        최근 언택트 시대에 프리랜서 시장이 활성화되면서 재능거래 플랫폼이 빠르게 성장하고 있다. 이러한 재능거래 플랫폼은 판매 금액의 일정 부분을 중개 수수료로 책정하여 수익을 올리는데, 서버-클라이언트 모델 기반의 서비스를 제공하기 때문에 서버 운영비용이 발생하여 수수료가 높다는 단점이 있다. 본 논문은 분산저장파일시스템(IPFS)을 활용한 이더리움 플랫폼 기반 분산앱(dApp) 시스템에 대한 것으로, 거래 정보 같은 가벼운 데이터는 블록에 올리고 블록체인에 올릴 수 없는 대용량 데이터는 IPFS에 분산 저장 후 파일 주솟값인 CID 값을 블록체인에 올려 서버 및 데이터베이스 운영비용을 절감하는 방법을 제안한다.

        계층적 군집화 기반 Re-ID를 활용한 객체별 행동 및 표정 검출용 영상 분석 시스템

        최근 영상 데이터의 급증으로 이를 효과적으로 처리하기 위해 객체 탐지 및 추적, 행동 인식, 표정 인식, 재식별(Re-ID)과 같은 다양한 컴퓨터비전 기술에 대한 수요도 급증했다. 그러나 객체 탐지 및 추적 기술은 객체의 영상 촬영 장소 이탈과 재등장, 오클루전(Occlusion) 등과 같이 성능을 저하시키는 많은 어려움을 안고 있다. 이에 따라 객체 탐지 및 추적 모델을 근간으로 하는 행동 및 표정 인식 모델 또한 객체별 데이터 추출에 난항을 겪는다. 또한 다양한 모델을 활용한 딥러닝 아키텍처는 병목과 최적화 부족으로 성능 저하를 겪는다. 본 연구에서는 YOLOv5기반 DeepSORT 객체추적 모델, SlowFast 기반 행동 인식 모델, Torchreid 기반 재식별 모델, 그리고 AWS Rekognition의 표정 인식 모델을 활용한 영상분석 시스템에 단일 연결 계층적 군집화(Single-linkage Hierarchical Clustering)를 활용한 재식별(Re-ID) 기법과 GPU의 메모리 스루풋(Throughput)을 극대화하는 처리 기법을 적용한 행동 및 표정 검출용 영상 분석 시스템을 제안한다. 본 연구에서 제안한 시스템은 간단한 메트릭을 사용하는 재식별 모델의 성능보다 높은 정확도와 실시간에 가까운 처리 성능을 가지며, 객체의 영상 촬영 장소 이탈과 재등장, 오클루전 등에 의한 추적 실패를 방지하고 영상 내 객체별 행동 및 표정인식 결과를 동일 객체에 지속적으로 연동하여 영상을 효율적으로 분석할 수 있다. Recently, the amount of video data collected from smartphones, CCTVs, black boxes, and high-definition cameras has increased rapidly. According to the increasing video data, the requirements for analysis and utilization are increasing. Due to the lack of skilled manpower to analyze videos in many industries, machine learning and artificial intelligence are actively used to assist manpower. In this situation, the demand for various computer vision technologies such as object detection and tracking, action detection, emotion detection, and Re-ID also increased rapidly. However, the object detection and tracking technology has many difficulties that degrade performance, such as re-appearance after the object’s departure from the video recording location, and occlusion. Accordingly, action and emotion detection models based on object detection and tracking models also have difficulties in extracting data for each object. In addition, deep learning architectures consist of various models suffer from performance degradation due to bottlenects and lack of optimization. In this study, we propose an video analysis system consists of YOLOv5 based DeepSORT object tracking model, SlowFast based action recognition model, Torchreid based Re-ID model, and AWS Rekognition which is emotion recognition service. Proposed model uses single-linkage hierarchical clustering based Re-ID and some processing method which maximize hardware throughput. It has higher accuracy than the performance of the re-identification model using simple metrics, near real-time processing performance, and prevents tracking failure due to object departure and re-emergence, occlusion, etc. By continuously linking the action and facial emotion detection results of each object to the same object, it is possible to efficiently analyze videos. The re-identification model extracts a feature vector from the bounding box of object image detected by the object tracking model for each frame, and applies the single-linkage hierarchical clustering from the past frame using the extracted feature vectors to identify the same object that failed to track. Through the above process, it is possible to re-track the same object that has failed to tracking in the case of re-appearance or occlusion after leaving the video location. As a result, action and facial emotion detection results of the newly recognized object due to the tracking fails can be linked to those of the object that appeared in the past. On the other hand, as a way to improve processing performance, we introduce Bounding Box Queue by Object and Feature Queue method that can reduce RAM memory requirements while maximizing GPU memory throughput. Also we introduce the IoF(Intersection over Face) algorithm that allows facial emotion recognized through AWS Rekognition to be linked with object tracking information. The academic significance of this study is that the two-stage re-identification model can have real-time performance even in a high-cost environment that performs action and facial emotion detection according to processing techniques without reducing the accuracy by using simple metrics to achieve real-time performance. The practical implication of this study is that in various industrial fields that require action and facial emotion detection but have many difficulties due to the fails in object tracking can analyze videos effectively through proposed model. Proposed model which has high accuracy of retrace and processing performance can be used in various fields such as intelligent monitoring, observation services and behavioral or psychological analysis services where the integration of tracking information and extracted metadata creates greate industrial and business value. In the future, in order to measure the object tracking performance more precisely, there is a need to conduct an experiment using the MOT Challenge dataset, which is data used by many international conferences.         관절외 배부 폐쇄 쐐기 절골술을 이용한 Freiberg병의 치료 결과

