        소아에서 플러스옵틱스 에스09로 측정한 굴절력의 신뢰성과 유용성

        <b>Purpose:</b> To compare the refractive measurements obtained using a photorefractor (PlusoptiX S09, PlusoptiX GmbH, Germany) with those obtained using cycloplegic refraction in children. <b>Methods:</b> We assessed the refractive status of 268 eyes in 134 children. The values acquired via photorefraction with a PlusoptiX S09 device were compared with those obtained by cycloplegic retinoscopy. Hyperopia (≥+3.5 D), myopia (≥-3.0 D), with the rule or against the rule astigmatism (≥-1.5 D), and oblique astigmatism (≥-1.0 D) were set as diagnostic criteria for refractive amblyopia risk factors (RARFs). The difference in the detection of RARFs by the two methods was the main outcome measure. <b>Results:</b> The average spherical refractive power was -0.81 ± 1.68 D for PlusoptiX S09 versus -0.26 ± 2.00 D for cycloplegic retinoscopy (average difference -0.54 ± 0.61 D; <em>p</em> < 0.001). The average spherical equivalent was -1.20 ± 1.62 D for PlusoptiX S09 versus -0.64 ± 1.94 D for cycloplegic retinoscopy (average difference -0.56 ± 0.62 D; <em>p</em> < 0.001). The average cylinder power was -0.79 ± 0.93 D for PlusoptiX S09 versus -0.76 ± 0.94 D for cycloplegic retinoscopy (average difference -0.03 ± 0.33 D; <em>p</em> = 0.135). Even though cycloplegic retinoscopy is considered the gold standard, the sensitivity and specificity for detecting RARFs with the PlusoptiX S09 were 88.0% and 96.3%, respectively. <b>Conclusions:</b> PlusoptiX S09 is a relatively useful method for detecting RARFs, but the device tends toward myopic shift compared to cycloplegic refraction, and hyperopia is underestimated.         삼체형과 일체형 비구면 인공수정체의 안정성 비교

        Purpose: The goal of the present study was to compare the stability of the 3-piece (TecnisⓇ ZA9003) and single-piece (AcrySofⓇ IQ) aspheric intraocular lenses (IOL) by testing decentration, tilt, anterior chamber depth, and refraction. Methods: The subjects of this study consisted of 101 eyes who had undergone cataract surgeries with a 3-piece aspheric IOL (54 eyes) and with a single-piece aspheric IOL (47 eyes). The decentration, tilt, anterior chamber depth, and refraction were measured on postoperative day 1, 1 month, and 2 months, using an anterior eye segment analysis system (EAS-1000, Nidek, Japan). Results: There was a statistically significant difference in the decentration on the postoperative day 1 (p = 0.04). However, there was no statistically significant difference on postoperative 1 month (p = 0.15) and 2 months (p = 0.13). There was no statistically significant difference in the tilt on postoperative day 1, 1 month, and 2 months. There was no statistically significant difference in the anterior chamber depth on postoperative day 1, 1 month, and 2 months. There was a statistically significant difference in the refraction on postoperative day 1 (p = 0.03). However, there was no statistically significant difference on postoperative 1 month (p = 0.07) and 2 months (p = 0.07). Conclusions: There was no statistically significant difference in the decentration, anterior chamber depth, and refraction between the 3-piece and single-piece aspheric IOL. Therefore, there is no difference between the 3-piece and single-piece aspheric IOL in the capsular bag stability until 2 months postoperatively. J Korean Ophthalmol Soc 2010;51(12):1584-1589

        척추 손상 후 발생한 맥락막삼출과 굴절률 변화 1예

        Purpose: We report a case of refractive power change associated with choroidal effusion and temporary ocular deviation after a spinal fracture. Case summary: A 29-year-old woman had compression fracture of 12th thoracic vertebra after falling and complained of decreased vision and diplopia. In the ophthalmic examination, she had -2.875 D and -3.25 D spherical equivalent refractive error of right and left eye, respectively. There were choroidal effusions of both eyes in fundus exam. The anterior chamber shallowing and the ciliary body edema were noted on ultrasound biomicroscopy. The prism cover test shows 8 prism diopters (PD) exotropia and 12 PD hypertropia of her left eye. The patient was observed without any special treatment for a month. Conclusions: After a month follow up, the choroidal effusion was completely resolved and the patient recovered her vision with almost emmetropic state. Her ocular deviation was also recovered and diplopia was disappeared.

