      • 뇌기저부 골수염으로 진행된 만성 침습성 진균성 부비동염 1예

        Chronic invasive fungal sinusitis is a life-threatening disease, but encompasses a more unhurried progression of illness, in contrast with acute invasive fungal sinusitis. This case is a fungal sinusitis with skull base osteomyelitis and meningitis progressed from fungal ball past years. Fungal ball is easily treated by endoscopic sinus surgery and rarely progress to invasive type. If invasive fungal sinusitis is not timely diagnosed by histopathologic evidence of fungal hyphae, it makes clinicians embarrassing and face poor clinical course of patient like this case. It should be noted that chronic invasive fungal sinusitis may be developed from even fungal ball in a diabetes mellitus patient and may extend to skull base osteomyelitis and meningitis.

        봉독에 의한 말초혈액단핵구의 증식 및 활성화 억제

        Background and Objectives:Bee venom (BV) has been used as a traditional medicine to treat a variety of diseases, and the anti-inflammatory, antinociceptive, and anticancer activity of BV has been studied over the past few years. This study was designed to investigate the effects of BV on the production of cytokines by peripheral blood mononuclear cells (PBMCs) and on the proliferation of these cells. In addition, we investigated the potential cytotoxic effects of BV on PBMCs. Materials and Methods:PBMCs from healthy human volunteers were stimulated with 5 μg/mL phytohaemagglutinin (PHA) and 10 μg/mL lipopolysaccharide (LPS) in the presence of different concentrations of BV. The cytotoxic effects of BV were measured using an aqueous cell proliferation assay kit and the supernatants were analyzed using an enzyme-linked immunosorbent assay. Tumor necrosis factor-α (TNF-α), interleukin-5 (IL-5), and interferon-γ (INF-γ) levels were measured to determine the activation status of PBMCs. Result:Although BV alone did not have a cytotoxic effect on PBMCs, PHA- and LPS-induced cytokine production was significantly inhibited by BV. TNF-α and INF-γ production was inhibited by a lower concentration (0.5-1 μg/mL) of BV than IL-5 (6 μg/mL). Conclusion:BV seems to be relatively safe as a modulator of cytokine production by PBMCs. These experimental data suggest that BV could be used to treat immunologic and inflammatory diseases.

        만성 부비동염 환자 비즙의 진균 배양

        Background and Objectives:Fungi have been increasingly recognized as important pathogens in sinusitis. Different types of fungi colonize in the nasal secretion of different individuals and the geographic distribution of fungi is different all across spectrum of fungal elements in the Korea population, and to clarify the association between fungal sinusitis with type I hypersensitivity. Materials and Methods:This prospective study evaluated 40 patients with chronic rhinosinusitis (CRS), 20 allergic rhinitis (AR), and 20 normal volunteers. Fungi were cultured with nasal irrigation and swab method, and skin prick test was done to determine the specific IgE against fungi. Results:Fungal cultures of nasal secretions were positive in Penicillium, Aspergillus were comonly cultured, but the fungal culture results were not so significantly different among three groups. Type I hypersensitivity to fungal allergens was not important in the majority of CRS patients. Conclusion:Fungi was present in every nasal secretion, and type I hypersensitivity seemed not important in the pathogenesis of CRS. (Korean J Otolaryngol 2002;45:479-82)

        Comparison of Manuka, Kanuka, and Black Locust Honey on the Production of Chemical Mediators by Peripheral Blood Mononuclear Cells

        Background and Objectives: Honey has various biological and pharmacological activities and has been used as treatment against various inflammatory diseases. The aim of this study was to compare the anti-inflammatory characteristics of manuka, kanuka, and black locust honey. Materials and Method: Peripheral blood mononuclear cells (PBMCs) from healthy human volunteers were isolated and then stimulated with lipopolysaccharide (LPS) with or without pre-treatment of various concentrations of honey for 72 hours. The cytotoxic effects of honeys were measured using an aqueous cell proliferation kit, and the supernatants were analyzed for interleukin-5 (IL-5), IL-10, interferon-γ (INF-γ), and tumor necrosis factor-α (TNF-α) using an enzyme-linked immunosorbent assay. Results: Samples of 1% manuka and kanuka honey were found to have cytotoxic effects on PBMCs. Honey itself enhanced the production of IL-10 and TNF-α production. Manuka and kanuka honeys suppressed LPS-induced IL-10 and INF-γ production, while black locust honey only suppressed IL-10 production from PBMCs. Conclusion: Honeys had immunomodulatory properties of both immunostimulatory and immunosuppressive effects on PBMCs. Different honeys might have different immune modulatory functions due to their different components.

