The aim of this study was to evaluate the clinical applicability of a ridge splitting technique for reconstruction of narrowed edentulous alveolar ridges for dental implantation. 41 patients with 47 edentulous areas were included in this study. After corticotomy of a rectangular buccal segment and about 3-4 weeks healing period, the alveolar ridge was splitted, implant was placed. The initial ridge width varied between 2.5 and 7.0mm, average was 3.99mm. 68 dental implants were placed, and the gap between the implants and the bone is filled with various bone graft material, or none. All splitted sites showed sufficient bone volume for insertion of the implants at the moment of implantation. Over 1 year of follow-up period, five implants(7.3%) had mobility in two patients, among them, 2 implants(2.9%) sustained infection and removed. And any other complications were not noted. Our conclussion is that ridge splitting of the narrow edentulous area is a reliable and safe procedure in horizontally deficient ridges.
가토 하악골 결손부의 자가골 이식시 혈소판 풍부혈장이 골형성 촉진에 미치는 영향에 관한 연구

Purpose : Recently, various materials were developed for enhancing bone formation capacity. Platelet rich plasma(PRP) is an autologous source with several growth factors and obtained by sequestering and concentrating platelets by gradient density centrifugation. This study was to evaluate the effect of PRP on healing of grafted bone. Materials and methods : Two blood samples were obtained and analysed for measuring platelet counts of normal blood and PRP. In experimental group, two defects of mandibular bone, 10mm in diameter and 4.0mm deep, were created in the mandible and immediately grafted with autogenous bone chips mixed with PRP. In control group, same bone defects were prepared and grafted with autogenous bone chips. Gelform was used for carrier of PRP. 2 weeks, 4 weeks, 8 weeks later, each group was evaluated with histologi-cal and histomorphometric analyses. Results : According to histological observation, experimental group was showed more anastomosing newly-formed woven bone having osteoblastic activation than control group. According to histomorphometric analysis, there were 9.11% more newly-formed bone volume in experimental group than control group at 2 weeks, 7.91% more at 4 weeks, 20.08% more at 8 weeks. Conclusion: Our results demonstrated PRP in autogenous bone graft could enhance the bone formation.

Purpose: The effect of platelet-rich plasma(PRP) on osteogenesis of marrow-derived osteoblasts on histomorphometric analysis in the mandible of rabbit was assessed. Materials and Method: Bone marrow cells were obtained from iliac bone of rabbits and were cultured in a Dulbecco's Modified Eagle's Medium(DMEM) with Dexamethasone, L-Ascortic acid, ${\beta}$-Glycerophosphate to proliferate and differentiate into osteoblasts for $4{\sim}5$ weeks. The expression of osteogenic mar-kers was detected by reverse transcription-polymerase chain reaction(RT-PCR) and silver nitrate stain. Then we prepared bony defects in the mandible of rabbit, 10.0mm in diameter and 4.0mm deep, by trephine bur. In the control group, the defects were filled with autogenous bone and cultured osteoblasts. In the experimental group, the defects were filled with autogenous bone, cultured osteoblasts and PRP. 2 weeks, 4 weeks, 8 weeks later, each group was evaluated with histological and histomorphometric analyses. Results: In vitro, osteoblasts were identified on RT-PCR and silver nitrate stain. According to histological observation, at 2 weeks well-developed anasto-mosing newly-formed woven bone was observed, at 4 weeks anastomosing newly-formed woven bone having osteoblastic activation was observed, and at 8 weeks thick newly-formed woven bone was observed in both control and experimental groups. According to histomorphometric analysis, there were 1.5% more newly-formed bone volume in experimental group than control group at 2 weeks, 28.4% more at 4 weeks, 4.3% more at 8 weeks. Particularly there were significant differences in bone volume at 4 weeks and 8 weeks new bone. Conclusion: Our results demonstrated PRP may enhance osteogenesis of marrow-derived osteoblasts at 4 weeks, 8 weeks.

