        후두개절제환자에서 시술한 인두위문합술 -1예 보고-

        The patient was 21-year old male who had gastrostomy and tracheostomy after swallowing lye-stuff in July 1971. He could restore his normal voice and breathing after removal of his destructed epiglottis obstructing his upper airway two years later. Pharyngogastrostomy was performed in Nov 1973. The esophagus which was totally obliterated in its full length was removed and the stomach was brought high up to the level of pharynx where it was anastomosed to the posterior wall of pharynx. His postoperative course was temporarily complicated by aspiration of small food into trachea which could be completely relieved with training, and he is doing his normal life quite well on the follow-up.

        원발성 폐 암육종

        A 45-year-old male was admitted to Department of Thoracic Surgery, Korea University Hospital with the chief complaints of cough and high fever of two months duration. His chest roentgenogram revealed homogenous ill-defined increased density in right lower lung, and bronchogram showed the abrupt cut-off sign of the proximal portion of the right intermediate bronchus. Preoperative evaluation of this patient revealed no contraindication of the pulmonary surgery. So Radical Right pneumonectomy was performed under the preoperative impression of lung cancer. And post-operative course was uneventful. Pathologic examination of the resected lung revealed Carcinosarcoma without regional lymphnode metastasis.

        생체판의 제작 및 실험

        Treatment of valvular heart disease with valve replacement has been one of the most popular procedures in cardiac surgery recently. Although, first effort was directed toward the prosthetic valve, it soon became popular that bioprosthesis, the valvular xenograft, was prefered in the majority cases. Valvular xenograft has some superiority to the artificial prosthetic valve in some points of thromboembolism and hemolytic anemia, and it also has some inferiority of durability, immunologic reaction and resistance to Infection. Tremendous efforts were made to cover the inferiority with several methods of collection, preservation, and valve mounting of the porcine valve or pericardium of the calf, and also with surgical technique of the valvular xenograft replacement. Auther has collected 320 porcine aortic valves immediately after slaughter, and aortic cusps were coapted with cotton balls in the Valsalva sinuses to protect valve deformity after immersion in the Hanks' solution, and oxidation, cross-linking and reduction procedures were completed after the proposal of Carpentier in 1972. Well preserved aortic valves were suture mounted in the hand-made tissue valve frame of 19, 21, and 23 mm J.d., and also in the prosthetic vascular segment of 19 mm Ld. with 4-0 nylon sutures after careful trimming of the aortic valves. Completed valves were evaluated with bacteriologic culture, pressure tolerance test with tolerane gauge, valve durability test in the saline glycerine mixed solution with tolerance test machine in the speed of 300 rpm, and again with pathologic changes to obtain following results: 1. Bacteriologic culture of the valve tissue in five different preservation method for two weeks revealed excellent and satisfactory result in view of sterilization including 0.65% glutaraldehyde preservation group for one week bacteriologic culture except one tissue with Citobacter freundii in 75% ethanol preserved group. 2. Pressure tolerance test was done with an apparatus composed of V-connected manometer and pressure applicator. Tolerable limit of pressure was recorded when central leaking jet of saline was observed. Average pressure tolerated in each group was 168 mmHg in glutaraldehyde, 128 mmHg in formaldehyde, 92 mmHg in Dakin's solution, 48 mmHg in ethylene oxide gas, and 26 mmHg in ethanol preserved group in relation to the control group of Ringer's 90 mmHg respectively. 3. Prolonged durability test was performed in the group of frame mounted xenograft tissue valve with 300 up-and-down motion tolerance test machine/min. There were no specific valve deformity or wearing in both 19, 21, and 23 mm valves at the end of 3 months (actually 15 months), and another 3 months durability test revealed minimal valve leakage during pressure tolerance test due to contraction deformity of the non-coronary cusp at the end of 6 months (actually 30 months) in the largest 23 mm group. 4. Histopathologic observation was focussed in three view points, endothelial cell lining, collagen and elastic fiber destructions in each preservation methods and long durable valvular tolerance test group. Endothel ial cell lining and collagen fiber were well preserved in the glutaraldehyde and formaldehyde treated group with minimal destruction of elastic fiber. In long durable tolerance test group revealed complete destruction of the endothelial cell lining with minimal destruction of the collagen and elastic fiber in 3 month and 6 month group in relation to the time and severity. In conclusion, porcine xenograft treated after the proposal of Carpentier in 1972 and preserved in the glutaraldehyde solution was the best method of collection, preservation and valve mounting. Pressure tolerance and valve motion tolerance test, also, revealed most satisfactory results in the glutaraldehyde preserved group.

