        망막 이상을 보이는 주버트 증후군 1예

        목적: 주버트 증후군은 소뇌와 뇌간을 주로 침범하는 드문 질환으로, 주버트 증후군에 동반된 망막 이상 1예를 경험하였기에 보고하고 자 한다. 증례요약: 9세 여아가 양안 저시력과 안진, 주시 장애를 주소로 내원하였다. 내원 시 최대교정시력 우안 20/80, 좌안 20/80이었으며 사시는 관찰되지 않았다. 색각 검사상 양안 전색약 소견을 보였다. 전안부 검사상 이상소견을 보이지 않았으나 안저검사상 양안 주변부 망막의 미만성 색소침착 및 변성, 혈관 세약, 시신경 유두의 창백이 관찰되었다. 환아는 전반적인 발달지연, 근긴장도 저하가 있었으나 유전자 검사 및 선천 대사질환 검사는 정상이었다. 뇌자기 공명영상에서 중뇌의 대구치 징후(molar tooth sign) 및 소뇌 하충부 결손, 제4 뇌실 확장이 관찰되었다. 결론: 주버트 증후군에서 망막 색소변성 및 시기능 이상이 발생할 수 있으므로 이에 대한 적절한 평가가 필요하다. Purpose: Joubert syndrome is a rare disorder which affects the cerebellum and the brain stem. Herein, we report a case of Joubert syndrome accompanied with retinal abnormality. Case summary: A 9-year-old female visited our hospital with chief complaints of low vision in both eyes, nystagmus, and lack of gaze movement. The best-corrected visual acuity in her right eye was 20/80 and in the left 20/80 and heterotropia was not observed. She appeared to have incomplete total color blindness on the color vision test. The anterior segment test showed no abnormal findings other than diffuse pigmentation and degeneration of the peripheral retina, vascular attenuation, and pale optic disc in both eyes on fundus examination. The patient showed overall developmental delay and decreased muscle tension, but genetic and congenital metabolic disease tests were normal. The molar tooth sign of the midbrain, defect in the lower part of the cerebellum and dilatation of the fourth ventricle were observed on magnetic resonance imaging. Conclusions: Appropriate evaluation of retinitis pigmentosa and visual function should be performed in Joubert syndrome patients.