Purpose: The purpose of this study is to evaluate the clinical availability of submentoplasty for esthethic improvement of the cervico-facial region of patients with obtuse chin-neck angle. Materials and methods: Case 1. We evaluate the changes of submental line length and chin-neck angle of 35-year-old woman with skeletal Class Ⅲ and mandibular excess with excessive submental fat before and after surgery: Bilateral sagittal split ramus osteotomy(BSSRO) setback(5㎜), Mandibular Angle Reduction, Reduction Malarplasty and Submentoplasty. In this case, It was done simultaneously with orthognathic surgery. Case 2. The changes of submental line length and chin-neck angle of 20-year-old man with skeletal class Ⅲ and maxillary defiency were evaluated before surgery, at first surgery: Lefort Ⅰ osteotomy(6㎜ posterior Impaction), BSSRO setback(9㎜), Paranasal Augmentation and at second surgery: genioplasty(6㎜ advanced) with submentoplasty, In this case, submentoplasty and advancement genioplasty were done after 2 months post-operative periods. Results : Case 1. In case of the Skeletal Class Ⅲ mandibular excess with submental fat deposit, It showed the improvement of submental angle and length of submental line after simultaneous submentoplasty. Submental angle is changed from 177° (pre-op) to 151° (post-op) and submental line length is changed from 8㎜(pre-op) to 36㎜(post-op). Case 2. The improvement of submental angle and length of submental line after delayed submentoplasty was aquired in case of the skeletal class Ⅲ maxillary defiency, Submental angle is changed from 154° (pre-op) to 161° (first surgery) and to 153° (second surgery) and submental line length is changed from 25㎜(pre-op) to 19㎜(first surgery) and to 23㎜(second surgery). Conclusion : The results suggest that Submentoplasty is useful surgical procedure for esthethic improvement of the cervico-facial region of patients with obtuse chin-neck angle.

Purpose: The short dental implant is considered as possible solution in the alveolar bone height deficient cases. The aim of this study was to evaluate clinical availability of short implants by measuring the marginal bone loss of short length implants and comparing with that of conventional length implants. Materials and Methods: The groups were composed of patients who had received at least one implant. The samples of this study were selected from patients who with functional loading after prosthetic treatment for 1 year follow up period. The implants with a length of 5.7㎜ and 6㎜ were considered short. (Bicon Dental implants, USA). The experiment group was composed of 4.5×6㎜, 5×6㎜, 6×5.7㎜ implants (total 18 implants were placed in 14 patients, 8 on maxilla, 10 on mandible). The control group was composed of 4.5×8㎜, 5×8㎜, 4.5×11㎜, 5×11㎜. All implants were selected only by implants placed on molar area. We evaluated marginal bone loss in radiographic images at baseline (implant loading) and 3, 6, 12 months after loading. Additionally, crown-to-implant ratio was evaluated, and marginal bone loss according to crown-to-implant ratio after functional loading was analyzed. Results: The short implant group had a mean marginal bone level of -0.52±0.69 ㎜; the 8㎜ group, -0.22±0.82㎜; and the 11㎜ group, -0.10 ± l.09㎜ after 1 year of functional loading. But significant differences were not detected between three groups at every follow-up period. Crown-to-implant ratio in short implant group was 1.55±0.23; 8㎜ group was 1.15±0.18; and 11㎜ group was 0.92±0.15. Additionally, significant differences between three groups were founded. (P<.0001) The greatest marginal bone loss after 1 year follow-up was founded at crown-to-implant ratio 1-1.49 range in short implant. Conclusion: The marginal bone loss of short implants was comparable to that of long implants. So, the short implants can be a clinically acceptable option.
변형 구개 유경 판막을 이용한 상하악 전방골 분절 성형술

This article presents a modified palatal pedicled incision design in anterior segmental maxillary osteotomy. This method includes the circumcrevicular incisions and two vertical relaxing incisions. Then, it is different from the Couper’s method with vestibular horizontal incision and other anterior segmental osteotomy techniques. The main advantages of this method are an improved visibility of surgical field and favorable preservation of palatal pedicle. A patient who had been operated by anterior segmental maxillary and mandibular osteotomy without pre operative orthodontic treatment was analyzed for advantages and complications during the intraoperative and early post operative period. There are no specific major complications such as infection, gingival recession and dehiscence. And the patient had a pleasing esthetic facial appearance.