        양대혈관 우심실 기시증: 폐동맥협착 동반례의 수술 치험

        The clinical findings with cardioangiography and successful surgical treatment of a 10 year old girl with double-outlet right ventricle is reported at The Dept. of Thoracic and Cardiovascular Surgery, Korea University Hospital, College of Medicine. The patient has been suffered from intermittent cyanosis, palpitation, and exertional dyspnea since 1 year after NFSD, and a holosystolic ejection murmur of grade 4 at the left 3rd intercostal space with mild cyanosis of the lips was the only physical findings at the time of this admission. Cardiac catheterization revealed ventricular septal defect with left to right shunt of 43% and right to left shunt of 10.2%. On cardioangiography from the left ventricle revealed all of the left ventricular outflow shunted into the right ventricle through the large ventricular septal defect, and the aorta originated from the infundibular chamber of the right ventricle with left, anterior sided pulmonary artery. The atria, viscera, and ventricles were normally located, and right ventricular out-flow was narrowed with infundibular hypertrophy and pulmonary valvular stenosis. Surgical correction was accomplished by closure of the ventricular septal defect in such a way that left ventricular outflow was routed via a Teflon felt prosthetic tunnel to the aorta, and pulmonary valvulotomy with infundibulectomy Was done to pass Hegar`s dilator No. 15 for reconstruction of the right ventricular outflow tract. The patient tolerated complete repair and has continued to improve over a period of three months after operation with normal school life. Details of the disease and method of repair are presented with related references.

        폐면균증(肺麵菌症)의 외과적(外科的) 치료(治療) -일례(一例) 보고(報告)-

        A 33 year old male patient was admitted with 20 years history of recurrent hemoptysis. On clinical examination, mild left chest discomfortness and foul odored sputum with occasional rusty hemoptysis were principal complaints noted. Chest X-ray film revealed moderately advanced active tuberculosis lesion on both upper lung fields, and hen-egg sized mass surrounded with linear crescent of air shadow in a cavity on his left upper lung field. On left thoracotomy, dense pleural adhesions on left apicoposterior segmental surface with multiple lymphnode enlargements were noted, and the soft encapsulated mass of $5{\times}5{\times}8cm$ was localized in the apicoposterior segment of the left upper lobe. Apicoposterior segment with anterior segment of the left upper lobe was resected. Cavity was opened to find a rusty grayish colored, fragile mass, which was confirmed as "fungus ball" of aspergillosis by histological section slide with Gomori staining. The authors report one case of pulmonary aspergilloma superinfected with previous long standing pulmonary tbc.

        The clinical cases of twenty consecutive intracardiac operations under the cardiopulmonary bypass with the disposable bubble oxygenator of RyggKyvsgaard and the Sigma Motor pump during past 3 months were reviewed to evaluate the effects of the extracorporeal circulation on the body by observing the hematologic and metabolic studies. The hematological examinations observed periodically revealed the effect of hemodilution with the lowered hemoglobin and- hematocrit values with which patients did well, and the tendencyy of mild anemia continuing even on dismissal from hospital. The platelet counts were decreased markedly along with the bypass and remained rather low after the operation in most cases. However, these. alterations did not result in clinically related significant complications. On the other hand, the changes in the serum electrolytes were minimal being kept in the near physiological range including calcium and magnesium levels_ Though, additional amounts of potassium were administered to keep the serum. potassium level a little high to avoid possible occurrence of cardiac arrhythmias after surgery. The metabolic effects of extracorporeal circulation on the body were studied with the arterial blood gas analyses performed on the blood samples taken periodically before, during and after bypass. The general tendency was toward moderate respiratory alkalosis during perfusion and mild metabolic acidosis with the result of keeping the pH values within the normal range. The respiratory component of the alterations were mainly the moderately lowered carbon-dioxide tension in the blood during perfusion rising only to normal level with the termination of the bypass. It was suggested that the addition of small amounts "of carbon dioxide to the oxygenator along with the sufficient amount of 'oxygen -may be beneficial to avoid these potentially dangerous respiratory imbalance .during perfusion. The metabolic acidosis present before perfusion weremuch corrected toward normal with perfusiorz'-probably from the result of bicarbonate .added during and after surgery. However, these metabolic acidosis were not so :severe that extraordinary amounts of bicarbonate should be given for the cor section. The average amount of bicarbonate given during and after perfusion was 133.8 mEq. with several cases getting a little more because of the prolonged perfusion. On the whole, the cardiopulmonary bypass was carried out successfully with the only mild deviation of the observed data from normal physiological values. There were 6 hospital deaths, 30% hospital mortality, 3 out of 10 cyanotic heart ,diseases, 2 out of 4 ventricular septal defects, none of 3 atrial septal defects and 1 out of 4 prosthetic valve replacements in 3 cases. The complications seen in the survivors from surgery were not definitely related to the bypass itself.
