Purpose: Nasal bone fracture is the most common type of facial bone fracture and most of nasal bone fracture is combined with septal fracture frequently. Nasal septum is important to support the distal nose and to maintain the nasal airway. But nasal septal fractures are usually unrecognized and untreated at the time of operation. Recently, various materials were using for nasal packing after closed reduction, however these materials are not focused on the correction of nasal bone and nasal septal fracture and many patients are suffered from nasal packing materials. Thus, the purpose of this study is to compare routine packing materials and rolled silastic sheet with respect to postoperative effect of correction of nasal bone fracture and discomfort of nasal packing materials. Methods: We examined 320 patients treated nasal bone fracture from January 2008 to December 2010. For Group I(n=92), Merocel^Ⓡ was used for nasal packing, for Group II(n=152) vaseline gauze was used, and Rolled silastic sheet(RSS) with vaseline gauze packing(VGP) was used for Group III(n=76). Under the general anesthesia, all patients were operated by closed reduction and nasal packing was done using three kinds of packing materials. At the time of postoperative 7 days, packing material was removed and studied for pt’s satisfaction and postop. complications. Results: In patients with RSS with VGP, the complaints (nasal obstruction, foreign compressive sensation and discomfort during food ingestion) of keeping the nasal packing were decreased(p<0.05) and the postoperative complication(deviation) were decreased comparing to vaseline gauze packing and Merocel^Ⓡ packing, however, these differences were not statistically significant(p>0.05). Conclusion: Postoperative nasal packing with RSS with VGP was more comfortable to the patients and it was more effective method to correct the nasal bone fracture and nasal septal fracture.

하안검 성형수술 후 발생한 양측 안구 뒤 혈종의 치험례

Retrobulbar hemorrhage is a rare but serious complication after blepharoplasty, mid-face injury, and treatment of facial bone fractures. The incidence of postoperative retrobulbar hemorrhage is 0.055% with an incidence of associated permanent blindness of 0.005%. A 69-year-old male came to the emergency room with pain on both orbital areas and uncontrolled bleeding after cosmetic lower blepharoplasty performed at a private clinic. He had not been evaluated preoperatively by the private clinic, but we found that he had undergone percutaneous transluminal coronary angioplasty and taken anticoagulants for 10 years. We performed an emergency operation to evacuate the hematoma. However, after surgery, he persistently complained of orbital pain, pressure and diminished visual acuity. Intraocular pressure was increased, and computed tomography demonstrated a retrobulbar hemorrhage with globe displacement. Emergent lateral canthotomy and cantholysis were performed. Intraocular pressure was decreased to 48/30 mm Hg immediately after the operation, falling within the normal range the next day. We recommend three points to minimize loss of vision by retrobulbar hematoma. Firstly, careful preoperative evaluation must be conducted including current medications, underlying diseases and previous history of surgeries. Secondly, cautious postoperative observation is important for the early diagnosis of retrobulbar hematoma. Lastly, immediate treatment is crucial to prevent permanent blindness.

Prevalence and Risk Factor of Neck Pain in Elderly Korean Community Residents

Neck pain is a common musculoskeletal condition, which causes substantial medical cost. In Korea, prevalence of neck pain in community based population, especially in elderly subjects, has scarcely been reported. We evaluated the prevalence, the severity and the risk factors of neck pain in elderly Korean community residents. Data for neck pain were collected for 1,655 subjects from a rural farming community. The point, 6-months and cumulative lifetime prevalence of neck pain was obtained in addition to the measurement of the severity of neck pain. The mean age of the study subjects was 61 yr and 57% were females. The lifetime prevalence of neck pain was 20.8% with women having a higher prevalence. The prevalence did not increase with age, and the majority of individuals had low-intensity/low-disability pain. Subjects with neck pain had a significantly worse SF-12score in all domains except for mental health. The prevalence of neck pain was significantly associated with female gender, obesity and smoking. This is the first large-scale Korean study estimating the prevalence of neck pain in elderly population. Although the majority of individuals had low-intensity/low-disability pain, subjects with neck pain had a significantly worse SF-12 score indicating that neck pain has significant health impact.

Inverted nipples have shortened lactiferous ducts and less fibrous and collagenous muscular tissue and less connective tissue present, thus giving less bulk and less tissue for nipple projection. Until now, many authors suggested various methods of correction of inverted nipple, but it shows that perfect method does not exist. From July 2008 to December 2010, 41 nipples in 21 patients were treated. 20 patients had bilateral inverted nipples. Under the local anesthesia with sedation, the nipple was everted. A small incision was made on both lateral side at the nipple-areola junction. After nipple traction, the lactiferous ducts and surrounding connective tissues were divided by sharp dissection only through vertical direction. Upper and lower purse-string sutures using Nylon 4-0 were performed for the maintenance of corrected nipple. The small incision was closed by the simple suture. The operation time averaged 15minutes for each nipple. The mean follow-up period was 6months. There were no complications associated with surgery, such as recurrence, infection, hematoma, nipple necrosis and sensory loss. We applied modified purse-string sutures to mild and moderate inverted nipple patients, and acquired good results. So we report our experience with a review of literature. Malignant salivary gland neoplasms constitute approximately 1% to 3% of all head and neck malignancies and only 0.3% of all malignant neoplasms. Malignant mixed tumors represent 2.2% of all salivary gland tumors. We experienced a huge malignant mixed tumor in the parotid gland, and obtained good results after surgical excision of the parotid gland and reconstructive operation. Therefore, we report on our experience and review the articles related to that tumor.

