        바이모달 보청기 적합 문헌 고찰

        Bimodal hearing refers to the combination of the cochlear implant in one ear and the hearing aid in the opposite ear. Recently, cochlear implant candidacy criteria have been expanded and the number of people receiving cochlear implants with residual hearing has increased, leading to the raised bimodal population. Providing professional fitting services based on systematic bimodal fitting methods or guidelines are essential for the successful bimodal intervention of the hearing impaired. However, there is a lack of evidence-based bimodal fitting method or guideline clinically applicable. In general, bimodal fitting consists of three areas of fitting; cochlear implant mapping, hearing aid fitting, and bimodal fitting. Among them, establishing evidence-based bimodal hearing aid fitting will be a key factor to improve bimodal fitting services of hearing aid centers. The purpose of this study is to review recent literature related to bimodal hearing aid fitting to establish bimodal hearing aid fitting guidelines for hearing aid professionals. This review involves five bimodal hearing aid fitting issues including frequency response, loudness, dynamic compression, frequency transposition, and hearing aid fitting formula. In addition, it described three recent clinical bimodal hearing aid fitting guidelines provided by manufacturers. Two factors contributing to bimodal hearing aid fitting optimization across the studies are frequency response and loudness balance. Conducting loudness balance in bimodal hearing devices and adjusting gains based on the loudness balance tests are one of the important procedures for bimodal fitting optimization. Hearing aid fitting based on wide-band frequency responses and conventional hearing aid formula is recommended for the initial step of the bimodal fitting.

        인공와우 이식수술 중 신경반응 원격 측정법 검사의 임상적 유용성

        Background and Objectives:Neural response telemetry (NRT) has been used as one of the most useful tools to measureneural responses ranging from spiral ganglion cells to electrical stimulation via the Nucleus 24 cochlear implant. The purpose ofthis study was to assess and analyze the result of NRT measured intraopertively and apply the result in postoperative mapping.Subjects and Method:The NRT test was administered to 72 patients (49 children and 23 adults) with the Nucleus 24 cochlearimplant. After inserting of the electrode array into the scala tympani, an impedance test was administered to check the integrityof each inserted electrode. In cases of partial insertion, the electrodes outside the cochlear were excluded along with thoseproven to be short circuited. The electrically evoked action potential (EAP) was recorded using version 3.0 of NRT software.One intra-cochlear electrode (electrode 2, 4, 6, 8, 10, 12, 14, 16) with MP1 as reference was chosen for the stimulating pair. Therecording electrode was located 3 electrodes away apically from the stimulating intracochlear electrode and referenced to MP2.The amplifier gain and sampling delay were optimized for each subject using electrode 4 and these same parameters were usedthroughout the test. Results:Out of 72 implantees, 56 patients (78%) were observed to produce reliable responses:16 (70%)of 23 adults and 40 (82%) of 49 children responded. The patients who failed to produce NRT response had higher T levels thanpatients who had NRT response. Conclusion:Eighty percent of the implantees without any inner ear anomaly showed reliableresponse to intraopertive NRT measurement. The intra-operarative NRT data may be used to predict electrical stimulation levelsin programming. (Korean J Otolaryngol 2006;49:892-6)

        인공와우를 통한 말지각과 반대편 귀 사용 효과에 대한 최근 연구 고찰

        The goal of cochlear implants restores functional hearing by transmitting electrical pulses to auditory nerves and improves speech perception. Although current cochlear implant users showed greatly improved sentence recognition scores in a quiet environment, the issues of understanding speech in a noisy environment and listening to music are still challenging for them. Limitations of spectral and temporal resolution are thought to be related to these difficulties of cochlear implant users. As one of the approaches to resolve these challenging issues, benefits from the contralateral device of cochlear implant users were introduced. Two different types of binaural placements, bilateral and bimodal hearing, were continuously increased and they demonstrated some amounts of speech perception benefits. Bilateral hearing improves sound localization and speech perception in noise mostly due to the benefits from head shadow and redundancy effects. On the other hand, bimodal benefits are delivered by using acoustic low frequency information which is not represented in electric stimulation via cochlear implants. A review of the literature related to these topics was addressed.

        완선환자 의복내의 원인균 오염상태

        N/A Contamination of Dermatophytes in the clothes of Patients with Tinea Cruris Su Hee Oh, M.D., Soon Bong Suh, M.D., Sung Hwa Kim, M.D., Jae Bok Jun, M.D.* Catholic Skin Disease Clinic, Taegu, Korea Department of Dermatology, Kyungpook National University School of Medicine*, Taegu, Korea Isolation of causative agents from clothes of patients with tinea cruris was carried out for 126 tinea cruris patients with Sabouradu’s cycloheximide-chlorampenicol agar petri dish. One hundred and thirteen strains were cultured from the panties.; They were Trichophyton rubrum in 108 strains and Epidermophyton floccosum in 5 strains. Seventy seven panties were contaminated over a moderate degree. The panties of patients infected by E. floccosum were more contaminated than by T.rubrum. The panties of females infected by T. rubrum were more contaminated than ones males. Briefs were more contaminated than boxer shorts. The panties worn for a longer period were more contaminated, but tests for causative agents were negative within two weeks following regular treatmen. Causative agents were isolated in 70.0~85.0% of the patients from the parts of the abdomen, thigh and lower leg of the patients’trousers, but the degree of contamination was much lower than that of patients’s panties.         인공와우에서 신경반응 원격 측정법 역치와 행동반응 결과의 비교

