavelength conversion infrared absorbing surface treatment on it's distal end ,which limits the depth of penetration to less than 0.5㎜ and increases the temperature gradient of the tissue. When we used Nd;YAG contact laser, the advantage were as the follows. In laserbrasion by the Nd;YAG contact laser, smokeless and bloodless field was possible. And penetration depth can be accurately controlled. It was easy and quick technique, so postoperative discomfort was minimal. In flap dissection and incision, penetration depth can be accurately controlled and there was-minimized bleeding, so visualization was excellent. Nd;YAG contact laser has good cutting ability with only a little heat damage of adjacent tissue, so precise dissection can be possible. The contact laser technique combines the advantages of both the scalpel and electrocautery. In conclusion, Nd;YAG contact laser system produces clinically favorable therapeutic effects in patients with epidermal or upper dermal lesions as well as flap dissection and incision.

        데이터 분석을 통한 한국산업보안연구 발전전략 수립 연구

        With the fourth industrial revolution and the spread of new COVID-19 infections, key bio technologies, including science and technology, are drawing global attention. Amid this interest, the importance of industrial security is emerging as competition for technological supremacy accelerates. Recognizing the importance of industrial security in Korea, the Department of Industrial Security was established at Chung-Ang University and Dankook University starting from Hanse University, and related projects under the jurisdiction of the government were conducted, and various seminars and academic conferences were held. As such, the business related to industrial security is gradually expanding, but there is a lack of research on industrial security itself academically. Therefore, all research papers published from the issue of publication until recently were systematically analyzed for the development of industrial security and the development of the Korean Industrial Security Research Journal. In particular, the papers that have been published in academic journals have been grouped into detailed fields in law/system, crime, technology, management, convergence and other fields, linking them to the future direction of development. In addition, a network tool called Gephi was used to network and visualize them by subject language and author. Therefore, this study is meaningful in that it presents the direction of further research inindustrial security and contributes to the vitalization of industrial security. 기술적·국제적 우위에 서기 위한 국가 간 기술 패권 경쟁이 가속화되면서 산업보안의 중요성이 대두되고 있다. 국내에서는 산업보안의 중요성을 인식하여 한세대학교를 기점으로 중앙대학교, 단국대학교 등에서 산업보안학과가 설립되었으며 정부 소관의 관련 사업 등이 실시되고 각종 세미나 및 학술대회가 개최되었다. 이처럼 산업보안과 관련된 사업은 점차 확대되고 있는 추세이나, 학술적인 관점에서 산업보안 분야의 정체성 확립과 연구 활성화를 위한 노력은 아직 부족한 것이 현실이다. 따라서 본 연구에서는 산업보안 분야의 대표 학술지인 한국산업보안연구의 발전을 위해 과거의 논문 게재 데이터를 바탕으로 발전방안을 제시한다. 이를 위해 한국학술지인용색인(www.kci.go.kr) 사이트에서 제공하는 분석 정보 서비스와 한국산업보안연구의 2009년 학술지 발간호부터 ‘2020 온라인 학술대회’가 개최되기 이전인 2020년 8월까지 게재된 모든연구 논문들을 체계적으로 분석하였다. 특히 그동안 학술지에 게재되었던 논문들을 법/제도 분야, 범죄 분야, 기술 분야, 관리 분야, 융합 분야, 기타 분야로 세부 분야로 분류하면서 앞으로의 발전 방향과 연결 지었고 네트워크 분석 툴(Gephi)를 사용하여 주제어와 저자별로 네트워크 하여 시각화하였다. 따라서 본 연구는 산업보안의 추후 연구 방향을 제시하고, 산업보안 활성화에 기여한다는 점에서 의의가 있다.
