체폐단락술에 사용되었던 PTFE 인조혈관 내면의 변화

본 연세대학교 의과대학 흉부외과학 교실에서는 1985년 부터 1992년까지 청색성 선천성 심장기형환자에서 시행한 체 폐동맥 단락술에 사용되 었던 PTFE (polytetrafluoroethylene) 인조 혈관을 재수술시 절 제하여 꽝학 현미경 및 전자 현미경으로 관찰하였다. 체폐 동맥 단락술의 종류는 Blalock-Taussig형 10 례. Waterstone-Cooley형 1례, Pott 형 1례등이었다. 사용된 PTFE 인조 혈관의 크기는 5mm가 10개, 4mm가 1개, 6mm가 1개였다 이들의 평균 사용 시간은 31개월(12개월에서 55개월)이었다. 10개월에서 20개월사이의 PTFE 인조 혈관은 틈새에 매우 많은 혈소판이 끼어 있었고 내막 비후가 관찰되었다. 40 개월이상 기능을 하는 PTFE 인조 혈관에서는 혈관 내피 세포가 광학 및 전자 현미경으로 관찰되었다. 막힌 PTFE 인조 혈관의 내면은 매우 심한 내막 비후와 혈전으로 차 있었다. 이와 같은 우리의 경험을 토대로 PTFE인조 혈관과 자신의 혈관이 연결되는 부위에 결체조직이 잘붙어 혈관내피세포가 덮을수 있게 된다면 혈소판의 응집을 막아 오랫동안 기능을 유지할 수 있을 것으로 생각된다. Histologic, and scanning electron microscopic observa ions were made of 12 biopsy specimens from polytetrdiluoroethylene (PTFE) grafts that had been Implanted as systemic-pulmonAry shunt for improving oxygenation of cyanotic congenital heart disease and harvested near the end of pulmonary artery side at the time of redo shunt or tonal correction between 1985 and 1992. The types of shunt operation are modified Blalock-Taussig shunt in 10 cases, Waterston-Cooley shunt in 1 And Potts shunt in 1 case. The 5 mm PTFE graft was used In 10, 4mm in 1 and 6mm in 1 case. The mean duration of implantalion was 0 $\pm$ 14.1 months(rl.on 12 months to 55 months). The plAtelets were aggregated between gaps'of Coretex surface and intimal thickening was noted about 10 to 20 months after implantation. Endothelial cells were found in the 40-months patent PTFE graft by light and scanning electron microscopy. In the specimen of poor flow or zero flow graft, severe intimal hyperplasia and thrombi which was made of platelets were noted . Based on this experience, we think that the patency will be maintained well if the connective tissue could be Hxed firmly over the Inner layer of the Goretex and the endothelial cell layer sllould form over the con- nective tissue and platelet aggregation should be prevented.

한힌샘 주시경선생의 생애와 학문세계

주시경은 국어의 연구와 운동을 병행했다는 점에서 대개 국엉ㄴ구에만 그친 다른 학자들과 구별된다. 그는 연구에 매진했을 뿐 아니라 끊임없이 후학들에게 자신이 고구한 바를 가르쳤으며, 나아가 겨레에게 국어에 대한 올바른 이해와 사랑의 정신을 심어주기 위해 애썼다.

