Pivotal clinical trials testing the efficacy of new antithrombotics for the prevention of stroke and systemic embolism in patients with atrial fibrillation have been published since the release of the first edition of Korean clinical practice guidelines for primary stroke prevention. From July 2007 to August 2012, 5 clinical studies and update of guidelines in Europe and North America were identified through systematic search. In patients with atrial fibrillation who were unsuitable for warfarin, the combination of clopidogrel and aspirin reduced the risk of stroke at the cost of increased major bleedings as compared to aspirin. In patients with nonvalvular atrial fibrillation and risk factors for stroke, new oral anticoagulants, dabigatran, rivaroxaban and apixaban,were as effective as or more effective than warfarin in preventing stroke or systemic embolism. The risks of major bleeding with novel anticoagulants were similar to or lower than that of warfarin. Particularly, the risk of intracranial bleeding was significantly lower with novel anticoagulants than with warfarin. In this report, we summarized the new evidences and updated our recommendations for primary stroke prevention in patients with atrial fibrillation.
Department of Neurology, College of Medicine, Seoul National University*Department of Neurology, Eulji University School of Medicine†Pulmonary arteriovenous malformation (AVM) is a rare cause of stroke via paradoxical embolism. We report a41-year-old woman with cerebral embolic infarct with occult pulmonary AVM, who presented with visual fielddefect of sudden onset. She has never experienced dyspnea, bleeding tendency or skin lesion, except migrainousheadache. Transesophageal echocardiography and transcranial doppler (TCD) revealed right-to-left shunt. TCDwith saline contrast medium showed multiple microembolic signals not only during Valsalva maneuver but alsoduring normal breathing. Several pulmonary AVMs were detected on chest CT, which were emboilzed with coil.We suggest that isolated pulmonary AVM might be one of the rare causes of stroke especially in young adults.대한뇌졸중학회지제6권제1호2004년5월
Hypertension is the most important risk factor for stroke. Blood pressure control results in signifi cant reduction in most cardiovascular events, especially stroke. Several recent studies investigated the effects of tight blood pressure control on the development of cardiovascular events including stroke. On the other hand, besides mean blood pressure, which is the conventional target for hypertension management, the importance of blood pressure variability has been issued recently. Optimal target of blood pressure level and blood pressure variability for stroke prevention will be discussed. (Korean J Stroke 2011;13:63-65)
