≪겨레말큰사전≫과 통일국어교육 - 『초등 국어 저학년 우리말 틀』을 중심으로 -

This thesis seeks to analize the "Framework for Korean Words used in Lower Grades of Elementary School" and review the development tasks in post-reunification Korean education for elementary school students. The Gyeoremalkeunsajeon Committee has to date held a total of 25 joint inter-Korean talks and agreed on the items needed to publish a dictionary. This process is still underway. In the category of orthography in the “Framework for Korean Words”, the suffixes of ‘-kkun/-gun’ and ‘pye/pe’ need to be disposed of, while the standard pronunciations for words need to be distinguished for North and South Korea. In the category of vocabulary, more words need to be included in the basic vocabulary needed for learning, such as music terms, art terms, grammar terms and synonums that have the same meaning and form but different frequency of use in North and South Korea. In addition, learning is needed on vocabulary that have the same form but differ in terms of meaning or use. In other words, vocabulary that have the same form and same conceptual meaning, but with a different analogical meaning due to the different socio-cultural backgrounds may be considered to be included, along with words that have a single meaning in South Korea but carry multiple meanings in North Korea, and words that have a different use in honorifics.

최근 골절 치료에 있어서 내고정은 기계적 중요성에서 생물학적 중요성으로 그 개념이 두드러지게 발전해 왔으며, 이를 위하여 LISS나 LCP와 같은 새로운 개념의 내고정 기구가 개발되었고 새로운 수술 기법으로 생물학적 고정 방법이 발전되어 왔다. MIPO는 생물학적 고정으로 골절 부위를 노출 시키지 않고 간접적으로 골절을 정복하고 최소 침습적 방법으로 LISS나 LCP와 같은 내고정 기구를 골막외 근하 또는 피하에 삽입하여 유연성 고정을 함으로써 조기에 가골 형성과 골유합을 얻는 술식이다. MIPO 기법을 통하여 골유합률을 높이고, 골이식의 빈도를 줄이고, 재골절과 감염 같은 합병증의 빈도를 낮추어 왔으나, MIPO 술식과 관련되는 부정 정렬이나 신경혈관 손상과 비록 빈도는 적으나 감염이나 골유 합의 장애 등의 합병증이 발생할 수 있다. 이러한 합병증들을 피하고 조기에 가골 형성으로 인한 골유합과 기능 회복을 얻기 위해서는 내고정의 안정성에 따른 골절의 치유 과정과 내고정의 생역학과 내고정물의 특성을 이해하고, 철저한 술 전 계획을 세우고, 간접 정복과 생물학적 고정 술식에 익숙하고, 술 후 처치를 잘 계획하여야 한다.

This study is to find out the effects of participation in job and social activity support projects for the elderly on life satisfaction, successful aging was verified. For this, a total 110 people were surveyed from October 2022 to November 2022 and analyzed using the spss 25.0 program As a result of the study, job participation for the elderly had a positive effect on successful aging, and the mediating effect of life satisfaction on the relationship between job participation, successful aging of the elderly was verified. This study is meaningful in that it confirmed that the elderly job project could be an alternative to support stable old age for the elderly.

광범위 외측 접근법으로 수술한 종골 골절의 술 후 조기 합병증

목적 : 광범위 외측 접근법으로 수술한 종골 골절에서 술 후 조기 합병증들을 분석하고 창상 합병증에 대한 위험 요인들을 알아보았다. 대상 및 방법 : 1990년 7월부터 2003년 2월까지 광범위 외측 접근법으로 수술한 종골 골절 104명, 116예를 대상으로 하였다. 환자들의 병록지에서 술 후 조기에 발생한 합병증들을 조사하여 분석하였으며, 창상 합병증의 위험을 증가시키는 요인들을 알아보기 위해 예견되는 변수들에 대하여 통계학적 분석을 하였다. 결과 : 감염이 14예 (12.0%)에서 발생하였으며, 10예 (8.6%)는 표재 감염이었고 4예 (3.4%)는 삼부 감염으로 수술적 치료가 필요하였다. 피부 괴사가 8예 (6.9%)에서 발생하였으며, 6예 (5.2%)는 수술창 변역부의 피부 괴사였고 2예 (1.7%)는 피판 괴사로 수술적 치료가 필요하였다. 비복신경 손상이 7예 (6.0%)에서 발생하였으나 부가적인 치료 없이 회복되었다. 통계학적 분석 결과 개방성 골절 군(p=0.003)과 수술시간이 90분 이상 연장된 군 (p=0.049)에서 창상 합병증의 발생률이 유의하게 높았다. 결론 : 광범위 외측 접근법으로 수술한 종골 골절에서 술 후 조기 합병증의 발생률이 높으므로 개방성 골절에서는 개방창을 철저하게 치료하고, 술 전 계획과 계획된 술기를 통하여 수술 시간을 90분 이내로 단축하고, 비복신경의 해부학적 위치를 고려하여 절개함으로 술 후 조기 합병증의 발생률을 감소시킬 수 있을 것으로 사료된다. Purpose : To analyze early postoperative complications of calcaneal fractures operated by a lateral extensile approach and to identify risk factors for would complications. Materials and Methods : From July 1990 to February 2003, 116 calcaneal fractures in 104 patients were treated by open reduction and internal fixation through a lateral extensile approach. The patient's records were reviewed for early postoperative complications. Statistical analysis was performed to determine significant relationships between predicted variables and the development of would complications. Results: Fourteen fractures (12.0%) developed infection. Ten of them were superficial infection and four were deep infection that required surgical treatment. Eight fractures (6.9%) developed skin necrosis. Six of them were marginal skin necrosis and two were flap necrosis that required surgical treatment. Seven fractures (6.0%) developed sural nerve injury, but their symptoms were improved without additional treatment. Open fracture (p=0.003) and prolonged operating time (p=0.049) increased significantly the rate of wound complications. Conclusion: The rate of early postoperative complications of calcaneal fractures operated by a lateral extensile approach is high. These complications can be reduced by meticulous treatment of an open wound, reduced operating time within 90minutes through preoperative planning and skillful technique, and correct incision to avoid damage of the sural nerve.

골수내 정 고정술은 여러 장점들이 있어 근위 경골 골절의 치료에 널리 선택되는 유용한 치료 방법이나 경골근위부의 넓은 골수강과 근의 견인력으로 인하여 골수내정 고정술 후 높은 빈도의 부정 정렬이 발생하므로 술식 중에 상당한 주의가 요한다. 그러므로 근위 경골의 해부학적 특징과 골수내 정의 구조를 잘 이해하고 술전 계획을 잘 세워 부가적인 술식과 함께 골수내 정의 삽입 술식을 정확하게 시행하여야 부정 정렬을 방지하고 만족스러운 결과를 얻을 수 있다.