        정상 안저 소견을 보이는 제2형 당뇨 환자에서 대비감도 변화와 내측 망막두께 분석

        목적: 정상 안저 소견을 보이는 제2형 당뇨 환자와 정상인의 대비감도를 비교하고 당뇨 환자의 내측 망막두께를 분석하여 대비감도 변화에 영향을 미치는 인자를 분석하고자 하였다. 대상과 방법: 정상 안저 소견의 당뇨 환자 75안과 정상인 41안을 대상으로 암소시와 명소시 조건하에서 6.3°, 4.0°, 2.5°, 1.6°, 1.0°, 0.64°의 시각(degree)을 가진 시표에 대해 대비감도 역치를 측정하였고, 당뇨 환자에서는 빛간섭단층촬영을 이용하여 전체 망막두께, 망막신경섬유층(retinal nerve fiber layer, RNFL) 및 신경절세포-내망상층(ganglion cell-inner plexiform layer, GC-IPL)의 두께를 측정하였다. 결과: 당뇨 환자군은 대조군에 비해 암소시 및 명소시의 모든 시각에서 대비감도 역치가 감소되었다. 당뇨 환자 중 대비감도 역치가 감소된 군은 감소되지 않은 군에 비해 유병 기간이 길었고, 전체 망막두께와 평균 GC-IPL 두께, 상측 RNFL과 상측 및 이측 GC-IPL 두께가 얇았다. 대비감도 저하의 위험인자는 당뇨 유병 기간과 전체 망막두께로 나타났다(각각 odds ratio=1.117, 0.942). 결론: 제2형 당뇨 환자에서 당뇨의 유병 기간이 길어질수록 대비감도가 감소되며, 신경망막의 변성은 내측 망막 및 전체 망막두께 변화를 야기하고 대비감도 변화에 영향을 미칠 수 있는 것으로 사료된다. 따라서 유병 기간이 긴 당뇨 환자에서는 정상 안저 소견을 보인다 하여도 대비감도 변화와 빛간섭단층촬영상 망막두께의 변화에 유념할 필요가 있다. Purpose: To compare the contrast sensitivities of type 2 diabetic patients without retinopathy and healthy subjects, and to assess the risk factors associated with a change in contrast sensitivity in diabetes. Methods: A total of 75 (diabetic patients without retinopathy) and 41 (healthy subjects) eyes were reviewed from the medical records. The threshold of contrast sensitivity was measured at 6.3°, 4.0°, 2.5°, 1.6°, 1.0°, and 0.64° under scotopic and photopic states. Optical coherence tomography (OCT) imaging was used to measure the retinal nerve fiber layer (RNFL) thickness and ganglion cell-inner plexiform layer (GC-IPL) thickness in diabetic patients. Results: Diabetic patients showed a lower threshold of contrast sensitivity at all degree measures than did the controls under both scotopic and photopic states. In subgroup analyses, diabetic patients with abnormal contrast sensitivity showed a longer duration of diabetes, decreased total retinal thickness, and decreased average GC-IPL, superior RNFL, superior GC-IPL, and temporal GC-IPL thicknesses. Multivariate logistic regression analyses showed that the duration of diabetes and total retinal thickness were significant predictive factors of decreased contrast sensitivity (odds ratio = 1.117 and 0.942, respectively). Conclusions: As the duration of diabetes increased, the contrast sensitivity decreased in type 2 diabetic patients. Neuroretinal degeneration changes both the inner retinal thickness and total retinal thickness and affects contrast sensitivity. Therefore, for longer-term diabetic patients, it is necessary to consider the changes in contrast sensitivity and retinal thickness on OCT evaluation, even if the patient presents with normal fundus findings.