청천댐 주변의 천부 P파 및 S파 속도구조

높이 23 m인 보령시 청천댐 상부와 인근에서 5 kg 해머를 이용하여 소규모 굴절법 및 표면파 탐사를 실시하였다. 인공 지진파의 수직 및 수평성분을 초동주시 토모그래피 및 레일리파 분산곡선 역산을 통하여 천부 P파속도(vp)와 S파속도(vs) 구조를 파악하였다. 중생대 퇴적암질 기반암의 평균 vp와 vs는 댐마루 30 m 깊이에서 각각 1650 m/s와 950m/s, 하류쪽 댐체 끝 지점 10 m 깊이에서 각각 1650 m/s와 940 m/s로 분석된다. 이 층들의 동포아송비는 0.24~0.25의범위로, 고화된 퇴적층의 값과 일치한다. 댐체로부터 152 m 하류지점의 깊이 45 m 시추공 부근에서의 SH파 굴절법 토모그램은 10~12 m 깊이에 평균 vs가 870 m/s인 임계굴절면이 존재함을 보여준다. 이 지역에서는 덮개층의 vp와 vs가각각 500 m/s와 200 m/s인 상부층과 깊이에 따라 속도가 거의 선형으로 증가하는 하부층으로 구성되어 있다. On and near the 23-m high earthen Cheongcheon dam in Boryeong City, Korea, short seismic refraction and surface-wave profiles were conducted using a 5-kg sledgehammer. From vertical and horizontal components of the seismic waves, near-surface P-wave velocities (vp) and S-wave velocities (vs) were derived by inverting first-arrival refraction times and dispersion curves of Rayleigh waves. Average vp and vs for the Jurassic sedimentary basement were determined to be 1650 and 950 m/s at a depth of 30 m directly beneath the dam and 1650 m/s and 940 m/s at a depth of 10 m at the toe of the dam, respectively. The dynamic Poisson`s ratio for these strata were therefore in the range of 0.24 to0.25, which is consistent with ratios for consolidated sedimentary strata. Near a 45-m borehole 152 m downstream from the dam crest, an SH tomogram indicates a refraction boundary with an average vs of 870 m/s at depths of 10~12 m.At this site, the overburden comprises the upper layer with relatively constant vp and vs around 500 and 200 m/s, respectively, and the lower layer in which both vp and vs increase with depth almost linearly. The dynamic Poisson`s ratios for the overburden were in the range of 0.30 to 0.43.

