        악안면 부위에 발생한 혈관종 및 혈관기형에 대한 후향적 연구

        The vascular origin diseases occurring in the oral and maxillofacial region can be divided into hemangiomas and vascular malformation. They are rare and early lesions are not easy to diagnosis, so they may be misdiagnosed as clinicians, or the method and timing of treatment may be inappropriate. For 10 years, from August 2005 to August 2015, we analyzed the clinical records, diagnosis, treatment, and prognosis of 23 patients with hemangioma or vascular malformation confirmed by clinical, histopathological or radiological examination of maxillofacial lesions at department of oral and maxillofacial surgery, Pusan National University Hospital and Pusan National University Dental Hospital. Chief complants of patients were mainly swelling, mass, and spot-like lesion on various intraoral sites. Treatments were performed with excision, embolization, sclerotherapy, depending on each lesions condition. Arteriovenous malformation cases were more frequently recurred than other lesions. Prognosis of lesions were relatively good but close observation should be needed.

      • 주관적 기억력 저하를 호소하는 노인 환자의 인지기능 양상

        Purpose:The purpose of this study was to find out cognitive function of the patients with subjective memory complaint. Material and Methods:From March 1st 2005 to May 31st 2009, 155 normal individuals without any medical illness who visited Yeungnam University Hospital to undergo medical checkup with neurocognitive test was enrolled, and checked by using Cognitive Assessment & Reference Diagnostic System. Results:107 of the patients had normal cognitive function, 21 patients (about 15%) were diagnosed with dementia, and 10 patients (about 7%) were diagnosed with considerable psychiatric illness, such as depression, anxiety disorder, adjustment disorder. In amnesia, agnosia, aphasia, attention, calculation, dysexecution, Dementia group and Psychiatric illness group has worse score than Normal individuals group. But, in apraxia, Dementia group has worse score than Psychiatric illness group and Normal individual group. Conclusion:Because the patients with subjective memory complaint can be diagnosed as any psychiatric illness as well as dementia, sensitive screening test and early psychiatric approach is needed.

        푸리에 방법을 결합한 타원편광분석법 전이점 연구

        Spectroscopic ellipsometry is an excellent technique for determining dielectric functions. A standard analytic CP expression is conventionally used for analyzing the second derivatives of the dielectric functions in order to obtain the critical point (CP) energy. When the signal-to-noise ratio is poor, the standard expression does not function well due to an enhancement of the high-frequency noise caused by the derivation. However, a reciprocal-space Fourier-transform analysis offers several advantages for determining the CP parameters in optical and other spectra. For example, low-, medium-, and high-index Fourier coefficients were classified with baseline, information, and high-frequency noise, respectively. The noise can be removed without distorting the information. In this work, we propose a standard analytic CP expression method combined with a noise-removal Fourier method. Noisy data synthesized from Lorentz oscillators were analyzed by using the proposed method to verify the significant improvement over the CP analysis alone. We could accurately verify an improvement over the CP analysis with the combined method. 타원편광분석법 (ellipsometry) 으로 측정되는 유전율 함수는 반도체 물질의 광특성과 전이점 연구에 유용하게 쓰인다. 유전율 함수로부터 전이점을 구하기 위해서는 보편적으로 이차미분 표준해석법을 이용한다. 그러나 측정된 유전율 함수의 노이즈가 심한 경우 이차미분법은 노이즈도 크게 증가시킬 수 있기 때문에 데이터 평탄화 (linear filtering) 처리가 필요한데, 과도한 평탄화 방법은 정보의 왜곡을 유발할 수도 있다. 본 연구에서는, 이차미분법에 푸리에 노이즈 제거 방법을 결합시켜 역공간 푸리에 변환 계수로부터 low-, medium-, high-index 를 각각 베이스라인 (baseline), 정보, 고주파수 노이즈로 구분하고 이 중에서 노이즈만을 효과적으로 제거함으로써 전이점 연구의 정확도를 높이고자 하였다. 이 방법의 효과를 평가하기 위해 로렌츠 오실레이터로 만든 가상의 유전율 함수에 노이즈를 추가한 스펙트럼으로 기존 방법들과 비교 분석하였고, 이차미분 표준해석법의 향상된 정확도를 확인하였다.

        The Retrospective Study of Closed Reduction of Nasal Bone Fracture

        Purpose: This study was conducted in order to investigate the therapeutic effect of closed reduction according to a classificationin patients with nasal bone fracture. Methods: The study was conducted retrospectively on 186 patients with a mean age of 38 years (range: 7 to 80 years). All patients were diagnosed by clinical and radiologic examination, and then classified according to Hwang’s classificationby computed tomography. The patients were further classified by their age, gender, causes of fracture, operation timingafter fracture, concurrent facial bone fracture, and complications. All patients underwent the same reduction and treatmentprotocol and were then followed up regularly for at least three months. Results: The cause of the fracture was slip down, and the highest prevalence was shown in the 20s. The mean operationtiming after fracture was 4.1 days (range: 1 to 14 days), and it tended to be longer in the case of defected septal boneor more severe fracture. The most common concurrent facial bone fracture was orbital blow-out fracture, and zygomaticomaxillarycomplex and maxillary fracture occured frequently. The largest number of complications occurred in class III and IIBs patients,and the main complication was postoperative pain. Conclusion: Results of nasal bone closed reduction on the 186 patients showed that serious complications rarely occurred. Closed reduction is generally an effective treatment for nasal bone fracture. However, in the case of severe concurrent septalbone fracture or comminuted fracture with depression, open reduction should be considered. Further study with a largernumber of patients and further classification is required.

        Proposal on the Diagnostic Criteria of Definite Isolated Otolith Dysfunction

        Background and Objectives: Dizzy patients with abnormal otolith function tests, despite a normal caloric response, are defined as having specific (isolated) otolith organ dysfunction. This study was performed to compare the differences in clinical presentation between isolated otolith dysfunction (iOD) patients with lab- and Sx-based iOD group and lab-based iOD symptoms. Subjects and Methods: The medical records of 23 iOD patients with normal caloric response but abnormal cervical vestibular evoked myogenic potential (VEMP), ocular VEMP, or subjective visual vertical were reviewed. Non-spinning vertigo was considered as otolith-related symptoms. The patients’ age, onset of dizziness, Numeric Rating Scale on the severity of dizziness, and concomitant vestibular disorders were analyzed. Results: Patients in the lab-based iOD group were significantly older than those in the lab- and Sx-based iOD group. Known vestibular disorders were significantly more common in the lab-based iOD group (83.3%) compared to the lab- and Sx-based iOD group (18.2%). Despite the normal caloric response, catch-up saccade was found in the video head impulse test in more than half (54.5%) of the lab-based iOD group patients. There was no catch-up saccade in the lab- and Sx-based iOD group. There were no significant differences in gender ratio, frequency of dizziness attacks, and duration of illness. Conclusions: We propose new definitions of definite iOD (lab- and Sx-based iOD) and probable iOD (lab- or Sx-based iOD). These new definitions may help researchers to identify patients who are more likely to have true iOD, and facilitate comparisons of results between different studies.