        한국 성인에서 안질환과 영양소 섭취와의 관련성에 관한 연구: 2015, 2016년 국민건강영양조사 자료를 이용하여

        This study was conducted to investigate the effect of the differences in nutrient intakes on the onset of eye disease in the ophthalmic disease group vs. the normal group. The analysis method of this study was performed by adjusting age and gender. The daily caloric intake was 1,672.1±26.4 kcal in the ophthalmic disease group which was significantly lower than the 2,041.5±13.6 kcal intake in the normal group (P=0.006). The intake of proteins (P=0.015) and carbohydrates (P=0.000) was significantly lower in the ophthalmic disease group than in the normal group. The incidence of eye diseases was found to decrease by about 0.79 (95% CI: 0.74∼0.83) times as the NAR index of protein was increased, followed by 0.79 (95% CI: 0.64∼ 0.98) times with an increase in niacin consumption. In this study, income, BMI, smoking, alcohol consumption, and quality of life were considered as the confounding variables related to eye disease, along with age and gender, but did not lead to show significant results. It was found that the ophthalmic disease group had an unbalanced nutritional intake compared to the normal group. Therefore, we conclude that nutrition education is necessary to ensurebalanced eating habits for management and prevention of degeneration after the onset of eye diseases.

        여러 가지 기기를 이용하여 측정한 전방깊이와 중심각막두께의 비교

        목적: 세 기기들을 이용하여 앞방깊이를 측정하고, 네 기기들을 이용하여 중심각막두께를 측정하여 각 기기 간의 정확성 및 임상적 유용성에 대해 알아보고자 하였다. 대상과 방법: 건강한 성인 90명의 180안을 대상으로 A-scan, Lenstar LS900Ⓡ, PentacamⓇ을 이용하여 앞방깊이를 측정하였고, 초음파 각막두께측정기, Lenstar LS900Ⓡ, PentacamⓇ, 전안부 빛간섭단층촬영계를 이용하여 중심각막두께를 측정하고 분석하였다. 결과: Lenstar LS900Ⓡ A-scan, PentacamⓇ을 이용하여 측정한 앞방깊이는 각각 3.27 ± 0.36 mm, 3.26 ± 0.36 mm, 3.25 ± 0.36 mm였고, 측정치 간에 유의하게 높은 양의 상관관계를 보였으며(p<0.001), 통계적으로 유의한 차이는 없었다(p=0.017). Bland Altman plot으로 분석한 일치도는 높지 않았다. 중심각막두께는 PentacamⓇ, 초음파 각막두께 측정계, Lenstar LS900Ⓡ, 전안부 빛간섭단층촬영계에서 각각 553.31 ± 25.23 μm, 547.26 ± 23.83 μm, 541.38 ± 24.49 μm, 531.40 ±22.33 μm로 측정치 간에 유의하게 높은 양의 상관관계를 보였으나(p<0.001) 통계적으로 유의한 차이를 보였다(p<0.05). Bland Altman plot으로 분석한 일치도는 높지 않았다. 결론: 여러 가지 기기로 측정한 앞방깊이 및 중심각막두께는 모두 상관성은 높으며, 앞방깊이는 유의한 차이가 없었으나, 중심각막두께는 유의한 차이를 보였다. 일치도는 두 군에서 모두 높지 않았다. <대한안과학회지 2016;57(10):1570-1576> Purpose: To compare the accuracy and clinical usefulness of different devices by measuring anterior chamber depth (ACD) with three devices and central corneal thickness (CCT) with four devices. Methods: In 180 eyes of 90 healthy subjects, ACD was measured using A-scan, Lenstar LS900??, Pentacam??, and CCT was measured using ultrasound pachymetry (USP), Lenstar LS900??, Pentacam??, and anterior segment optical coherence tomography (OCT). Results: The average ACT measurements using Lenstar LS900??, A-scan, and Pentacam?? were 3.27 ± 0.35 mm, 3.26 ± 0.36 mm, and 3.25 ± 0.36 mm, respectively. The measurements were significantly correlated (p < 0.001) but without statistically significant difference (p = 0.017). The Bland-Altman plots showed a low degree of agreement. The average CCT measurements using Pentacam??, USP, Lenstar LS900??, and OCT were 553.31 ± 25.23 μm, 547.26 ± 23.83 μm, 541.38 ± 24.49 μm, and 531.40 ± 22.33 μm, respectively. The measurements were significantly correlated (p < 0.001) and statistically significantly different (p < 0.05). The Bland-Altman plots showed a low degree of agreement.         경두개 도플러초음파를 이용한 중심장액맥락망막병증 환자의 안동맥 혈류속도 측정

