        화농성 무릎관절염의 수술 후 관절강직에 대한 한의복합치료: 증례보고

        A 50-year-old Korean male tour guide without any medical or family history complained of left knee pain. After receiving umbilical cord blood injection treatment, the pain gradually worsened. After being diagnosed with septic arthritis in the left knee, arthroscopic lavage, debridement, antibiotic treatment, and routine rehabilitation therapy were performed, but the symptoms persisted. In the hospital, acupuncture, pharmacopuncture, acupotomy, Chuna manual therapy, and cupping therapy were performed in addition to the usual treatment for 59 days. To evaluate the patient’s improvement, the numeric rating scale, EuroQol 5-dimension, pain disability index, and Lysholm knee scoring system were used. After treatment, the symptoms improved in all assessment tools, swelling, and range of motion of the joint. Through this study, it was found that complex Korean medical therapies may be effective for postoperative ankylosis in septic arthritis of the knee, and further studies are needed to clarify the therapeutic effect.

        Zerumbone 처리 헬리코박터 파이로리균의 전사체 분석 비교

        본 연구에서는 제럼본에 처리에 의해 유도된 H. pylori 유전자의 전사적 변화를 분석하였다. NGS를 사용하여 RNA 발현 변화를 분석한 다음 그 결과를 검증하기 위해 RT-PCR을 수행하였다. NGS 분석 결과, 1,632개의 유전자 중 총 23개가 제럼본 처리에 의해 유의하게 발현이 변화된 특이발현 유전자로 분석되었다. DNA 복제와 전사, 병원성 인자 및 T4SS 성분과 관련된 유전자 중 10개는 현저하게 하향 조절되었고 5개는 상향 조절되었다. RT-PCR을 이용하여 유전자의 발현 수준을 재확인하였고 그 결과, 14개 유전자에서 NGS와 동일하게 발현 양상이 변화하였다. RT-PCR은 제럼본 처리에 의해 10개의 유전자(dnaE, dnaQ, rpoA, rpoD, secA, flgE, flhA, virB5, virB8, virB9)의 발현 감소와 4개의 유전자(flaA, flaB, virB4, virD4)의 발현 증가를 보였다. 이러한 본 연구의 결과는 제럼본이 다양한 H. pylori의 병원성과 관련된 인자들을 조절함으로써 H. pylori 감염의 잠재적인 치료제가 될 수 있음을 시사한다. Helicobacter pylori (H. pylori) establishes infection in the human gastric mucosa for a long time and causes severe gastric diseases such as peptic ulcer and gastric cancer. When H. pylori is exposed to the antibacterial agents or inhibitors, the expression of pathogenic associated genes could be altered. In this study, we analyzed the transcriptional changes of H. pylori genes induced by zerumbone treatment. RNA expression changes were analyzed using next-generation sequencing (NGS), and then reverse transcription-polymerase chain reaction (RT-PCR) was performed to verify the results. As a result of NGS analysis, a total of 23 out of 1,632 genes were differentially expressed by zerumbone treatment. RT-PCR confirmed that zerumbone treatment regulated the expression level of 14 genes. Among the genes associated with DNA replication, transcription, virulence factors and T4SS components, 10 genes (dnaE, dnaQ, rpoA, rpoD, secA, flgE, flhA, virB5, virB8 and virB9) were significantly down-regulated and 4 genes (flaA, flaB, virB4 and virD4) were up-regulated. The results of our current study imply that zerumbone might be a potential therapeutic agent for H. pylori infection by regulating factors related to various H. pylori pathogenicity.

        실리카흄을 이용한 경량기포콘크리트의 역학적 특성과 성토재로서의 적용성에 관한 연구

        경량기포콘크리트는 경제성 및 시공의 용이성으로 인해 건축물과 성토체로 널리 사용되고 있는 재료이다. 그러나 토목분야에서는 널리 사용되지 않고 있다. 경량기포콘크리트의 재료적인 특성을 파악하기 위한 수많은 연구가 실행되었음에도 불구하고 여전히 미흡한 실정이다. 본 연구에서는 성토재로 사용하기 위한 경량기포콘크리트의 재료적 특성을 파악하고자 실험을 하였다. 물, 모래, 실리카흄의 혼합비를 달리하여 압축강도를 측정하여 서로 비교해보았다. 또한 연구결과를 통해 성토재의 최적배합비 산정과 연구결과를 이용하여 압축강도를 정의하였으며, 설계 시방을 통해 성토재로서의 경량기포콘크리트 적용성을 검증하였다. Lightweight Foamed Concrete is one of the most-used materials for insulation of building and embankment. Since it is economic and workability of construction. However, it is not widely used in civil engineering area. Although several researches were performed to investigate material properties of light weight foamed concrete, it is still remained in unexplored field. In this research, laboratory tests were performed to figure out the material properties of light weight foamed concrete to use embankment. The compressive strengths varying with mixing ratio with water and sand as well as silica fume were compared each other. According to research results, the optimum mixing ratio to use embankment material is suggested and the compressive strength using the results is also defined. The adaptability of light weight foamed concrete as an embankment material is checked with design spec and verified.