        Background and Objectives:Electrically evoked compound action potential thresholds recorded using neural response telemetry can be used to set MAP thresholds (T-levels) and maximum comfort levels (C-levels) especially in children implanted with the Nucleus 24 device. However, the thresholds and C-levels change over time and the relationship between neural response telemetry (NRT) measurements and behavioral levels in children may also change. The purpose of this study was to apply the neural response threshold to the behavioral levels at the beginning (up to first 6 months). Subjects and Method:The neural response telemetry test was administered in 23 patients (mean age was 16.8) with Nucleus 24 device. Four intra-cochlear electrodes (electrodes 5, 10, 15, and 20) were tested. The behavioral threshold and the maximum comfort level at immediate, 1, 2, 3 and 6 months post-implantation were compared with the neural response threshold. Results:Among 23 patients, 15 had their NRT thresholds within their dynamic range in the 6 months post-mapping period. Although the time when NRT thresholds comes within the dynamic range may differ from patient to patient, it was common finding that this occurred after 2 months of post-mapping in most of the patients. Conclusion:During the first few months of post-implantation, care must be exercised when interpreting NRTmeasurements. (Korean J Otolaryngol 2006;49:274-8)

        General Framework of Hearing Aid Fitting Management

        Hearing aids are one of the most widely used treatment options for the hearing impaired and optimal outcomes of hearing aids are supported by comprehensive hearing aid fitting protocols. Currently, the term ‘hearing aid fitting’ is prevalently used among service and industry sectors with its comprehensive procedures not systematically explicated. In addition, a variety of non-normalized guidelines for hearing aid fitting has led to non-uniform care, outcome variability, and dissatisfaction of the use of hearing aids. The main purpose of the present study is to suggest a general framework of standardized practice for hearing aid fitting management including its pre- and post-fitting stages. The management framework centers on its fitting process with its prior steps of assessment as well as its posterior steps of followup, thereby eliminating diverging interpretations and non-uniform practices.         한국어판 성격적응척도(K-JPAQ:Korean Joines Personality Adaptation Questionnaire)의 신뢰도 및 타당도 연구

        The purpose of the present study was to examine the reliability and validity of the Korean version of Joines Personality Adaptation Questionnaire(K-JPAQ). Data were collected from 1,445 Korean adults while Minnesota Multiphasic Personality Inventory(MMPI) was additionally administered to 150 subjects only. Eight clinical experts were interviewed to examine a content validity of the K-JPAQ. Data were analyzed using a variety of statistical methods including a correlation analysis, a Cronbach alpha analysis, and a principal component analysis. In general, most clinical experts agreed that the items of the K-JPAQ were well constructed and translated for assessing the six personality adaptations in Korea. Cronbach's alpha was .790 for all the items of the K-JPAQ. The six personality adaptations was significantly correlated with their MMPI counterparts. Six factors were extracted using principal component analysis, which was consistent with the six personality adaptations originally established by Joines and Stewart (2002). Overall, these results indicate that the K-JPAQ is reliable and valid for Korean adults. Thus, K-JPAQ could be utilized as a reliable and valid tool for clinicians and researchers in Korea. 본 연구의 목적은 Vann Joines(1986)가 성격적응을 측정하기 위해 개발한 성격적응 척도(JPAQ: Joines Personality Adaptation Questionnaire)를 국내에서 사용할 수 있도록 한국어 판 성격적응 척도(Korean Version of Joines Personality Adaptation Questionnaire; K-JPAQ)의 신뢰도와 타당도를 검정하는 것이었다. 본 연구는 1,445명을 대상으로 설문조사를 실시하였으며 전체 연구 대상자 중 150명을 선별하여 MMPI 검사를 실시하였다. 수집된 자료에 대해 다양한 분석을 실시하였는데, JPAQ 척도의 내용 타당도를 검정하기 위해 8명의 국내 심리상담 분야의 전문가들의 의견을 수렴하였고, JPAQ 척도와 MMPI 와의 상관관계 분석으로 준거타당도를 확인하였으며, JPAQ 척도의 요인구조를 파악하여 척도 개발자가 제시하고 있는 요인구조와 비교하였고, 끝으로 JPAQ 척도 전체와 하위요인별 내적일치도를 분석하였다. 본 연구의 주된 결과는 다음과 같다. 첫째, JPAQ 척도 전체와 하위요인들의 Cronbach' ɑ는 척도 전체가 .790으로 비교적 높은 신뢰성이 파악되었으며 하위요인들은 .510 이상의 신뢰도가 확인되었다. 둘째, JPAQ 척도는 전문가들에 의해 내담자들의 성격적응에 대한 구체적인 질문으로 구성되어 있는 것으로 확인되었는데 내용적합성 판단 결과가 평균 3.5로 상당히 높게 나타났다. 셋째, JPAQ 척도의 하위요인들과 MMPI 임상척도들 간에 상관관계가 있는 것으로 나타나 일정 정도 준거타당도가 확보되었다. 마지막으로 JPAQ 척도에 대한 요인분석 결과 척도 개발자가 제시하고 있는 것과 마찬가지로 생존 및 실행적인 성격적응이 각각 3개의 요인으로 구성되는 것으로 나타났다. 이러한 연구 결과를 바탕으로 연구의 의의와 한계 및 후속 연구를 위한 제언을 하였다.