        Purpose: The aim of this study was to evaluate the result of extraarticular dorsal closing wedge osteotomy in Freiberg's disease. Materials and Methods: Between February 2012 and July 2014, total 10 patients who underwent dorsal closing wedge osteotomy and followed up more than 1 year were selected for inclusion. Average age was 16.3 years, and average follow-up period was 15.5 months. The diagnosis was made using magnetic resonance imaging of those with a limitation in walking or usual activity due to pain in the metatarsal head. During operation, we removed loose body, and synovectomy was done. Osteotomy at the metatarsal neck and fixation with Kirschner wire were performed. X-ray was taken to check shortening of 2nd metatarsal and bone union. Moreover, we checked the active range of motion of 2nd metatarsophalangeal joint before and after surgery. At the last follow-up, the shortening of metatarsal, American Orthopaedic Foot and Ankle Society (AOFAS) score, visual analogue scale (VAS), and patient's subjective satisfaction were evaluated. Results: According to the Smillie's stage, there were 3 cases of stage II, 4 cases of stage III, and 3 cases of stage IV. Average bone union time on the osteotomy site was 8 weeks. Average shortening of metatarsal was 2.53 mm. Average AOFAS score improved significantly from 56.9 to 82.8 points at final follow-up (p<0.05), and average VAS score also improved significantly from 6.4 to 1.4 points at final follow-up (p<0.05). Average active range of motion at metatarsophalangeal joint improved from $28.0^{\circ}$ preoperatively to $46.5^{\circ}$ at the final follow-up. Other complications, such as metatarsalgia and arthritis, were not found; however, there was 1 case of delayed union with no symptom. Conclusion: In Freiberg's disease, dorsal closing wedge osteotomy is recommended for the improvement of clinical symptoms and range of motion.

        수술 방법에 따른 리스프랑 관절 손상의 결과 비교: 배부 가교 금속판, 관절 관통 나사 및 혼합 고정법

        목적: 리스프랑 관절 손상 시 관혈적 정복술 및 관절 관통 나사를 이용한 고정법이 흔한 방법으로 여겨졌으나, 관절면의 손상 등의 추가적인 합병증이 발생한다. 이에 본 연구는 리스프랑 관절 손상 시 배부 가교 금속판, 관절 관통 나사및 혼합 고정법에서의 임상적, 방사선학적 결과를 비교하고자 하였다. 대상 및 방법: 2015년 6월부터 2021년 3월까지 본원에서 리스프랑 관절 손상으로 수술 후 6개월 이상 추시가 가능하였던 43예의 환자 중, 추시가 되지 않은 1예와 합병증으로 절단술을 시행한 1예, 사망한 1예를 제외한 40예를 대상으로 후향적으로 진행하였다. 최종 추시 시 Wilppula 분류로 정복도를 평가하였고, AOFAS midfoot score를 평가하였다. 결과: 배부 가교 금속판 고정 시 AOFAS midfoot score와 정복도가 가장 높았다. 그리고 정복도가 좋을수록, 열 고정 개수가 적을수록 AOFAS midfoot score는 높았다. 합병증으로 는 배부 가교 금속판을 이용한 군에서 고정 나사의 파손 1예, 혼합 고정법을 이용한 군에서 창상부 괴사로 피부 결손이 발생하였고, 음압 폐쇄 드레싱 및 피부이식술 후 호전된 1예가 있었다. 결론: 리스프랑 관절 손상 환자의 수술 시 배부 가교 금속판을 이용한 고정은 리스프랑 관절 손상 시 시행할 만한 적절한 술식이다. Purpose: In Lisfranc joint injury, the traditional treatment has been open reduction and internal fixation with a transarticular screw. Despite this, additional complications, such as damage to the articular surface and breakage of the screw, have been reported. Therefore, this study compared the clinical and radiological outcomes of dorsal bridge plating with those of transarticular screws and combination treatment in Lisfranc joint injury. Materials and Methods: Among the 43 patients who underwent surgical treatment due to Lisfranc joint injury from June 2015 to March 2021, 40 cases followed for more than six months after surgery were analyzed, excluding three patients: one lost to follow-up, one had to amputate, and one expired. The radiological parameters were measured using the Wilppula classification in the last follow-up. The clinical outcomes were evaluated using the American Orthopaedic Foot and Ankle Society (AOFAS) midfoot score. Results: The AOFAS midfoot score, according to the surgical method, was significantly higher in the dorsal bridge plating (p=0.003). The radiological outcomes showed significantly better anatomical reduction when dorsal bridge plating was used (p=0.040). According to the Wilppula classification, the AOFAS midfoot score improved as the quality of anatomical reduction improved (p=0.018). Finally, the AOFAS midfoot score decreased as the number of column fixations increased (p=0.002). There were two complications: screw breakage in dorsal bridge plating and superficial skin necrosis in the combination treatment. Skin defects caused by necrosis improved after negative pressure wound therapy and split-thickness skin graft. Conclusion: In treating Lisfranc joint injuries, open reduction and internal fixation by dorsal bridge plating can be an appropriate treatment option. Nevertheless, studies, such as long-term follow-up research, on complications, such as osteoarthritis, will be needed.