니체의 ‘힘에의 의지’ : 자유의지론과 결정론을 넘어서

니체의 자유의지론 비판은 잘 알려진 사실이다. 이 때문에 그는 결정론, 운명론, 비합리적인 신비주의의 옹호자로 종종 오해된다. 그러나 니체는 자유의지론도 결정론도 옹호하지 않으며, 그 이분법적 극단을 넘어 생리-심리학에 근거한 ‘힘에의 의지’라는 제3의 영토를 그려 보여준다. 니체는 자유의지론에 담긴 권력의지를 계보학적으로 추적하고 자유의지 개념의 허구성을 드러낸다. 또한 그는 인간의 부자유 의지를 주장하는 결정론을 자연과학적, 도덕적 심급에서 각각 비판하여 그것이 결국 숙명론으로 연결되는 약자의 도덕이자 데카당 도덕임을 드러낸다. 니체는 힘에의 의지로서의 이 세계의 유물론적인 메커니즘을 간파한다. 그러나 그것이 법칙을 갖는다는 것을 부정한다. 그는 법칙이라는 것은 인간이 만들어낸 기호 체계를 자연에 투사한 것에 불과하다고 보며, 필연적 법칙이 세계를 지배한다는 관념을 부정한다. 따라서 니체가 바라본 힘에의 의지로서의 이 세계는 어떠한 통일적인 법칙도 없이 다양한 힘에의 의지들이 전개되고 분화되어 나가는 끊임없는 생성의 과정일 뿐이다. 창조란 바로 이러한 철학적 관점의 토대 위에서 가능해진다. Nietzsche’s critique of the Theory of Free Will is well known. Because of that, he is often misunderstood as a supporter of determinism, fatalism, and unreasonable mysticism. However, Nietzsche does not support either the theory of free will or determinism and travels beyond the extreme of a dichotomy, so presents the third territory called ‘Will to Power’ based on physio-psychology. Nietzsche genealogically traces the will to power in the theory of free will and reveals the falsities of the concept of free will. In addition, he criticizes determinism which advocates a human’s lack of free will(unfree will) in natural science and on a moral level, hence he discloses that the determinism leading to fatalism is not only a moral of the weak but also a moral of décadent. He penetrates the material mechanism of the world as the will to power, but he denies it has laws. He regards laws as the things that human beings invent. These laws are no more than a system of signs applied to nature and Nietzsche denies this idea that necessary laws govern the world. Therefore, this world as will to power by Nietzsche is a constant, becoming course which various wills to power develop and differentiate without any unified laws. Creation can be possible on the basis of such a philosophical perspective.

극 배치 기법을 기반으로 한 단상인버터의 제어기 설계

This paper presents a PID controller and PI-PI dual loop controller for a single-phase inverter. The control parameters of conventional controllers are very difficult to make. But parameters of controllers based on pole-assignment are easy to make. Simulation results are presented for the system under R-L load, no load and change reference voltage.

통풍 환자의 요산억제제에 대한 순응도

Objective: Gout is crystal-induced arthritis with hyperuricemia. Uric acid lowering agent (UALA) is the maintenance drug of its treatment. Drug adherence is an important factor that influences treatment outcome. The purpose this study was to examine the drug adherence to UALA of 303 gouty patients and to analyze the factors associated with compliance. Methods: We retrospectively assessed adherence to UALA in 303 patients who visited three hospitals affiliated with Hallym University. Patients were diagnosed as gout and used UALA with at least 6 month follow-up. Two adherence measures were calculated, the percentage of days covered (PDC) and the time until an extended break (at least 60 days) in treatment. A PDC <70% was considered poor adherence and factors associated with poor adherence were examined. Results: Among the 303 patients, 283 (93.4%) were male. Mean age was 55.4±13.7 years. Comorbidities included hypertension in 117 (38.6%), diabetic mellitus in 108 (35.6%), angina in 107 (35.3%), myocardial infarction in 108 (35.6%), and heart failure in 113 (37.3%). The mean PDC was 65.8% with 63.4% of patients considered poorly compliant over the study period. Mean treatment duration before an extended break occurred was 203.5 days. Factors associated with good adherence included hypertension and prescription from rheumatologist. Conclusion: In our study, adherence to UALA in gouty patient was poor. Understanding the factors associated with adherence to UALA and proper education of gouty patients to improve drug adherence are needed.