        대구지역 일개 대학병원에서 분리된 Acinetobacter baumannii의 항균제 내성현황과 분자역학적 특성

        Acinetobacter baumannii is a gram-negative bacterium that causes nosocomial infections, such as ventilator-associated pneumonia, bacteremia, and urinary tract infections, particularly in immune-compromised patients. A. baumannii has a remarkable capacity to acquire mechanisms that confer resistance to various types of antibiotics. Therefore, rapid and sensitive molecular detection in clinical isolates is greatly needed for the appropriate therapy and outbreak control of Multi drug resistant A. baumannii (MDRAB). This study was conducted for the purpose of a basic study on the current status of antimicrobial resistance and types of resistance mechanisms in MDRAB by confirming the resistance patterns and distribution of the major antimicrobial resistance genes of A. baumannii isolated from a university hospital in Daegu City. All 512 isolates carried blaOXA-23 and blaOXA-51 genes, showing resistance to carbapenems. In addition, resistance genes to aminoglycosides and 16S rRNA methylase were retained at high frequency. Sequencing analysis demonstrated that the resistance to Quinolones was acquired by amino acid substitution due to point mutation occurring at position 83 (Ser83) of gyrA and position 80 (Ser80) of parC. In the REP-PCR profile, all isolates showed band patterns similar to each other and were analyzed to belong to a common cluster by dendrogram. This suggests that A. baumannii, which originated from a common ancestor, has spread clonally within hospitals or communities. Thus, it is necessary to continuously monitor the antimicrobial resistance rate and resistance gene distribution to establish a system that can detect in hospital transmission or regional spread early.

      • 라벨링 기법을 활용한 병해충 진단 시스템 개발

        본 논문에서는 CNN 알고리즘을 활용한 병해충진단 처방 시스템을 제안한다. 총 11 개층으로 설계하였으며 병해충 한 종류당 하나의 분류 방식을 사용하는 대신 종류당 여러 개의 특징을 학습하도록 설계하였다. 이렇게 분류된 결과를 라벨링 과정을 통해 병해충 단위로 묶는 과정을 수행하게 하였다. 이를 통해 방향에 따른 특징 차이를 보완할 수 있는 수단을 마련하였다.

        척수경막동정맥루로 인한 척수병증 환자의 색전술 이후 한의복합치료: 증례 보고

        A 38-year-old Korean man without any other history was diagnosed with myelopathy due to a spinal dural arteriovenous fistula. Following embolization and high-dose steroid therapy, the patient was treated with complex Korean medical therapies including acupuncture, pharmacopuncture, electro-acupuncture, Chuna manual therapy, herbal medicine, and cupping therapy in addition to conventional treatment. To assess the patient’s improvement, international standard for neurological classification of spinal cord injury, Korean version of modified Barthel index, functional independence measure, spinal cord independence measure III, walking index for spinal cord injury II, modified Ashworth scale were used. After treatment, the muscle strength and sensory function of the lower extremities were improved, and the spasticity was reduced, resulting in a rapid improvement in performance of daily activities. These results suggest that complex Korean medical therapies may be effective for myelopathy, and further clinical studies are needed to clarify their effects.         제럼본의 요소가수분해효소 활성 억제 평가