        전내측 도달법을 이용한 Bifurcate Ridge 중심의 해부학적 전방십자인대 재건술 후 대퇴 터널 위치에 대한 평가

        목적: 전내측 도달법을 이용한 해부학적 전방십자인대 재건술 시 이분 융기선을 대퇴 터널의 중심으로 터널을 만든 후 3차원 computed tomography (CT)를 이용하여 위치의 적정성을 평가하고자 하였다. 대상 및 방법: 2012년 3월부터 2013년 2월까지 해부학적 전방십자인대 재건술을 시행하였던 35예 중 후향적인 방법으로 수술 후 3차원 CT를 측정하였던 32예에 대해 Mimics을 이용하여 대퇴골 외과의 내측연을 재건한 후 Bernard의 quadrant 방법을 이용하여 터널의 위치를 평가하였다. 결과: 대퇴 터널의 중심은 대퇴 외과의 심부에서 표층으로 32.2% (27.4-37.6%), Blumensaat’s line에 수직인 선을 따라 상위에서 하위로 46.7% (43.5-51.1%)에 위치하였고, 이는 기존의 대퇴 부착부 위치를 분석한 문헌과 비교하여 대퇴 부착부 안에 존재하였으나 부착부의 중심보다는 약간 표층 및 하위에 위치하였다. 결론: 4자 모양 하지 위치에서 전내측 도달법을 이용하여 해부학적 전방십자인대 재건술을 시행하는 경우 이분 융기선은 좋은 해부학적 지표가 될 수 있으며 대퇴골 외과 후방 피질골 등의 손상 없이 해부학적 단일 대퇴 터널을 만드는 데 유용한 방법으로 생각된다. Purpose: The purpose of this study is to analyze the position of the 10-mm-sized femur tunnel drilled aiming for the bifurcate ridge using anteromedial portal technique with ‘Figure of 4 position’ by 3-dimensional computed tomography (3D-CT) reconstruction images after anatomic anterior cruciate ligament (ACL) reconstruction and to evaluate it’s propriety. Materials and Methods: Out of 35 patients who underwent anatomic ACL reconstruction from March 2012 to February 2013, 32 patients who undergone postoperative 3D-CT scans were included in this study retrospectively. Medial surface of the lateral femoral condyle was reconstructed using Mimics, and then the position of the femoral tunnel was evaluated using the Bernard quadrant method and the results were compared with those of published literatures. The mean age of the patients was 32.6 years old. There were 25 cases of double-bundle ACL reconstruction with one femoral-two tibial tunnel. There were 7 cases of single bundle ACL reconstruction with one femoral-one tibial tunnel. Results: The mean distance of the femoral tunnel center was 32.2%±2.9% (range, 27.4%-37.6%) along the line parallel to the Blumensaat’s line from the posterior condylar surface and 46.7%±2.3% (range, 43.5%-51.1%) along the line perpendicular to the Blumensaat’s line from the roof of the notch. In comparison with the results of published literature, although the center of the femoral tunnel was presented in the femoral footprint, it was located slightly more shallow and inferior than the center of the ACL footprints. Conclusion: The bifurcate ridge may be a good anatomic landmark when making a 10-mm-sized single femoral tunnel in ‘Figure of 4 position’ using the anteromedial portal technique.