        정상안압녹내장 환자에서 단기간 프로스타글란딘 제제 사용이 각막생체역학에 미치는 영향

        목적: 정상안압녹내장 환자에서 단기간 프로스타글란딘 제제 사용이 각막생체역학인자에 미치는 영향에 대해 알아보고자 한다. 대상과 방법: 본원에서 정상안압녹내장으로 진단 받은 52안(52명) 중, tafluprost를 사용한 환자는 27안(27명), travoprost 25안(25명)이며, 모든 환자는 치료 전과 8주 후 각각 goldmann applanation tonometer (GAT), ocular response analyzer (ORA)를 이용하여, 안압 및 각막생체역학인자를 측정하였다. 결과: 치료 전과 8주 후 GAT를 이용한 안압 및 ORA를 이용한 Goldmann-correlated intraocular pressure, corneal-compensated intraocular pressure 값은 전체 대상에서 모두 통계적으로 유의하게 감소하였다(각각 p〈0.001). 각막생체역학인자들의 치료 전후의 값은 전체 대상에서, corneal hysteresis는 10.29 ± 1.78 mmHg에서 10.61 ± 2.06 mmHg, corneal resistance factor (CRF)는 10.64 ± 2.07 mmHg에서 9.86 ± 2.08 mmHg로 CRF만이 유의하게 감소하였다. Tafluprost 사용군과 travoprost 사용군 역시 CRF만이 각각 10.39 ± 2.29 mmHg에서 9.66 ± 2.19 mmHg로, 10.91 ± 1.80 mmHg에서 10.07 ± 1.98 mmHg로 통계적으로 유의하게 감소하는 결과를 보였다(각각 p〈0.001, p=0.025, p〈0.001). CRF 변화량(치료 전 CRF-치료 후 CRF)은 다중회귀분석결과 치료 전 안압이 높을수록 더 커지는 것으로 나타났다(β=0.134, p=0.017). 결론: 정상안압녹내장 환자에서 프로스타글란딘 제제 사용에 따른 CRF의 유의한 감소로 치료 후 안압이 실제의 안압보다 높게 측정될 수 있으므로 이에 따른 약물의 안압하강효과 판정에 주의가 필요하다. CRF는 프로스타글란딘 제제 사용 후 단기간의 일시적 안압변화에 대해 민감하게 반응하는 지표이다. Purpose: To evaluate the effects of short-term prostaglandin analogues treatment on the corneal biomechanics of patients with normal tension glaucoma. Methods: This study included 52 eyes of 52 patients who were diagnosed with normal tension glaucoma. All patients were divided into two groups; one group (27 eyes) received tafluprost while the other group (25 eyes) received travoprost. Intraocular pressure, Biomechanical properties were measured by using goldmann applanation tonometer, ocular response analyzer before treatment and at 8-week after treatment. Results: The mean decrease in intraocular pressure, Goldmann-correlated IOP (IOPg), corneal-compensated intraocular pressure by using Goldmann applanation tonometer, and Ocular response analyzer were statistically significant in total patients, tafluprost, and travoprost group after using prostaglandin analogues (p 〈 0.001, p 〈 0.001, p 〈 0.001, respectively). Corneal hysteresis showed no statistical differences after treatment in total, tafluprost and travoprost group but corneal resistance factor (CRF) showed statistically significant decrease after using prostaglandin analogues in total, tafluprost, and travoprost group (p 〈 0.001, p = 0.025, p 〈 0.001). Upon multivariate analysis, the higher initial IOPg and the lower initial CRF checked, the variation of CRF (CRF in baseline - CRF at 8 weeks) got higher (β = 0.134, p = 0.017). Conclusions: It is needed to carefully monitor and evaluate the effects of prostaglandin analogues on intraocular pressure associated with initial intraocular pressure and the changes of CRF after prostaglandin treatment in normal tension glaucoma patients. CRF is sensitive factor to short-term changes of intraocular pressure after prostaglandin analogues treatment, and it is required to consider the properties of CRF when we evaluate between progression of glaucoma and corneal biomechanical properties.

        소아에서 각막굴절교정렌즈 착용의 임상 결과

        Purpose: This study was designed to investigate the clinical efficacy, maintaining effect, and safety of orthokeratology lens (Ortho-K LKTM, Lucid Korea, Korea) in children over 12 months. Methods: Fifty eight eyes of 29 Children were enrolled. Slit lamp examination, uncorrected visual acuity, spherical equivalent and astigmatism, corneal thickness, anterior chamber depth and corneal endothelial cell count were measured before fitting, and 1 week, 2 weeks, 1 month, 3 months, 6 and 12 months after lens fitting. The clinical complications were also checked after lens fitting. Results: Uncorrected visual acuity was significantly increased to log MAR 0.03 from 0.68 ± 0.11 after 12 months (p = 0.001), and spherical equivalent was decreased to -1.00 ± 0.41 D from -3.06 ± 1.60 diopter (D) during this period (p = 0.013). The mean keratometry was changed from 43.50 ± 1.75 D to 41.90 ± 1.28 D (p = 0.01), and astigmatic values was significantly decreased from 0.46 ± 0.32 D to 0.17 ± 0.06 D (p = 0.013). Also there were no significant changes in corneal thickness, anterior chamber depth and corneal endothelial cell count. We did not find the any other clinical complication relate with Orthokeratology lens. Conclusions: Orthokeratology lenses seems to be effective to correct visual acuity and safe to maintain fitting OK lens in children.