        요소가수분해효소는 요소를 암모니아와 이산화탄소로 가수분해하는 효소로 요로감염을 일으키는 세균의 주요 병원성 인자이다. 따라서 요소가수분해효소는 세균이 요로감염증을 유발하는데 중요한 역할을 한다. 본 연구에서는 요소가수분해효소를 생성하는 6가지 종류의 세균에 (K. oxytoca, K. pneumoniae, M. morganii, P. mirabilis, P. vulgaris, S. saprophyticus) 대한 제럼본의 억제효과와 요소가수분해효소 활성능을 평가하였다. 최소억제농도와 최소살균농도 실험에서 제럼본은 요소가수분해효소를 생성하는 6가지 종류의 세균에 대해 억제효과를 보였으며, 최소억제농도는 0.5-2 mM, 최소살균농도는 1-4 mM를 나타내었다. 요소가수분해 활성억제 실험에서 제럼본은 요소가수분해효소의 억제제로 사용하는 아세토히드록사민산 보다 뛰어난 요소가수 분해 활성억제효과를 보였다. 그러나 제럼본은 요소가수분해효소를 이루는 소단위체의 발현양에는 영향을 주지 않았다. 이러한 결과들을 종합하여 볼 때, 제럼본은 요소가수분해효소를 생성하는 세균에 대한 살균력을 가질 뿐 만 아니라 훌륭한 요소가수분해 활성억제력도 보유하고 있는 것으로 사료된다. A key virulence factor for urinary tract pathogens is the enzyme urease, which catalyzes the hydrolysis of urea into ammonium ions and carbonic acid. Urease activity plays an important role in the pathogenesis of urinary tract infection. In this study, the inhibitory effect of zerumbone against six urease-producing bacteria (Klebsiella oxytoca, K. pneumoniae, Morganella morganii, Proteus mirabilis, P. vulgaris, and Staphylococcus saprophyticus) and their urease activities were evaluated. The results of the minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) tests showed that zerumbone had antibacterial effect against these six urease-producing bacteria. The MIC and MBC of zerumbone ranged from 0.5 to 2 mM and 1 to 4 mM, respectively. In the urease inhibitory assay, zerumbone showed better urease inhibition (56.28 ± 2.45-37.83 ± 3.47%) than the standard urease inhibitor, acetohydroxamic acid (40.46 ± 1.94 - 22.99 ± 3.53%). However, zerumbone did not affect the levels of the urease subunit. These results clearly indicated that zerumbone has antibacterial potential against urease-producing bacteria and possesses excellent bacterial urease inhibition properties.

      • 이동하중과 3차원 모델링을 통한 접속부 지지강성연구

        The Transitional zone between bridge abutment and earthwork is one of the representative vulnerable zones in railway where differential settlements may take place due to the different supportive stiffness. Although transitional zones are managed with stricter standards than those of the other earthwork zones either in the design and construction stages it is very difficult to prevent differential settlement perfectly. A three-dimensional numerical analyses were performed by applying train moving load in this study. The analytical model including abutments and earthwork zones was constituted with rail sleepers track concrete layer (TCL) hydraulic stabilized base (HSB) reinforced road bed and road bed using railway and road base structure. The clamp connecting the rail and sleeper were also modeled as the element with spring coefficient. The train wheel is modeled in the actual size and moved on the rail with 300 km/hr speed. The deformation characteristics at each point of the rail and the ground were considered in detail when moving the train wheel. The analysis results were compared with those from the two-dimensional analysis without considering moving load. The research results show that displacement and stress were greater in the three-dimensional analysis than in other analyses and the three-dimensional analysis with moving load should be performed to evaluate railway performance.

        근골격계 수술 후 한방재활치료의 효과: 후향적 관찰연구

        Objectives This study aimed to evaluate the effectiveness of postoperative Korean rehabilitation treatment for patients with musculoskeletal disorders. Methods The medical records of patients undergoing Korean medical rehabilitation after a musculoskeletal surgery, from January 1, 2016, to December 31, 2019, were analyzed retrospectively. The effect of treatment was evaluated by using a numeric rating scale (NRS) and questionnaire for each surgical site. When the same patient was re-admitted after discharge, it was treated as the same case if it was within 7 days from the discharge date; otherwise, it was treated separately. If the period of hospitalization was within 7 days or the chief complaint was not related to musculoskeletal surgeries, the case was excluded. Results In total, 480 cases were included in this study. Acupuncture (99.8%), moxibustion (95.6%), cupping (97.5%), Chuna manual therapy (81.5%), Korean medical physiotherapy (61.0%), Daoyin exercise therapy (26.3%), and herbal medicine (87.7%) were used as the Korean medical rehabilitation treatments. A significant improvement was noted when comparing the NRS and questionnaire results for each surgical site at the time of admission and discharge. Conclusions Korean medical rehabilitation can be effectively used for patients who have undergone musculoskeletal surgeries, to relieve pain and enable returning to daily activities. However, further research with a high level of evidence is necessary to support this finding. (J Korean Med Rehabil 2020;30(3):151-162)