        목적: 경두개 도플러 초음파를 이용하여 중심장액맥락망막병증 환자의 이환된 눈과 무증상 반대편 눈, 장액성망막박리의 관해를 보인 눈에서 안동맥 혈류속도를 측정하고 이를 비교하고자 하였다. 대상과 방법: 31명(20-52세)의 중심장액맥락망막병증으로 진단받은 환자에서 power motion mode Doppler 150 digital transcranial Doppler 초음파를 이용하여 이환된 눈과 반대편 눈의 안동맥 최대수축기혈류속도, 최대이완기혈류속도 및 저항지수를 측정하였다. 2개월 경과 후, 장액성망막박리가 소실된 23안에서 혈역학적 지표를 재측정하였다. 결과: 이환된 눈의 최대수축기 및 최대이완기혈류속도는 각각 30.05 ± 5.34 cm/sec, 14.07 ± 2.90 cm/sec로 반대편 눈의 측정치 33.03 ± 2.00 cm/sec, 17.17 ± 2.76 cm/sec에 비해서 낮았으며 이는 통계적으로 유의하였다(각각 p=0.004, p〈0.001). 저항지수는 이환된 눈에서 0.52 ± 0.08, 반대편 눈에서 0.43 ± 0.04로 이환된 눈에서 유의하게 높았다(p〈0.001). 2개월 경과 후, 관해를 보인 23안에서 최대수축기 및 최대이완기혈류속도는 32.94 ± 6.24 cm/sec, 15.54 ± 2.88 cm/sec로 처음 이환 시에 비해 높았으며 통계적으로 유의하였다(각각 p=0.031, p=0.045). 저항지수는 0.48 ± 0.04로 낮았으나 통계적 유의성은 없었다(p=0.073). 결론: 중심장액맥락망막병증 환자의 이환된 눈은 무증상 반대편 눈에 비해 안동맥 혈류속도의 감소, 저항지수의 증가 소견을 보였고, 이환된 눈의 관해에 따라 혈류속도 측정치가 증가하였으며 이는 중심장액맥락망망병증의 발생기전 및 병태생리에 안동맥의 혈류인자가 관련이 있음을 시사한다. Purpose: To measure and compare the ocular blood-flow velocity of ophthalmic artery in patients with central serous chorioretinopathy (CSC); affected eyes, asymptomatic fellow eyes, and CSC resolved eyes using transcranial Doppler ultrasonography. Methods: A total of 31 patients (age 20-52 years) with CSC were evaluated using power motion mode Doppler 150 digital transcranial Doppler ultrasonography. The peak systolic velocity (PSV), peak diastolic velocity (PDV), and resistance index (RI) were measured in the ophthalmic artery of both affected and asymptomatic fellow eyes. After 2 months, 23 patients with resolved serous retinal detachment underwent repeated measurement of the above hemodynamic indices. Results: The PSV and PDV of the ophthalmic artery were 30.05 ± 5.34 cm/sec and 14.07 ± 2.90 cm/sec in affected eyes and 33.03 ± 2.00 cm/sec and 17.17 ± 2.76 cm/sec in asymptomatic fellow eyes, respectively. Both indices were significantly lower in affected eyes (p = 0.004, p 〈 0.001, respectively). The RI was 0.52 ± 0.08 in affected eyes and 0.43 ± 0.04 in fellow eyes, a significant difference (p 〈 0.001). After 2 months, in 23 eyes with resolved CSC, the PSV and PDV were 32.94 ± 6.24 cm/sec and 15.54 ± 2.88 cm/sec, respectively. Both indices were significantly higher than at baseline (p = 0.031, p = 0.045, respectively). However, RI was 0.48 ± 0.04 and was not significantly different (p = 0.073). Conclusions: The CSC-affected eyes showed lower ocular blood-flow velocity and higher RI than the asymptomatic fellow eyes. The ocular blood-flow velocity was increased as CSC was resolved. These findings provide insights into the ophthalmic arterial vascular factors related to the pathogenesis of CSC.
