하악공 전달마취법과 Gow-Gates법의 치수마취 효과

하악 견치에 대한 하악공 전달마취법과 Gow-Gates법간의 치수마취 효과를 비교하고 치수마취와 연조직 마취 양상의 관련성을 알아 보기 위하여, 24세 이상 29세 이하의 남녀 19명 (남 16명, 여 3명, 평균 나이 25.9세)을 대상으로 국소마취를 시행하고 시험 견치 및 반대측 대조 견치에 대해 전기치수검사를, 연조직에 대해서는 마취 징후와 prick-pin test를 시행하였다. 동일 피검자에 대한 두 가지 마취법의 시험은 10일 이상의 간격을 두고 시행하였으며, 전기치수검사는 마취 전과 마취 주사 종료 후 1분, 매 5분 간격으로 60분 간격 시점까지 매 10분 간격으로 100분 경과 시점까지 시행하였고, 대조 치아의 전기치수검사 역치와 전기치수검사기 출력 전압 등에 근거하여 치수마취 심도를 마취 실패, 마취 가능 그리고, 완전 마취로 구분하였다. 마취 주사 종료 후 하순과 설이 주관적 마취 징후를 나타내는 시간을 기록하였으며, 연조직 prick-pin test를 제 1대구치 협측 치은, 하악 견치 협, 설측 치은에 대해 5, 10, 20분 경과 시점에 시행하였다. SPSS 전산통계처리법을 이용하여 모든 반응 인자의 정규 분포를 확인한 다음, paired t-test를 이용하여 마취 방법에 따른 전기치수 검사 역치를 비교하였고 비모수 통계법인 월콕슨 순위검증을 이용하여 마취심도를 비교하였으며 연조직과 치수마취의 상관 관계를 분석하여 다음과 같은 결론을 얻었다. 하악공 전달마취법과 GOW-GATES법 간에 마취 후 전기치수 검사 최대역치와 마취심도 그리고 최대 역치 도달 시간에 있어 유의한차이가 나타나지 않았다.(P>0.005). 하악공 전달마취법과 Gow-Gates법 공히 치수마취와 하순의 마취 징후 및 연조직 반응 사이에는 유의한 상관관계가 없었다. The purpose of this study were to evaluate and compare the pulpal anesthesia induced by an inferior alveolar nerve block and that by Gow-Gates technique, and to investigate the relationship between pulpal anesthesia and intraoral soft tissue responses. After one side of mandibule was anesthetized with inferior alveolar nerve block or Gow-Gates techinque using 2% lidocaine with 1:100,000 epinephrine in 19 volunteers of ages between 24 and 29 (16 males and 3 females, average age 25.9 yrs.). electric pulp tests were done on the canine teeth of the anesthetized side and contralateral one before, at 1 min, continued at every 5 minutes until 60 min, and every 10 minutes until 100 min after completion of local anesthetic injection. Degree of pulpal anesthesia was classified as anesthetic failure, possible anesthesia and complete anesthesia by acriteria based on the thresholds to electric pulp test of contralateral canine and the currents of the electric pulp tester. Subjective signs on the lower lip and tongue were checked and prick-pin tests were done on the buccal gingiva of the first molar, buccal and lingual gingiva of the canine tooth at 5, 10, 20 min after the completion of anesthetic injection. Thresholds to electric pulp test, degree of pulpal anesthesia and relationship between the pulpal anesthesia and soft tissue reponses were analyzed with SPSS, paired t-test, Wilcoxon matched-pairs signed-ranks test and correlation analysis. The results were as follows: No significant differences were found in the peak thresholds to electric pulp test, in the induction time to it and in the depth of pulpal anesthesia between inferior alveolar nerve block and Gow-Gates technique (p>0.05). There was no significant relationship between pulpal anesthesia and soft tissue responses in both inferior nerve block and Gow-Gates technique.
