151 patients, with hypertensive putaminal hemorrhage, which confirmed by brain CT, were analyzed on the basis of Glasgow coma scale and Glasgow out come scale. The results of the treatment of hypertensive putaminal hemorrhage and the assessment of the prognostic factors were as follows: 1) The sex ratio was similar and the incidence was highest in the 5th decade for female and the 6th decade for male ; 75 percents of all cases were in the 5th and 6th decades. 2) A history of hypertension was a poor prognostic factor but a history of cerebrovascular disease was not a specific factor affecting the prognosis. 3) In the patients with low blood pressure, high G.C.S, and small amount of hematoma, were better the prognosis than in the patients with high blood pressure, low G.C.S, and large amount of hematoma. The prognosis was poor in old aged and in the patients with signs of herniation. 4) In the patients with G.C.S between 7-9, cerebral herniation signs, and the amount of hematoma between 40-80cc, the prognosis was better in operative treatment than conservative treatment. 5) The surgical modalities were not related to the prognosis.

The authors report a rare case of intracranial gliosarcoma associated with extracranial metastases. Computerized tomography and magnetic resonance imaging revealed a large mass involving the left sylvian fissure and a small round mass in the right frontal area. Chest X-ray showed a suspicious coin lesion; but a few small nodular densities, suspicious of metastasis, were noted on the chest CT. Grossly total resection of the tumor was achieved via CT-guided stereotactic fronto-temporal craniotomy. The supero-lateral part of the tumor was well demarcated from the surrounding non-neoplastic brain tissue, but at the medio-basal part, it was difficult to distinguish the tumor margin from the adjacent brain tissue. Histologically the tumor was diagnosed as gliosarcoma composed of two components, namely sarcomatous and glioblastomatous ones. The masses in the lung and breast were diagnosed to be a metastatic gliosarcoma by percutaneous needle biopsy. The patient received radiation therapy after surgery.

The authors report a case of very rare primary intracranial histiocytosis-X involving the cerebello-pontine angle. A 43-year-old female presented with 2-months history of occipital headache and dizziness Computerized to mography and magnetic resonance imaging revealed a mass involving the cerebellopontine angle with marked surrounding brain edema Angiogram showed no abnormal tumor blush Total resection of the mass was achieved via right retromastoid suboccipital approach. Histologic findings revealed pathologic features of histiocytosis-X.

Background: Glossopharyngeal neuralgia (GPN) is a rare type of cranial nerve rhizopathy that accounts for roughly 1% of trigeminal neuralgia cases and presents as extreme neuralgic pain in the ipsilateral deep throat, tongue base, or ear. Pain is usually intense and electric shock-like, occurring either without warning or triggered by swallowing. The purpose of this article is to provide a comprehensive review of the diagnosis and treatment of GPN and to differentiate it from trigeminal neuralgia. Current Concepts: In this review, our experiences, including cases of misdiagnosis and diagnostic pitfalls, are presented in detail. Treatment of GPN with microvascular decompression (MVD) has a success rate of over 90%. The use of “off-the-root entry zone” MVD, which eliminates the need for an adjuvant rhizotomy, is the best treatment for GPN. In addition, although gamma knife radiosurgery is categorized as a destructive procedure, it can still be another option if a patient is ineligible for MVD. Discussion and Conclusion: Practitioners must consider GPN when diagnosing patients with cranial nerve rhizopathy; it should be distinguished from other pain syndromes, especially trigeminal neuralgia. With an accurate diagnosis, an appropriate treatment plan can be developed.

Linear programming (LP) is widely used for development of energy and emission reduction analysis models because of its simplicity and ease of operation, but actual model users sometimes do not understand all the structures of models based on linear programming. The reason for this is that the bottom-up models have become more complicated with decision variables and constraints while subdividing the space, time, and operating modes. Besides LP's core constraints, various user-defined constraints added to reflect various restrictions of reality can be another obstacle for users. This paper summarizes efficient structures of LP formulation which have core decision variables and constraints specific to the road transport sector and minimal types of additional constraints for bottom-up energy system models. It shows the process of systematically extending from the most basic formulation to the case of technology with vintage and multiple activities. It also defines three types of constraints and their combinations that can encompass all kind of constraints often used by energy modelers. This work will contribute to better understanding of bottom-up energy models developed with LP and efficient building-up of the models for the road transport sector.