        간 , 담도 및 췌장 : 간 경변증 환자에서 혈중 α - human Atrial Natriuretic Peptide , Renin 활성도 및 Aldosterone에 관한 연구

        N/A Recent studies have shown that atrial natriuretic peptide, which is a thrird factor that is important in electrolytes and volume homeostasis, is increased or decreased in patients with liver cirrhosis that complicate ascites or not. For further evaluation about levels and effects of circulating ANP in liver cirrhosis author measured simultaneously the plasma concentracion of α-hANP, renin activity, aldosterene by radioimmunoassay and 24-hour urine sodium levels in series of 22 patients with liver cirrhosis, and compared these values with those obtained in 18 normal controls. The results were as follows: 1) The plasma levels of α-hANP (pg/ml) in normal controls (18 cases), cirrhotics without ascites (10 cases), and cirrhotics with ascites (12 cases) were 71.56+-9.35, 73.62+-8.72, and 60.88+-8.60 respectively: and those of cirrhotics with ascites were significantly decreased(p<0.01). 2) In cirrhotics with ascites, plasma renin activity (ng/ml/hr) and plasma aldosterone levels (pg/ml) were 8.52+-2.70 and 303.50+-48.52 respectively and were significantly increased, as compared to those in the normal controls and cirrhotics without ascites(p<0.001). 3) There was no significant correltation between plasma α-hANP levels and renin activity in normal controls and cirrhotic patients (r= 0.45). 4) There was no significant correlation between plasma α-hANP levels and 24h excretion of sodium in normal controls and cirrhotic patients (r=0.225).

        소화성 궤양환자의 흡연여부에 따른 혈청 Pepsinogen 1 치의 변화

        N/A Serum group I Pepsinogen (sPG I) levels were measured by radioimmunoassay in 159 patients undergoing endoscopy, then all patients were classified by endoscopic diagnosis, gastric mucosal histology, and smoking habits. The results were as follows: 1) Both nonulcer dyspepsia and duodenal ulcer smokers had significantly higher sPG I levels than their nonsmoking controls (p<0.005). 2) The frequency distvibution of superficial gastritis in the nonulcer dyspcpsia group with smoking habits revealed significantly higher levels than that of the nonsmoking controls (p<0.0025). 3) Serum PG Iin nonulcer dyspepsia patients revealed significantly higher levels in patients with superficial gastritis but there were not related to cigarette smoking. 4) The relationship between the mean level of serum PG I and fundic mucosal histology in duodenal ulcer patients was found to be statistically insignificant, but the mean level of sPG I in the smoking group showed a higher value than that of the nonsmoking controis (p< 0. 05). 5) The frequency distribution of high serum PG I level (> 80 ng/ml) in duodenal ulcer smokers was higher than that of the nonsmoking control group (chi square test: p<0.). From the above findings, it is concluded that the smoking induced increase in sPG I in duodenal ulcer is proposed to release an augmented pepsin secretory capacity, which can have aetiologic significance in association between smoking and duodenal ulcer.

      • 굼벵이와 누에 번데기의 아미노산 및 지방산 조성에 대한 연구

        Physical and chemical properties, fatty acid compositions and amino acid compositions of the white grub (Mimela lucidula) oil and the chrysalis of silk worm (Bombix mory) oil were investigated by a gas chromatograph and an amino acid auto analyzer. The results were as follows. 1. White grub oil had refractive index 1.4620, smoke point 170℃, iodine value 116.5, saponification value 183 and acid value 3.4 in its physical and chemical properties, and in the case of chrysalis of silk worm it had refractive index 1.4720, smoke point 165℃, iodine value 127.6, saponification value 190 and acid value 4.3, as the results, both oil would be semidrying oil, higher unsaturated fatty acid. 2. The contents of crude fat were 2.25% in white grub and 16.75% in chrysalis of silk worm, and the crude protein contents 10.80% in white grub and 21.23% in chrysalis of silk worm 3. Fatty acid compositions of white grub oil identified by a gas chromatograph were 33.10% of oleic acid, 29.47% linolenic acid and so on, and in chrysalis of silk worm, the compositions were 32.20% of oleic acid, 30.46% of linolenic acid and so on. 4. Total amounts of amino acid were 47,090㎎/100g in white grub, the compositions were 7,7l0㎎/100g of glutamic acid and so on, and in chrysalis of silk worm 41,580㎎/100g, and the compositions were 5, 450㎎/100g of glutamic acid and so on. 5. Total amounts of essential amino acid were 2,065㎎/gN in white grub and 2,610㎎/gN in chrysalis of silk worm.
