        가족-보건교사 간 학교건강관리 파트너십에 관한 통합적 문헌고찰

        Purpose: This study is an integrative review of previous research conducted on family-school nurse partnership in school health care, as perceived by school nurses, children, and families. Method: The study was conducted according to the five stages of integrative review suggested by Whittemore and Knafl (2005), which involved problem identification, literature search, data evaluation, data analysis, and presentation of results. Suitable literature was found using portals such as PubMed, CINAHL, Embase, and Web of Science. Key words such as school nurses, family, parents, children, and partnership were used to narrow the search results. Studies published in peer-review journals between 2006-2018 were selected. The quality of studies was appraised using critical appraisal tools provided by the Joanna Briggs Institute (2020). Result: Thirteen studies (5 quantitative and 8 qualitative) were finalized for the final review process. After an extensive review, four key themes of school nurse-family partnership in school health care were identified: partnership components, partnership barriers, partnership facilitators, and strategies to promote partnership. Conclusion: Partnership and its components, barriers, and facilitators, along with strategies for its promotion should be considered in school health care. Further studies are needed on the perceptions held by parents and children regarding school health care partnership.

        액션러닝 기반 의사소통 역량강화 프로그램의 효과

        의료기관의 환자안전 및 간호의 질 증진을 위해 의료진들의 효과적인 의사소통이 강조되고 있고 이에 따라 간호 대학생의 의사소통 역량강화에 대한 요구가 증가하고 있다. 학생들은 일 방향성의 강의식 수업보다 학생들의 적극적인 참여를 유도하는 교수법에 더욱 만족하고 수업성과를 효과적으로 달성하는 것으로 보고되고 있다. 학생중심 교수법의 일환으로 액션러닝은 학생들이 다수의 사람들과 양방향으로 소통할 수 있는 기회를 제공한다. 이에 본 연구는 일 간호대학의 2학년 재학생 45명을 대상으로 액션러닝 기반의 의사소통 역량 강화 프로그램을 개발하고 의사소통 교과목에 적용하여 효과를 평가하고자 시행되었다. 자료는 2017년 9월부터 12월까지 자가 보고식 설문조사를 통해 수집되었고, SPSS 23.0 프로그램을 이용하여 분석하였다. 본 연구결과 프로그램 적용 후 학생들의 대인간 의사소통 )t = -2.291, p= .027) 및 대인관계 능력 (t = -3.013, p = .004)이 유의하게 증가하였다. 반면, 자아존중감 (t = -0.661, p = .512)의 차이는 유의하지 않았다. 본 결과에 따라 의사소통 교과목에 액션러닝 기반의 의사소통 강화프로그램이 확대 적용되어야 할 것이다. Effective communication among healthcare workers is crucial for quality of nursing. Students have achieved their course outcomes and been more satisfied in the teaching method guiding active involvement. Action learning provides students opportunities that they can communicate bilaterally with others. We conducted this study to develop a communication enhancement program based on action learning and evaluated the effects of the program on nursing students’ interpersonal communication, relationship, and self-esteem. Subjects were 45 sophomores nursing students who enrolled in a communication class. Data were collected using a questionnaire and analyzed using SPSS 23.0 program. Results indicated that the communication enhancement program significantly increased interpersonal communication (t = −2.291, p = .027) and relationship (t = −3.013, p = .004). However, self-esteem was not significantly increased (t = −0.661, p = .512). Such programs including the communication enhancement based on action learning should be applied extensively in communication curriculum.

        일 소아 중환자실에서의 12시간 교대근무가 간호사의 직무만족도, 삶의 질, 안전사건 보고 및 시간 외 근무에 미치는 영향

        The aim of this study was to compare job satisfaction, quality of life (QOL), incident report rate and overtime hours for 12-hour shifts and for 8-hour shifts in a pediatric intensive care unit (PICU). Methods: A descriptive survey was conducted with a convenience sample of 36 staff nurses from a PICU in a regional hospital in Korea. Data were collected using self-administrated questionnaires regarding job satisfaction and QOL at 6 months before and after the beginning of 12-hour shifts. Incident report rate and overtime hours for both 12-hour and 8-hour shifts were compared. Comparisons were made using x2-test, paired t-test and Mann-Whitney U test. Results: After 12-hour shifts were initiated, job satisfaction significantly increased (t=3.93, p<.001) and QOL was higher for nurses on 12-hour shifts compared to 8-hour (t=7.83, p<.001). There was no statistically significant change in incident report rate (x2=0.15, p=.720). The overtimes decreased from 36.3±34.7 to 17.3±34.9 minutes (Z=-8.91, p<.001). Conclusion: These results provide evidence that 12-hour shifts can be an effective ways of scheduling for staff nurses to increase job satisfaction and quality of life without increasing patient safety incidents or prolonged overtime work hours.

        선천성 심질환으로 고식적 수술을 시행 받은 영아의 홈모니터링의 성과

        Purpose: Common conditions, such as dehydration or respiratory infection can aggravate hypoxia and are associated with interstage mortality in infants who have undergone palliative surgery for congenital heart diseases. This study was done to evaluate the efficacy of a home monitoring program (HMP) in decreasing infant mortality. Methods: Since its inception in May 2010, all infants who have undergone palliative surgery have been enrolled in HMP. This study was a prospective observational study and infant outcomes during HMP were compared with those of previous comparison groups. Parents were trained to measure oxygen saturation, body weight and feeding volume and to contact the hospital through the hotline for emergency situations. Telephone counseling was conducted by clinical nurse specialists every week post discharge. Results: Forty-one infants were enrolled in HMP. Nine hundred telephone counseling sessions were conducted. Seventy-three infants required telephone triage with the most common conditions being gastrointestinal (50.7%) and respiratory symptoms (32.9%). With HMP intervention, interstage mortality decreased from 18.6% (8/43) to 9.8% (4/41) ( χ²=1.15, p=.283). Conclusion: Results indicate that active measures and treatments using the HMP decrease mortality rates, however further investigation is required to identify various factors that contribute to hemodynamic complications during the interstage period.