        피기백 인공수정체를 이용한 백내장 수술 후의 잔존 굴절 이상의 교정 1예

        Purpose: To report a case of corrected residual refractive error after cataract surgery with sulcoflex piggyback intraouclar lens (IOL). Case summary: A 77-year-old man was diagnosed as having hypermature cataract in the right eye and his corrected visual acuity in the same eye was hand-motion before surgery. Refractive error was +9.0 diopter (D) 6 months after conventional cataract surgery in the right eye. The authors performed additional cataract surgery using a piggyback method inserting a +10.0 D IOL in the ciliary sulcus. Four weeks after surgery, his refractive error was -1.25 D, visual acuity increased to 120/200 and 60/200 (with and without correction, respectively). No complication was observed during follow-up time and the patient was satisfied with his results. Conclusions: Correction of residual refractive error after cataract surgery with sulcoflex piggyback IOL is safe and effective. J Korean Ophthalmol Soc 2014;55(12):1890-1894

      • 우리나라 온천호텔의 사업환경 변화에 따른 테마파크 도입방안에 관한 연구

        The change in social structure leads to the significnat change in the perceptions of general public about tourism. Changes in the pattern of spa tour and tourists' needs require an introduction of the facilities with a new concept and the development of theme park, which extends the concept from remedy and invigoration further to an area of leisure. The purpose of this study is to present alternatives and a plan for heightening the competitiveness in the context of the change in environments surrounding spa hotel business. As a result, first, the conditions regarding the medical benefits, the quantity and the temperature of hot springs ate 1) it should be mediclly effectivee, 2) the quantity should be affluent, 3) it maintains moderate temperature. Sencond, the conditions involving the environment of spa attraction are 1) sanitary conditions should be satisfactory, 2) it has beautiful scenery, 3) the weather should be appropriate for recuperation, 4) facilities for remedy and recuperation are present, 5) it should be easy to access. Under the assumption that the conditions mentioned above are met and there exist attractive tour products, government's active involvement and successful inducement of private investment, Korean spa tourist business with its high quality hot springs could play an important role in the international tourism market and contribute to the revitalization of Korean economy.

      • Ethanol의 低溫心臟에 對한 抗細動作用에 關한 硏究

        This experimental study was performed to clarify the antifibrillatory effects of intravenous infusion of ethanol upon hypothermic hearts. Thirty six mongrel dogs were anesthetized with thiopental sodium and were ventilated with mechanical respirator throughout the experiment. Ethanol, 2.0g/kg, diluted with 5% dextrose solution was intravenously administered in divided doses for the expermental dogs and same amount of 5% dextrose solution for control dogs. Animals were cooled with ice water and rewarmed with hot water. Heart rates and cardiac arrhythmia were checked by ECG. Central venous pressure, right ventricular and pulmonary arterial pressures were measured by means of cardiac catheterization. Animals were divided into two groups. Group 1. Twenty dogs were cooled until death. Among those, 10 experimental dogs were infused ethanol intravenously. Group 2. Sixteen dogs were cooled down to body temperature of 20℃, when cardiac inflow and out-flow were occluded for the period of 15 minutes. Then rewarmed the animals. The results were as follows. 1. Heart rates, arterial pressure, right ventricular and pulmonary arterial pressures were markedly decreased during the period of cooling. Ethanol infusion showed no significant influence upon those decreases. 2. Central venous pressure increased to the level of right heart failure of the cooled animal, and ethanol revealed no protective action on cardiac impairment. 3. In group 1, 10% of ventricular fibrillation was seen among the ethanol infused dogs while 30% of ventricular fibrilltion among the control group. In group 2, there was no significant difference of ventricular fibrillation between ethanol and control groups. 4. Ethanol blood level at 20℃ of body temperature was 636mg%. 5. It was suspected that ethanol failed to protect the hypothermic heart from ventricular fibrillation when cardiac inflow were occluded for the 15 minutes of period.
