말기 폐림프관 평활근종증 환자에서의 단측 폐이식술 -1예 보고-

Lymphangioleiomyomatosis (LAM) is a rare disease in women in childbearing age leading to progressive respiratory failure. LAM is characterized by an abnormal harmartomatous proliferation of smooth muscle cells surrounding the blood vessels, lymphatics and airways in the lung. This proliferation leads to airway obstruction, cystic alveolar change and lymphatic obstruction. Patients present with dyspnea, pneumothorax, cough, chest pain, hemoptysis, and chylous effusion. Although lung transplantation is the only therapeutic modality in end-stage LAM with respiratory failure, there has not been any report of successful treatment in Korea. We report one case of successful left single lung transplantation in a 40 year old woman suffering from end-stage LAM. 폐림프관 평활근종증은 주로 가임기 여성에서 발병하며 점진적으로 호흡부전에 이르게 하는 드문 질환이다. 폐의 혈관, 림프관 그리고 기관지를 구성하는 평활근 세포의 비정상적 과오종성 증식으로 인해 기관지 폐쇄, 낭성 기포로의 변환, 림프관 폐쇄 등의 특징이 있으며 대부분 호흡곤란, 기흉, 기침, 흉통, 객혈, 유미흉 등을 동반한다. 호흡부전을 동반한 말기 폐림프관 평활근종증은 폐이식이 유일한 치료법으로 국내에서 시도된 적은 있으나 아직 성공사례에 대한 보고가 없었다. 본원에서는 폐림프관 평활근종증을 앓고 있던 말기 호흡부전의 40세 여자 환자에게 좌측 단일 폐이식술을 시행하여 좋은 결과를 얻었기에 이를 보고하는 바이다.
심정지 혹은 심인성 쇼크에서 원심성 펌프를 이용한 단기목표의 기계적 순환 보조 -5예 보고-

기계적 순환 보조는 만성 심부전 상태의 환자들에게 이용될 뿐만 아니라, 심정지 혹은 심인성 쇼크에서 생명을 구할 수 있는 필수적인 요소가 되기도 한다. 상용화된 다양한 박동형의 순환보조 장치들이 장기간의 좌심실 보조를 위해 사용될 수 있지만 비용이나 제조사로부터의 기술적 지원이라는 측면에서 그 사용에 제약이 되고있다. 최근에는 심장이식의 가교로서, 또는 심장회복을 위해 원심성 펌프를 단기간 사용한 예들이 보고되고 있다. 본원에서는 원심형 펌프를 이용한 체외 생명구조 장치나 좌심실 보조장치로 심정지 혹은 심인성 쇼크의 5명의 환자에게 적용하여 성공적으로 치료하였다. 이들중 2명이 허혈성 심질환에 의한 심정지로 응급 상황에서 시행되었고, 술 후 심인성 쇽을 보인 허혈성 질환 1예와 급성 바이러스성 심근염에 의한 저심박출량 증후군을 보인 2명은 예정 하에 수술하였다. 모든 환자에서 기계적 순환보조 이탈이 가능하였으며, 기계적 순환 보조와 관련된 합병증에는 출혈 및 급성 신부전등이 있었고 대퇴동, 정맥 캐눌라 삽관과 연관된 합병증은 없었다. 원심형 펌프를 이용한 기계적 순환보조는 심정지 혹은 심인성 쇼크에서 생명보존이나 유지에 필수적인 역할을 할 수 있으며 항시 고려해야할 요소로 판단되기에 이를 보고하는 바이다. Mechanical circulatory support (MCS) has been used for myocardium failure, but moreover, it may be essential for the life support in cardiac arrest or cardiogenic shock. Many commercial devices can be used effectively for the long-term support. However, there are some limitations in the aspects of the cost and technical support by production company. Short-term support with centrifugal type has been reported numerously with the purpose of bridging to heart transplantation or recovery. We successfully treated 5 patitents who were in the status of cardiogenic shock (n=3) or arrest (n=2) with the technique of extracorporeal life support system (ECLS) or left ventricular assist device (LVAD) using the centrifugal type pump. The MCS were performed emergently (n=2) under cardiac arrest caused by ischemic heart disease, and urgently (n=3) under cardiogenic shock with ischemic heart disease (n=1) or acute fulminant viral myocarditis (n=2). All patients were weaned from MCS. Complications related to the use of MCS were bleeding and acute renal failure, but there were no major complications related to femoral cannulations. Mechanical circulatory support may be essential for the life support and rescue in cardiac arrest or cardiogenic shock.
Delayed Gastric Emptying after Esophagectomy: Management and Prevention

The quality of life associated with eating is becoming an increasingly significant problem for patients who undergo esophagectomy as a result of the improved survival rate after esophageal cancer surgery. Delayed gastric emptying (DGE) is a common complication after esophagectomy. Although several strategies have been proposed for the management and prevention of DGE, no clear consensus exists. The purpose of this review is to present a brief overview of DGE and to help clinicians choose the most appropriate treatment through an analysis of DGE by cause. Furthermore, we would like to suggest some tips to prevent DGE based on our experience.
