        요양병원 간호사의 간호행위 위임정도, 간호전문직관 및 이직의도와의 관계

        This study was aimed to prepare the evidences for establishing policies on nursing manpower by identifying the relationship among delegation level of nursing activities, nursing professionalism and turnover intention in long-term care hospital nurses. The study was a descriptive survey research that used a structured questionnaire. 146 nurses agreed to take part in the research. They worked in six long-term care hospitals located in A and B province. Data were collected from July to August 2016 and analyzed through SPSS 20.0 program, using descriptive statistics, t-test, ANOVA and pearson’s correlation coefficients. As a results, first, as for the delegation of nursing activities by long-term care hospital nurses, the levels of delegation of direct nursing activities were 34.63±14.12, and the level of delegation of indirect nursing activities were 1.71±1.75. Second, the mean score of nursing professionalism and the turnover intention of long-term care hospital nurses were 3.62±0.77 and 2.71±0.97, respectively. Third, the delegation of direct nursing activities by long-term care hospital nurses significantly differed depending on the number of nurses in the ward and the number of caregivers in the ward. The delegation of indirect nursing activities significantly differed depending on the number of nurses in the ward, the presence of conflicts and stress related to delegation. Forth, there was no significant correlation of the delegation of nursing activities by long-term care hospital nurses with nursing professionalism and with turnover intention. Most of nursing activities, were delegated to nurse aids, since there were no clear guidelines or legal procedures on nursing activities in long-term care hospital nurses. Therefore it is required to find how to clearly establish positive nursing professionalism and to develop a proper nursing manpower arrangement and the guidelines for the delegation of nursing activities in a long-term care hospital in terms of policy. 본 연구는 요양병원 간호사의 간호행위 위임정도, 간호전문직관 및 이직의도를 파악하고 이들간의 관계를 확인함으로써 향후 간호행위 위임에 필요한 정책적 지침 및 절차에 관한 기초자료를 제공하고자 시도되었다. 본 연구는 서술적 조사연구로 구조화된 설문지를 이용하여 2개 광역도 소재 6개 요양병원의 간호사 146명을 대상으로 실시하였으며, 자료수집기간은 2016년 7월부터 8월까지 이었다. 수집된 자료는 SPSS 22.0을 이용하여 기술통계, t-test, ANOVA, Pearson 상관분석을 실시하였다. 연구결과, 요양병원 간호사의 간호행위는 위임에 대한 명확한 지침이나 법적 절차가 없는 상태에서 7개 행위를 제외한 대부분의 간호행위가 간호보조인력에게 위임되 고 있었다. 요양병원 간호사의 간호행위 위임정도와 간호전문직관, 이직의도와의 유의한 관련성은 없었으나 간호전문직관과 이직의도는 유의한 음의 상관관계를 보였다. 따라서, 요양병원 간호사의 이직의도를 낮추기 위해서는 개인적인 측면으로 긍정적인 간호전문직관을 확고히 하는 방안이 모색되어야 하며, 정책적인 측면에서는 요양병원의 적절한 간호인력배치 기준을 마련하여 간호행위 위임지침을 개발해야 한다.

        딕토글로스 활동을 통한 초등학교 학습자의 듣기·쓰기 성취도 및 정의적 영역 변화 연구

        Dictogloss is a focus-on-form instruction task which contributes to the development of accuracy through meaning-focused listening and writing activities. This study is aimed at exploring possible changes of listening and writing achievement and affective domain when dictogloss activities were implemented to elementary students. For the study objective, forty-one fifth-grade elementary students participated in eight forty-minute sessions of dictogloss instruction for six weeks. Four types of activities were used for dictogloss lessons: traditional dictogloss, picture dictation, partial dictation dictogloss, and scrambled dictogloss. To gather research data, listening and writing tests were administered along with a survey questionnaire before and after the dictogloss classes. Since listening and writing tests used for this study were formative assessment, Wilcoxon matched pairs signed rank test, a nonparametric test for repeated t-test, was used. The main findings of the study are as follows: First, there was a significant improvement in students’ listening and writing achievement. Compared to listening, writing achievement has greatly improved during the experimental period. Second, the affective domain also significantly improved; in particular, students’ anxiety level decreased and confidence increased. Research and pedagogical implications are provided at the end of the paper.

