          Parameter Optimization for Applying the Prepulse Gap Paradigm to Humans

          Background and Objectives: Turner and colleagues introduced a new method that can detect tinnitus in animals. The stimulus is composed of a small background noise that is identical to the pitch of the tinnitus and a large pulse noise that can evoke a startle response. In normal rats, the gap decreases the startle reflex. However, in tinnitus rats, the gap does not decrease the startle reflex. The goal of this study was to optimize the stimulation paradigm so that the prepulse inhibition of N1-P2 amplitude would be maximized in the normal human subjects. Subjects and Methods: Seven normal control subjects without tinnitus were recruited. The stimulus was composed of two different sounds: the softer background noise and the louder pulse noise. A 50 msec silent gap was inserted before the pulse noise as the gap condition (G condition) but not in the no-gap condition (N condition). The averaged amplitude of the N1-P2 cortical response was recorded for the G and N conditions. Results: The G/N ratio was the smallest when the gap was 20 msec prior to the pulse noise. The G/N ratio was 84.8±16.8% with the Hanning window and 78.5±5.9% without the window. The G/N ratio was 91.1±24.9%, 78.0±5.4%, and 79.0±18.1% when the intensity of the background noise was 10, 20, and 32 dB SL, respectively. When the intensity of the background noise was 20 and 32 dB SL, the N1-P2 amplitude of the G condition was significantly smaller than that of the N condition. Conclusions: The optimal stimulus should be composed of the 1 kHz pulse noise without Hanning window. The intensity of the background noise should be 20 dB HL and the location of the gap should be 20 msec prior to the pulse noise. It seems that with these optimized parameters we could expect a 78.0% inhibition of N1-P2 amplitude in normal subjects without tinnitus.

          Long Term Outcomes of Early Cochlear Implantation in Korea

          Objectives. The objective of this study was to compare the long-term auditory performance and language skill depending on the age of cochlear implantation in the Korean population. We especially tried to separate the effect of maturation/ development from that of the age at implantation. Methods. Eighty-six pre-lingual children with profound hearing loss who underwent a cochlear implantation before the age of six and had been followed for more than 3 yr were included in this study prospectively. Categories of Auditory Performance (CAP) and Korean Picture Vocabulary Test (K-PVT) were serially followed up. In order to separate the age at implantation effect, K-PVT results were readjusted to the child's chronological age in the normal hearing population. Results. When the CAP and K-PVT scores were directly compared without chronological readjustment, we failed to show a significant difference for improvements according to the age at implantation. Early cochlear implantation was associated with better language development, only when the K-PVT scores were readjusted to percentile scores of their chronological age. Conclusion. Early cochlear implantation was associated with better language development even within the critical period. This advantage may be recognized only when the effect of the age at implantation is separated from the effect of maturation/development. Objectives.           방사성동위원소 폐기물의 국내육상운반에 관한 방사선영향 평가

          경주 중·저준위 방사성폐기물 처분시설의 운영에 따라, 한국원자력연구원(대전)에 임시보관 중인 방사성동위원소폐기물을 처분시설로 육상운반하였다. 본 연구에서는 방사성동위원소폐기물의 국내육상운반에 따른 작업자 및 일반인에 대한 방사선 피폭선량을 평가하고 그 결과를 국내 방사선피폭 법적제한치와 비교하였다. 또한 방사성폐기물의 상하차 작업 시 작업자와 드럼 간 거리 및 방사성핵종 누출율의 변화에 따른 예상피폭선량의 민감도를 분석하였다. 정상 및 사고조건에서의 예상피폭선량은 국내 법적제한치를 충분히 만족하였음을 확인하였다. Korea Radioactive Waste Agency (KORAD) began to operate the low and intermediate level radioactive waste disposal facility in Gyeongju and to transport the radioactive waste containing radioactive isotopes from Daejeon to the disposal facility for the first time at 2015. For this radioactive waste transportation, in this study, radiological impact assessment is carried out for workers and public. The dose rate to workers and public during the transportation is estimated with consideration of the transportation scenarios and is compared with the Korean regulatory limit. The sensitivity analysis is carried out by considering both the variation of release ratios of the radioactive isotopes from the waste and the variation of the distances between the radioactive waste drum and worker during loading and unloading of radioactive waste. As for all the transportation scenarios, radiological impacts for workers and public have met the regulatory limits.

          Abstract - Nowadays, the usage rate of LCD beam projector is being increased because of good connection with a computer and various functions. But even though it has many functions, it is almost used only for display. Especially, in case of presentation user's action is limited by using mouse or remote controller. So in this paper pc-camera is used for more efficient usage of the beam projector and convenient user's action. And we have done research an indicator to carry out instruction transmission after fing out position from pc-camera and detect a position which is pointed by it.

          메니에르병에서 내림프낭감압술 후 장기 추적관찰에 따른 치료 결과

          The treatment result of endolymphatic sac decompression (ELSD) is controversial especially after a long term follow-up period. The aims of this study are to review the long term treatment outcome of ELSD and to analyze the factors associated with the long term prognosis. Subjects and Method:A retrospective review of medical records was performed. Sixteen patients (18 ears) who had been diagnosed with definite Ménière's disease, had undergone ELSD and been followed up for more than 2 years were included in this study. The average follow up period was 88.6 months, with the range spanning from 25.7 to 243.4 months. All results were described according to the 1995 AAO-HNS criteria. Results:After a long term follow-up, vertigo was successfully controlled in 66.6% and hearing was preserved or improved in 58.8%. Tinnitus and aural fullness were relieved in 27.3% and 60.0% respectively. The preoperative hearing threshold (p=0.03) and caloric test (p=0.05) showed a close relationship with the long term vertigo control after ELSD. Conclusion:Although vertigo and hearing may deteriorate again after 2 years, ELSD generally seems to be able to control vertigo even after a long term follow up. Preoperative hearing threshold and caloric test may be able to serve as prognostic factors. (Korean J Otorhinolaryngol-Head Neck Surg 2008;51:319-25)
