Background: Atrial fibrillation (AF) has been linked to an increased risk for in-hospital and long-term mortality rates in patients with acute myocardial infarction (AMI). Obesity and metabolic syndrome (MS) are known to play an important role in cardiovascular morbidity and mortality. However, it is uncertain whether obesity and MS increase the risk of AF in patients with AMI. Therefore, we investigated independent risk factors for the occurrence of new-onset AF in patients with AMI who received optimal percutaneous coronary intervention (PCI). Methods: We prospectively analyzed the association between MS and the incidence of cardiac arrhythmia in 146 patients with AMI who underwent PCI. Twenty-four-hour Holter monitoring was performed 3 days after AMI. We divided the patients into two different groups based on the development of AF and analysed their obesity based on body mass index (BMI) (kg/㎡) and evaluated the existence of MS, as well as visceral obesity with fat computed tomography. Results: Seventy-five patients (51.4%) were obese (BMI ≥ 25) and 64 (44%) had MS. AF occurred in 33 (22.6%) patients. Age, MS, and visceral obesity were significantly associated with AF (p = 0.001, p = 0.003, and p = 0.03, respectively). There was no difference between obese and non-obese patients in the incidence of AF and VT. Multivariate analysis revealed that age and MS were independent risk factors of post-AMI AF. Conclusions: MS is an important and modifiable risk factor for new-onset AF especially in patients with AMI who underwent PCI.

본 논문에서는 중 약진 지역에서 중력 저항시스템인 중간 모멘트골조로 설계된 3층, 6층 RE 플랫플레이트 구조물을 KBC 2005를 만족하도록 RC구조물에 강판과 가새/ BRB 등의 보강방법을 적용하여 보강하고, 내진성능을 평가하여 보강 효과를 검증하였다. 비탄성 정적해석과 동적해석 결과에 따르면 내진 보강된 구조물은 강도와 강성이 크게 향상된 것으로 나타났다 특히 기둥을 철판으로 보강한 경우 슬래브를 철판으로 보강하여 조기 뚫림 전단파괴를 방지함으로써 강도를 크게 향상할 수 있다. BRB로 보강된 구조물은 Brace로 보강된 구조물보다 다소 연성적 거동을 보였으며, 그 효과는 3층 모델에서 현저하게 나타났다. In this study 3- and 6-story flat plate structures designed only for gravity load are retrofitted with steel plates and braces and their seismic performances are evaluated to verify the effect of seismic retrofit. According to the analysis results obtained from nonlinear static and dynamic analyses both the strength and stiffness are significantly enhanced as a result of the seismic retrofit. Especially the effect of column jacketing could be enhanced significantly when slabs were reinforced to prevent premature punching shear failure. When buckling-restrained braces are used instead of conventional braces, the structures showed more ductile behavior, especially in the 3-story structure.

In radiofrequency (RF) ablation for idiopathic left ventricular tachycardia (ILVT), the termination of tachycardia during RF ablation is considered a hallmark of success. However, in cases of patients with difficulty of induction of ventricular tachycardia (VT), the evaluation of procedural success can be problematic. We have observed thermal responses reflected as ventricular rhythm change to RF energy delivered on sinus rhythm for ILVT. We therefore describe the significance of repetitive ventricular responses. The study subjects were 11 ILVT patients for whom RF energy was delivered during sinus rhythm because of difficulty in re-induction of tachycardia. During each energy delivery, we focused on the occurrence of repetitive ventricular responses especially exhibiting a similar morphology to clinical VT. The repetitive ventricular responses were noted in 10 of 11 patients. Two patients received a second procedure due to the recurrence of ILVT. The mean follow-up period was 36.2±12.8 months. The clinical course of the remaining patients was favorable and without recurrence of ILVT. Based on the favorable clinical outcomes, ablation-induced repetitive ventricular responses with similar QRS morphology to clinical ILVT are useful markers for selecting an ablation site and could be used as an additional mapping method, termed as ‘‘thermal mapping’’.
인중에 대한 지속적 주의와 자율신경계 반응 간의 관계: 촉각주의집중과제가 심박변이도와 정서 변화에 미치는 영향

본 연구의 주요 목표는 명상에서 공통으로 강조되는 요소 중 하나인 인중에 지속적인 주의를 기울이는 것이 자율신경계 반응에 미치는 영향에 대해 살펴보는 것이다. 이를 위해 촉각주의 집중과제(Focused Tactile Attention Task, FTAT)를 개발하였다. FTAT는 특정 신체 부위에 지속적 으로 가해지는 촉각 자극에 주의를 기울여야 하며 자극의 강도가 변화할 때마다 이에 반응해 야 하는 과제이다. 본 연구에서는 코와 입술 사이인 인중 부위에 자극이 제시되도록 하였다. 명상 경험이 없는 136명의 참여자를 과제 수행 실험 집단과 휴식 통제 집단에 무선 할당하였 고, 1) 휴식 단계와 2) 과제 수행 단계 동안 심박변이도를 측정하였다. 그리고 과제 수행 단 계 전후로 긍정 및 부정 정서를 측정하였다. 결과에서 통제 집단에서는 유의한 변화가 나타 나지 않았으나, 실험 집단은 과제를 수행하는 동안 심박변이도가 유의하게 증가하였고 자기 보고로 측정한 긍정정서는 유의하게 감소하였다. 심박변이도의 증가는 부교감 신경의 활성화 를 반영하는 것으로 볼 수 있다. 그리고 긍정 정서의 감소는 긍정정서가 조절되어 안정된 상 태에 이르렀기 때문이거나 과제 수행이 참여자에게 다소간의 스트레스로 작용하였기 때문으 로 판단된다. 본 연구의 결과는 인중의 감각에 지속적으로 주의를 집중하는 것이 명상의 중 요한 요소이자 스트레스 완화와 건강 증진에 도움이 될 수 있음을 시사한다. In this study, we developed focused tactile attention task(FTAT), and measured changes in heart rate variability following FTAT to investigated relationship between attention regulation and autonomic activity in meditation. FTAT emphasize focused and sustained attention to sensation of philtrum which is given by persistent external tactile stimulus(air). Whenever intensity of stimulus changes, one must recognize and respond by pressing keyboard arrow keys. In this study, 136 participants who had no meditation experience were assigned to FTAT and resting control. Heart rate variability was assessed during (1) baseline resting phase and (2) task performance phase. Positive and negative affect were measured before and after task phase. Analysis revealed a significant increase in HRV during FTAT and a significant decrease in positive affect after FTAT, but not for controls. These results suggest that FTAT is experienced as mental stress subjectively but promote parasympathetic vagal tone and induce relaxation. Our findings imply focused and sustained attention to sensation of philtrum is important component of meditation and may help enhance wellbeing and stress reduction