휘튼대회의 복음전도와 사회참여에 관한 선교사역적 이해

The Wheaton Conference is the challenge of ecumenical movement and the official proclamation of the stances of evangelical movement and the exhibition of plans of evangelical references in 1966. This was a response to the ecumenical absorption of IMC, which was at first evangelical up until 1958, during the 3rd WCC in 1961. 1. The Wheaton conference was a movement to challenge the ecumenism. As part of a way to challenge the ecumenical movement and make a firm stance, the motto of the evangelical movement was “It is not about heresy, enemy, sin, church,” and mentioned that evangelical movement will give up its conversations with WCC. They will proclaim the evangelical unity and consistency is affected by the pressure, the persecution and the pain caused by the ecumenism. Especially since the identity of protestantism became confused as well as at times chaotic because of the dialogue with the roman catholics and greek orthodox; something that is largely promoted by ecumenism but not welcomed by evangelicalism. The evangelical movement is strongly against the ecumenical movement only because it allows the Gospel to be deteriorated, replaced, and at extreme cases, it does not even carries the meaning of the Gospel. This tendency is a recourse of Hudson Taylor’s proclamation during the formation of IFMA in 1917. 2. The Wheaton Conference reveals the biblical stance of the evangelical movement. The difference between evangelical movement and ecumenical movement is evident. The changes happening in mission fields and the conflicts between the two movements affecting the social works shows us that the evangelical movement can no longer take a step backward when it comes to issues concerning the Gospel. The ecumenical movement limits salvation to one’s own salvific destiny which is not what the evangelical movement believes. The evangelical movement believes in the repentance led by the Holy Spirit in accordance to the authority of the Bible, the belief in the forgiveness caused by the crucifixion of Jesus Christ, and the discipleship through the devotion of one’s life to the Word. The ecumenical movement solely focus on the worldly works that is needed in the rapidly changing society. As a result, it is revealed that ecumenical movement creates wastelands, according to evangelicals, and seconds the work of the salvation. Evangelical missionaries therefore, works on missions that will heads toward the salvation of people regardless of if it is less focused on social welfare. There is a belief that works of social welfare will gradually appear. However, even if the social works are less funded, the spiritual works is always given priority. 3. It is impossible to unify the two sides. The Wheaton Conference is a proof that the evangelical and ecumenical movements are fundamentally different in foundation, hence they can never be unified. Evangelicals believe that without being born-again, there is nothing but eternal death. This is their statement. Even if there is nothing but proclamation of the Word, the spiritual aspects must come first before anything. This is where it differs from ecumenism. It only makes sense that evangelical movements feel that they are being pressured by the ecumenical movement in the mission field, complain about their ongoing excursions and pressure, and acts only to fulfill the great commission of our Lord. Nevertheless, one with passion for the Gospel will take part in social welfare works, and proclaim the Gospel without worrying about ecumenism. 4. In conclusion, although there is no need for unification between the two movements, with the approval of the biblical authority and the identity of the Gospel, there are room for cooperation between the two movements. There is no need for ecumenism to solve the problems between the Roman Catholics and Greek orthodox. If proselytism is a problem for ecumenism, problems concerning sharing the Gospel need to be addressed. 휘튼대회는 1958년까지 함께한 IMC가 1961년 3차 WCC에 귀속됨으로써 설자리를 잃어버린 선교단체와 선교회, 개 교회와 교단선교부들이 1966년 휘튼에서 에큐메니컬 선교에 도전하고, 복음주의 선교의 입장을 공식적으로 선언하며, 복음주의 일치를 꾀하고자 개최되었다. 1. 휘튼대회는 반에큐메니컬 운동과 선교를 선포하였다. 복음주의 진영은 에큐메니컬 운동에 대해 “이단, 적, 죄악, 교회가 아니다”라는 강경한 어투를 사용하면서 반대하였으며, “WCC와의 만남과 대화도 포기”한다고까지 하였다. 또한 에큐메니컬 진영으로부터 받은 압력과 고난, 고통을 제시하면서 복음주의의 통합과 일치를 강조하였다. 특히 로마가톨릭과 정교회가 에큐메니컬 운동에 가입한 사건을 개신교 정체성의 혼란과 혼동을 야기한 주된 요인으로 보고, 심지어 로마가톨릭도 선교의 대상으로 보았다. 복음주의 선교가 에큐메니컬 운동을 거부하는 이유는 그 운동이 복음을 혼란, 변경, 대체 혹은 가감함으로써 복음의 정체성을 흔드는 혼합주의적 경향을 가지고 있기 때문이었다. 이런 경향은 1917년 IFMA를 구성할 때 허드슨 테일러의 공식적인 선언에서 발견된 내용과 일치한다. 2. 휘튼대회는 복음주의 선교의 성경적 입장을 근거로 에큐메니컬선교와 통합이 불가능하고, 복음주의 선교와 에큐메니컬 선교가 근원적으로 다르다는 점을 밝혔다. 복음주의는 선교를 성서의 권위를 근거로 성령의 인도하심에 따라 회개하며, 예수 그리스도의 십자가로 용서함을 믿고, 말씀의 삶을 결단하는 제자화를 강조한다. 이때 제자화란 복음주의 선교의 사회적 삶의 실천을 말한다. 에큐메니컬 선교는 이것을 두 영역으로 나누어 하나는 개인구원, 다른 하나는 사회구원이라는 이분법적 도식으로 정의하였다. 에큐메니컬 선교는 급변하는 현실 속에서 여러 과제 중 상황적으로 긴급한 사역에 집중하였지만, 이는 결과적으로 선교 현장을 영적 황무지를 만들고 영혼구원을 포기하는 결과를 낳았다. 반면에 복음주의는 오직 복음으로 회개한 영혼을 구원하는 활동에 집중하며, 영적 사역이 이루어지면 자연스럽게 사회봉사 사역이 성취된다고 보았다. 휘튼대회는 양 진영이 더 이상 연합할 필요가 없음을 입증하였다. 복음주의 선교는 에큐메니컬 운동의 지속적인 압력과, 선교지에서의 회유와 압박을 고통스럽게 호소하며, 오직 복음전도로 주님의 대위임령을 수행하려고 한다. 복음의 열정이 있으면 사회참여와 봉사가 이어질 것이지만, 설령 사회봉사를 실천하지 않았다고 해도 복음제시가 이루어졌다면 그것은 선교를 행한 것으로 인정된다. 따라서 에큐메니컬 선교와의 문제는 걱정할 필요가 없다. 3. 양 진영을 통합하려는 노력보다 서로의 특징을 살려 독립적 선교운동이 되게 하는 것이 바람직하다. 성서적 근거를 바탕으로 한 복음주의 선교는 에큐메니컬 선교의 핵심적 동심원이다. 따라서 가운데 영역에 복음주의 선교를 놓고 그 주변 원으로 에큐메니컬 선교를 배치해야 한다. 정리하자면, 양 진영을 통합하려는 시도보다 성경의 권위와 복음의 정체성을 포기하지 않으면서도 서로를 배려함으로써 사역의 다양성을 전제로 서로 협력할 것은 협력하고 독립적으로 할 것은 독립적으로 할 필요가 있다. 복음주의 선교의 개종 문제나, 로마가톨릭과 정교회 문제는 에큐메니컬 선교에서 관여하지 않는 것이 바람직하다. 에큐메니컬 선교의 개종 문제나 복음주의 선교의 복음 제시에 관한 문제에 관해서 양 진영이 서로 논쟁하는 것은 매우 소모적이다. 오히려 복음주의 선교는 근본적으로 복음의 정체성을 다룬다는 점에서 에큐메니컬 선교를 떠받치는 토대로 보는 것이 좋다. 반면에 에큐메니컬 선교는 다양한 선교 영역을 접할 수 있는 정책과 전략을 지원하며, 연합과 통합된 사역 등의 장점을 가지고 있다. 양 진영의 사역들은 복음증거 사역을 전제로 한 영역으로 보면 된다. 그럼에도 불구하고 에큐메니컬 선교는 로마가톨릭과 정교회가 개신교의 교리와 다르다는 사실을 인지해야 한다.

