증례 : Ciprofloxacin에 의해 유발된 중증 면역성혈소판감소증 1예

저자들은 60세 남자 환자에서 ciprofloxacin 투여 12일 후, 매우 심한 비출혈을 동반하고 혈소판 수혈에도 호전되지 않는 혈소판 감소증이 발생하였고, 스테로이드와 면역글로불린 치료로 혈소판 수치의 급격히 호전을 보인 ciprofloxacin에 의한 면역성 혈소판 감소증 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. Ciprofloxacin is generally considered to be a safe drug with infrequent hematological side effects such as thrombocytopenia. Here, we report a case of severe immune thrombocytopenia associated with the use of ciprofloxacin. A 64-year-old man developed generalized purpura and severe epistaxis associated with severe thrombocytpenia that occurred after the use of ciprofloxacin. Thrombocy-topenia persisted in spite of repeated platelet transfusion. After discontinuation of ciprofloxacin and the use of corticosteroid and immunoglobulin, his platelet count rapidly recovered to normal value and his bleeding symptoms disappeared. Melanoacanthoma, a rare benign variant of seborrheic keratosis, was first introduced by Mishima and Pinkus in 1960. It is a slow-growing, painless, pigmented plaque with a verrucous surface and a diameter ranging from a few millimeters to 10 cm. It tends to develop during middle-age with an equal sex incidence, and the lesion usually presents on the head, trunk, and especially the lip. It also has a histopathologic finding of mixed neoplasm composed of melanocytes scattered throughout keratinocytic lobules, and is characterized by hyperkeratosis, parakeratosis, acanthosis, papillomatosis and pseudo-horn cysts in the epidermis. We report a case of melanoacanthoma occurring in a 52-year-old female, who presented with an easily bleeding mass on her right external auditory canal, an extremely uncommon site. Objective£ºIn generally, diagnosis of schwannoma is achieved by observation for tumor location, symptoms, radiological studies or surgical findings. However, it is reported that schwannoma can present a wide spectrum of histological patterns and show uncommon variants. The hypothesis of this study is that the tumor of a patient showing a second attack may show unexpected specific characteristics or phenotypic expression for genes associated with merlin or schwannomin. According to this hypothesis, the purpose of this study is to observe the progression state of vestibular schwannoma with molecular biological analysis. Methods£ºHistological and molecular biological analysis using immunohistochemistry and reverse transcription-polymerase chain reaction(RT-PCR) method were carried out using collected tumor tissues from a patient with recurrent vestibular schwannoma. Results£ºCollected tumor tissues showed a histologically typical schwannoma with spindle-shaped cells. Immunohistochemical study revealed that the tumor tissues were stained with S-100, vimentin, CD44, CD34 and desmin antibodies. However, glial fibrillary acidic protein(GFAP), HMB45 and cytokeratin antibodies were not reacted. In contrast, expression of GFAP was only examined in normal nerve tissue. Schwannoma related gene expression profiling was also examined. Neurofibromatosis type 2(NF2) and GFAP mRNA expressions in vestibular schwannoma were relatively lower than normal nerve tissues. Furthermore, the vestibular schwannoma expressed high level of FIR mRNA, which regulates neurite remodeling, compared to normal nerve tissues. Conclusion£ºThese results provide evidence that benign vestibular schwannoma tissue may progress to malignant tumor and suggest that these diagnostic options for managing a vestibular schwannoma might be supported to make an appropriate diagnosis and best treatment.

Invasive Aspergillus sphenoidal sinusitis rarely occurs and it is a potentially life threatening disease. It can be divided into an acute fulminant form with rapid disease progression and a chronic non-fulminant form. We report a case of fulminant invasive Aspergillus sphenoidal sinusitis in a 69-year-old diabetic woman who presented with ptosis of left eye and headache. Neuroimaging studies showed osteolytic tumor-like lesion in the sphenoidal sinus. Mass removal was performed using endonasal transsphenoidal approach and surgical biopsy revealed fungal hyphae in the specimen. In spite of using massive antifungal agents, the patient died of brain invasion and septisemia.
2-acetylaminofluorene의 투여에 의한 실험적 간암 발생과정에서 Cyclin D₁의 발현상태

Uncontrolled proliferation is a universal property of tumor cells. Deregulation of cell cycle regulators(cyclin D₁and cyclin-dependent kinase 4 inhibitor, etc) results in uncontrolled proliferation of cells. Recently, cyclin D₁, one of the cell cycle regulators has been accepted as a cellular proto-oncogene. Although the role of it is not well understood, deregulated overexpression of cyclin D₁occurs in several types of human cancers. To investigate the role of cyclin D₁ in multistep hepatocarcinogenesis, the author undertook an experimental hepatocarcinogenesis induced in 50 rats by feeding 2-acetylaminofluorene in diet. The animals were periodically sacrified and each liver was examined grossly and microscopically. And the immunohistochemical stain for cyclin D₁(Novocastra, Newcastle, U.K.) was performed and assessed. 1. At the third week of the experiment, the oval cells proliferated markedly. The altered cell lesions were noted at the fourth week. The clear cell areas were seen in the eighth week. At the third month, hepatic nodules developed, and carcinomatous nodules were noted in the seventh month. 2. The immunohistochemical stain of cyclin D₁ for oval cells and altered hepatic cell foci were exclusively negative. However, regenerating and adenomatous nodules were positive in four of 23 nodules (17%) and 17 of ten nodules (70%), respectively. And overexpression of cyclin D₁in hepatocellular carcinoma occurred in four of eight cases (50%). The above results hepatocinogenesis induced by 2-AAF, especially in transition period from altered cell lesion to regenerating nodule or from regenerating nodule to adenomatous nodule. However, the overexpression of cyclin D₁seems no active role in the transition from adenomatous lesion to hepatocellular carcinoma.
