        음주 습관에 따른 혈중 지질 농도의 차이 비교에 대한 단면연구: 제 7기 국민건강영양조사 자료 활용

        Objectives: This study compared serum lipid concentration according to drinking habits. Methods: We analyzed data from the 7th Korea National Health and Nutrition Examination Survey (KNHANES VII). The study included 8,525 adults (3,651 males and 4,874 females), aged 30–59years. Results: There were differences in age, gender, education level, smoking status, physical activity, and waist circumference between drinkers and abstainers. The serum low-density lipoprotein-cholesterol (LDL-C) level of the drinkers was lower than those of the abstainers (P < 0.05). The serum triglyceride (TG) and high-density lipoprotein-cholesterol (HDL-C) concentrations were highest in the group that consumed alcohol ‘more than twice a week’ relative to the other groups (P < 0.001). The LDL-C and atherogenic index (AI) levels were lowest in the ‘more than twice a week’ drinking group compared to the other groups (P < 0.001). The serum TG and HDL-C concentrations were the highest in the ‘7 glasses/ time’ group (P < 0.001). The serum LDL-C concentration was the lowest in the ‘7 glasses/ time’ group (P < 0.001). Notably, the higher the frequency of binge drinking (7 glasses or more), the higher the concentration of TG (P < 0.001). The serum HDL-C concentration was significantly higher in the ‘no binge’ and ‘more than once a week’ groups compared to the other groups (P < 0.001). The serum LDL-C concentration and AI score were the lowest in the ‘more than once a week’ group (P < 0.001). Conclusions: As the quantity and frequency of drinking increased, the serum TC con- centration increased. Moreover, an increase in the serum HDL-C concentration led to a decrease in AI. The factors exacerbating cardiovascular disease increased simultaneously due to drinking. Our results suggest that for individuals with hypertriglyceridemia and patients with low HDL-cholesterolemia, separate guidelines based on the quantity and frequency of alcohol consumption are warranted.

        Gargoylism

        It is becoming clear that the clinical entities of three diseases of Gargoylism, Morquio's disease and recently known Morquio-Ullrich's disease are ot sharply defined syndrome. Although inborn errors and metabolism of mucopolysaccharide become sharp and aid the diagnosis of gargoylism and Morquio-Ullrich's disease probably by keratosulfate of mucopolysaccharide but each may have other features clinically, particulary with Morquio's disease. Among these intermediate forms are difficult cases. It is in these cases radiology can help the diaegnosis. We are presenting five various gargoylism cases and its discussion.

        Rickets is known as rare disease nowadays. Among 269.649 concerned clinic patients in 6 and one half years since 1965 to June 1971 at Yonsei Univ. Severance Hospital, there were 33 cases of various rickets with annual increasing rate since 1968. Compared to the 1965 and previous years to 1968 almost 5 times increase in number noted. The incidence of rickets was 8171:1 and ratio was 0.012 percent. There was none specific sex difference. The analysis of rickets disclosed vitamin D deficient type to be highest incidence of 90.9 %. The rest was 3.03 % each of vitamin D resistant rickets, renal rickets, and infantile tetany. Among 33 cases of rickets X-ray study was done in 24 cases of 72.2%. All showed positive rachitic bony changes. Blood chemistry disclosed elevated alkaline phosphatase generally byt serum phosphorus and calcium was supportive evidence for rickets due to overlap with other idseases of malabsorption, starvation andvomiting in low serum phosphorus even I high alkaline phosphatase differentiation should be made in fracture of bone, hyperparathyroidism and metastatic bone disease however these factor can be eliminated some extend to the age prevalence in childhood of rickets. In X-ray study of among 24 cases, 21 of knee joints disclosed metaphyseal ricketic bony changes. And 13 cases in wristsm, 11 in ankles, 7 in long bones and 12 in pelvis all show rachitic changes in 100 percent, however, in thoracic cage of 12 cases of half only disclosed rosary, and 2 looser zones. Therefore X-ray study still stands best aid for diagositic value in rickets.

        연수경색에 의한 중심성 안면마비 2예

        According to the anatomical pathways of the corticobulbar tract, it is known that the responsible lesion site of central facial palsy is at the level of the midpons or the more rostral portion. In rare cases, central facial palsy is found in medullary lesions. We report two cases of medullary infarctions which presented as central facial palsy. These cases suggested that the hypothesis that part of the pathway of the facial corticobulbar fibers descend ipsilaterally to the lower medulla before decussating and ascending contralaterally to the facial nucleus.

        점성과 마찰형 감쇠를 가지는 동조질량감쇠기의 설계 최적화

        본 연구의 목적은 마찰력의 크기에 따른 동조질량감쇠기(Tuned mass damper, 이하 TMD)의 성능변화를 조사하고, 이에 기초 하여 TMD의 최적 설계 파라미터를 결정하는 것이다. 일반적인 TMD 설계는 레일의 마찰력을 최소화하는 것을 전제로, 진동수비와 감 쇠비의 최적값이 제시되어 있다. 본 연구에서는 선형점성, 마찰력, 그리고 점성과 마찰을 동시에 가지는 TMD에 대하여 조화하중과 랜덤하중을 사용한 수치해석을 통해 최적진동수비와 최적감쇠비의 변화를 조사하였다. 마찰력의 경우에도 점성감쇠와 같이 특정 크기까지는 제어효율이 증가하나, 특정 값 이상에서는 TMD 성능이 급격히 저하되는 특성을 가진다. 점성감쇠와 마찰력이 동시에 존재하는 경우, 마찰력이 증가함에 따라 최적감쇠비가 감소하였으며, 마찰력의 크기를 반드시 고려하여 최적감쇠비를 결정해야한다. 풍하중을 받는 76층 벤치마크 구조물에 설치된 TMD에 대한 설계를 통해 제안된 최적 파라미터가 제어성능을 향상시키는 데 있어 유효함을 확인하였다. The purpose of the study is to investigate the performance variation of a tuned mass damper(TMD) with frictional force and to determine optimum design parameters of the TMD. In previous study, optimal values of frequency ratio and damping ratio of the TMD have been suggested under assumption that the frictional force is minimized. In this study, the changes of optimum frequency ratio and damping ratio were investigated by numerical analysis using harmonic and random loads. General linear viscous damping, friction, and viscous-friction mixed models were considered. The frictional damping could enhance the control efficiency of the TMD until the frictional force reached an optimal value, such as the effect of the viscous damping, but the performance deteriorated sharply beyond the optimal value. When both the viscous and the friction damping existed, the optimum damping ratio decreased with increasing the friction. This implies that the optimum damping ratio should be determined by considering the magnitude of the friction. The TMD was designed using the proposed optimized parameters for the 76-story benchmark structure subject to wind load, and it was confirmed that the TMD could effectively improve the control performance.