        소아 상악동후비공 폴립의 부비동내시경 수술이 부비동 성장에 미치는 영향

        Background and Objectives:Pediatric endoscopic sinus surgery (ESS) has become widely used for the treatment of chronic or recurrent rhinosinusitis that is unresponsive to medical management. But the effect of pediatric ESS on facial growth or sinus growth are controversial and not well understood. The object of the present study was to determine whether pediatric ESS causes the volumetric restriction of the sinuses. Subjects and Method:Fifteen cases of patients who underwent unilateral endoscopic sinus surgery for antrochoanal polyp were reviewed retrospectively. Postoperative maxillary and ethmoid sinus volumes were measured by computed tomography at 73.5 (51-101) months after surgery, and these were compared with non-operated sinus volumes. Results:The mean volume of the operated and non-operated maxillary sinuses were 18.9±6.4 ml and 21.7±5.3 ml. and the mean volume of the operated and non-operated ethmoid sinuses were 4.1±1.2 ml and 3.7±1.2 ml. There was no significant difference in sinus growth between operated and non-operated sinuses. Conclusion:There was no evidence that pediatric ESS affected maxillary and ethmoid sinus growth.

        중심이동을 이용한 추적제어에 관한 연구

        To study the characteristic of the weight center control of humans, the tracking control capability of circular and wave motion by weight center movement was conducted by using the force platform. The control performance(the integrated value of the $|Object\;value(X)-Control\;Value(Y)|^{2}$) and control trace record was used to evaluate the individual performance characteristics. The size of the population for this study was 73, which consisted of engineering students, students majoring in taekwondo, students majoring in dance, all of which were in their twenties, and also people in their sixties. The results of this study indicate that the weight center control characteristics of humans can be represented by the evaluation method and values. People who were capable of tracking the object did not stop nor overshot the objective. In addition, habits or training characteristics and aging seemed to influence the performance of the subjects. In the future, development of different objectives for weight center control could be used to determine the severity of the disease of the subject and the effects of the treatment.

        말초혈액 단핵구 활성화에 대한 편백정유의 항염증 작용

        Background and Objectives The biological activity in antibacterial and antioxidative action of essential oils (EOs) have been investigated. In this study, we tried to evaluate the effects of Chamaecyparis obtusa EOs on producing chemical mediators by peripheral blood mononuclear cells (PBMCs). Subjects and Method PBMCs from healthy volunteers were stimulated with lipopolysaccharide (LPS) and phytohaemagglutinin (PHA) in the presence of varying concentrations of EOs. Cytotoxic effects of EOs were measured using an aqueous cell proliferation assay kit and supernatants were analyzed using enzyme-linked immunosorbent assay. Tumor necrosis factor-α (TNF-α), interleukin-5, and interferon-γ (INF-γ) protein levels were measured to determine the anti-inflammatory effects of essential oil. Results EOs were found to have cytotoxic effects on PBMCs at levels of over 1%. EOs not only could induce PBMCs to produce chemical mediators, but it also significantly inhibited the LPS induced TNF-α and INF-γ productions as well as the PHA induced INF-γ production. Conclusion EOs had cytotoxic effects at high concentrations and modulated chemical mediator productions from PBMC. These data suggest that EOs could be used to treat immunologic or inflammatory diseases. Korean J Otorhinolaryngol-Head Neck Surg 2017;60(11):565-9

        고령자에 적합한 가공식품 마개의 Torque에 관한 연구

        Korea has become an aged society according to the UN specifications. The number of the aged population will increase drastically, and the number of single aged people will also increase. Products used in our daily life are manufactured within the normal capabilities of people. Thus, aged people have encountering problems in using them. Numerous products are used in our daily lives. The purpose of this study is to find the acceptable torque for aged people to open the caps of processed food containers. Depending on the size and shape of the opening hand, hand operation has been classified into three categories. A total of one hundred and nine people participated in this study. Fifty nine people in the 20-30 year old range and fifty people in their 60-70's. Results of the study are as follows: 1) For all categories the acceptable cap torque data for the aged people are established. To satisfy 95% of the aged people, it was found that $74N{\cdot}cm$ for type A caps, and $141N{\cdot}cm$ for B type caps. and $214N{\cdot}cm$ for type C were the acceptable torque levels. 2) The appropriate level of torque for opening the cap was found to be 70% of the maximum torque for the aged people.(71.4% for type A. 71.4% for type B. 72.2% for type C). 3) Capabilities of the 70's drops sharply in function related to opening cap (maximum torque, gripping force, and wrist movement range). 4) Among aged subjects, 65.0% for type A, 42.0% for type B, and 68.8% for type C, had difficulties in opening the container caps.
