M&L 심리치료가 불안장애를 가진 취약계층 청소년의 건강한 성장에 미치는 영향 - 단일증례의 양적, 질적 분석

Objectives: This study investigates the impact of M&L psychotherapy on an adolescent with anxiety disorders transitioning into adulthood, using a retrospective mixed-method approach. Methods: An adolescent with Diagnostic Statistical Manual of Mental Disorders (DSM)-5-diagnosed anxiety disorders underwent combined treatment with M&L psychotherapy and complex Korean medicine therapy over a period of approximately two years. Patient records tracked the diagnosis, treatments, and progress. In-depth interviews were also conducted. The study used a convergent parallel mixed-method approach, integrating quantitative analysis from psychological assessments, including the State-Trait Anxiety Inventory (STAI-X), the Core Seven Emotions Inventory-short form (CSEI-s), and the Minnesota Multiphasic Personality Inventory-2 (MMPI-2), with qualitative analyses of traditional Korean medicine counseling session notes, interviews, and other qualitative psychological assessments. Results: State anxiety decreased from 40 to 36, and trait anxiety decreased from 42 to 34. MMPI-2 L scale scores decreased slightly to 64T, whereas S scale scores increased to 62T, and FRS scores decreased to 70T from an initial score of 78T. Qualitative analysis revealed an increase in the MMPI-2 Es scale, identified as “subjectivity.” Joy (喜) emerged as the highest emotional profile score in the CSEI-S. Scores for anger (怒), thought (思), fear (恐), and fright (驚) decreased, indicating improved negative emotions. Qualitatively, expressions of determination to “move forward” and “strengthen the mind” were noted, suggesting enhanced “lower danjeon (下丹田).” Conclusions: Integrating traditional Korean medicine treatment with M&L psychotherapy for a vulnerable adolescent with anxiety disorders demonstrated efficacy. Further research is warranted to substantiate the effectiveness of M&L psychotherapy and advocate for its wider adoption in mainstream practice.

Primary adenosquamous carcinoma of the liver is generally considered as an extremely rare subtype of cholangiocarcinoma. It has been reported mostly in a form of case studies. As far as we know, there was only one case report on tumor related with biliary fistula. Recently, we experienced a case of primary adenosquamous carcinoma of liver with a formation of tumor-colonic fistula. A 54-year-old man was transferred to our hospital due to liver mass detected by abdominal ultrasonogram. Dynamic computed tomogram of liver showed a large irregular hypodense mass without rim enhancement in right lobe of liver and also suggested a fistula formation between the tumor and hepatic flexure of right colon. Colonoscopic examination showed a large colonic wall defect in hepatic flexure and a friable, nodular mucosa around the defected colonic wall. Extended right lobectomy and right hemicolectomy were done. Microscopically, the tumor was composed of squamous cell carcinoma mainly with foci of the adenocarcinoma component. The purpose of this paper is to suggest IDE (Incremental Displacement Estimation) algorithm for the previously proposed visually servoed paired structured light system. The system is composed of two sides facing with each other, each with one or two lasers with a 2-DOF manipulator, a camera, and a screen. The 6-DOF displacement between two sides can be estimated by calculating the positions of the projected laser beams and rotation angles of the manipulators. In the previous study, Newton-Raphson or EKF (Extended Kalman Filter) has been used as an estimation algorithm. Although the various experimental tests have validated the performance of the system and estimation algorithms, the computation time is relatively long since aforementioned algorithms are iterative methods. Therefore, in this paper, a non-iterative incremental displacement estimation algorithm which updates the previously estimated displacement with a difference of the previous and the current observed data is introduced. To verify the performance of the algorithm, experimental tests have been performed. The results show that the proposed non-iterative algorithm estimates the displacement with the same level of accuracy compared to the EKF with multiple iterations with significantly less computation time.

Hydatid cysts are caused by an infestation with larval tapeworms of the genus Echinococcus. The disease is endemic in developing countries but has rarely been reported from immigrant workers in Korea. This paper reports a case of hepatic hydatid cyst in a 27-year-old female. She was referred with abdominal pain that had persisted for the past 2 months. The patient was a foreign worker from Mongolia. The physical examination was unremarkable, and blood tests showed peripheral blood eosinophilia and elevated liver enzymes. Abdominal ultrasonography showed a well-circumscribed cystic mass with septation in the liver. A surgical resection was performed for complete removal. After uncomplicated postoperative recovery, the patient was discharged with albendazole 400 mg twice daily. The hydatid cyst is an important disease that should be considered in the differential diagnosis of cystic lesions in the liver, particularly in those who have lived in endemic areas. A correct early diagnosis based on the typical image findings is important for early treatment before the rupture of the cyst, which is associated with low morbidity and mortality. A current surgical resection combined albendazole are effective treatments for hepatic hydatid cysts, associated with low recurrence rates. (Korean J Gastroenterol 2021;77:35-38)
Design of Gas Flow Channel Routes in a PEM Fuel Cell System Using Topology Optimization

연료 전지의 유동채널에 관한 효과적인 위상최적화 방법은 이미 기체유동 채널의 위상최적화 분포를 찾는 분야까지 재발되고 적용되었다. 이 논문은 주로 기본적인 밀도 계산, 기체에 관한 확산방정식 및 incompressible한 Navier-Stakes Equation들로 결합시킨 위상최적화의 수치적 공식을 파생시키고 해결하는 방법을 기술한다. FEM을 이용하여 유체의 공간적 디자인 도메인을 정의하였다. 이 과정에서 도메인과 mesh는 최적화 과정에서 변하지 않았고, 최적화 변수 0은 Stake flow, 1은 Porous flow를 표현한다. 유한요소해석법으로 Navier-Stokes Equation를 이용하여 PDE form의 유체 유동 속도와 압력을 계산하였고 차후 해석하게 될 민감도를 찾기 위해 요소 행렬을 계산하였다. Motivation of this study is to apply the topology optimization technique and to find the route of gas flow channels in the bipolar plate of a fuel cell. The goal of optimization is to maximize the mean reaction rate of gases by finding the optimal porosity distribution. In an example analysis, a solution is pumped through a catalytic bed, where a solute species reacts as it gets in contact with the catalyst. Proton exchange membrane fuel cell (PEMFC) has a lot of virtues such as high-power density, high-energy efficiency, low-temperature operation, fast start-up and zero-emission, so it can be used for a car, an electrical equipment, portable power, and stationary products. The performance degradation of PEMFC is mainly induced by the unreasonable design route of bipolar plates. The optimum shape and route of gas flow channels in bipolar plates can make the reaction rate more uniform.