        발달지체유아의 자기결정에 관련된 자신의 행동에 대한 특수교사와 발달지체유아 어머니의 인식

        The purpose of this study was to investigate the perception of special education teachers and mothers on their behaviors for self-determination of children with developmental delays The subjects of this study were 105 early childhood special educators and 99 mothers of children with developmental delays. The subjects responded to the survey questionnaire composed of 27 questions related to their behaviors for self-determination such as self-awareness, self-management, choice- making, self-advocacy, support network, community use, social skills, cooperative skills, and stress management. Multiple regression and independent t-test were used to analyze the data. The research findings were as follows: (a) The teachers with more experiences of teaching children with disabilities perceived that they conducted more activities to improve the self-determination skills of children with developmental delays. (b) The mothers having jobs conducted more activities to improve the self-determination skills of their children than the mothers not having jobs. (c) The teachers conducted more activities to improve the self-determination skills of children with developmental delays than the mothers. The implications of the research results were discussed. 본 연구의 목적은 발달지체유아의 자기결정에 관련된 자신의 행동에 대한 유아특수교사와 발달지체유아 어머니의 인식을 밝히는 것이었다. 연구대상인 특수교사 105명과 발달지체유아의 어머니 99명이 27문항으로 구성된 자기결정기술 척도를 이용하여 발달지체유아의 자기결정에 관련된 자신의 행동에 대한 인식을 보고하였다. 자료를 분석하기 위하여 회귀분석과 두 독립변인 t 검증을 실시하였다. 연구결과는 다음과 같다. 첫째, 장애아동 교육경력이 많은 유아특수교사일수록 발달지체 유아의 자기결정에 관련된 행동을 많이 한다고 인식하고 있었다. 둘째, 직업이 있는 어머니가 전업 주부인 어머니들에 비해 발달지체 유아의 자기결정에 관련된 행동을 많이 한다고 인식하고 있었다. 셋째, 유아특수교사가 발달지체유아의 어머니보다 발달지체유아의 자기결정에 관련된 행동을 많이 한다고 인식하고 있었다. 유아특수교사는 자기인식능력, 선택기술, 협력기술 순으로 발달지체유아의 자기결정에 관련된 행동을 많이 한다고 인식하고 있었고, 어머니는 자기인식능력, 스트레스해소기술, 지원망구성기술 순으로 발달지체유아의 자기결정에 관련된 행동을 많이 한다고 인식하고 있었다. 본 연구결과는 발달지체유아의 자기결정기술을 향상시키기 위한 학교-가정 연계프로그램을 구성하기 위한 기초자료로 활용될 수 있다. 후속연구를 통해 발달지체유아의 자기결정에 관련된 자신의 행동에 대한 아버지의 인식, 자기보고 자료와 직접관찰에 의한 자료의 비교, 유아의 장애유형이 교사와 부모의 행동에 미치는 영향 등을 밝힐 필요가 있다.

        두부외상후 정신행동 장애의 임상심리검사 결과

        We investigated neurobehavioural sequelae after head injury in consecutive 104 patients who were evaluated by psychometric examinations from 1990 to 1994. Organic brain syndrome(OBS), posttraumatic stress disorder(PTSD), and others were observed in 55 patients(52.9%), 37 patients(35.6%), and 12 patients(11.5%), respectively. OBS was more common when the initial Glasgow Coma Score was low or when there was an intradural demage(p<0.025). OBS and PTSD did not differ significantly in age, sex, cause of injury, interval from injury to psychometric examination, skull fracture, cranial operation, or the Injury Severity Score. In OBS, the mean verbal Intellegence Quotient(IQ) was 91.3, performance IQ 77.8, total IQ 84.9 and memory quotient 70.7. In PTSD, it was 101.4, 100.1, 100.9 and 77.8, respectively. Performance IQ and total IQ of OBS and PTSD differed significantly, but verbal and memory quotient did not. In OBS, the mean verbal IQ of patients with right hemispheric damage was generally higher than that of patients with the left hemispheric damage, except in vocabulary and comprehansion. But that of perfomance IQ was the reverse. In OBS, simplification, distortion, rotation, omit, impaired judgement, labile affect impulsivity, depression, flatness, apathy, and bluntedness were commonly observed. While somatization, hostility, frustration, faking bad, and retarded manner were more frequently found in PTSD. The burden imposed on the family is primarily attributable to neurobehavioral sequelae rather than motor or sensory deficits. We should concerned about the adequate management of neurobehavioral sequelae as much as the proper treatment of head injury during the acute period.