FTIR을 이용한 다관능성 에폭사이드 화합물 처리 양모섬유의 정량분석

The various techniques of FTIR method, such as KBr-pellet, ATR(attenuated total reflectance) and PAS (photoacoustic spectroscopy), were investigated in order to analyze the amount of epoxide added onto wool fibers. The multifunctional epoxide of PPE(polyglycerol pclyglycidyl ether) was used with different concentrations for the preparation of samples with various add-on. As a result of ailalysis of FTIR spectrum, typical ether peak of PPE epoxide appeared at around $1111 cm^{-1}$ for the KBr-pellet and PAS method while this peak shifted to $1070 cm^{-1}$ in the case of ATR method. The absorbance ratios of amide I,II,III band and ether band were plotted against epoxide add-on individually for determining the linear relation. The best linear fitting was observed with the absorbance ratio of amide III and ether band in both KBr-pellet and ATR method. Therefore, the amount of epoxide could be determined quantitatively by using FTIR spectrum. The slopes of linear fitting were calculated for both methods and compared these values with each other. The difference in slope suggests that the reaction occurs predominantly at the surface rather than inside of the fiber. This result can also be confirmed by FTIR-PAS method, in which IR spectrum was obtained under the control of penetration depth.oss-linked polyurethanes exhibited higher stress than linear polyurethane at the same strain level. The chemical cross-linkages between diacetylene groups appeared to make more stable hard domain to the external deformation.

복잡선천성 심장기형 완전교정수술후 시행한 복막투석의 임상적 고찰

본 연세대학교 심장혈관센타에서는 1992년 I월부터 1993년 12월가지 체외순환을 시행하여 완전교정 을 시행한 복잡심장기형 환아 198명중 22예에서 복막투석을 시행하였다. 이들의 나이는 10일에서 6세 (평균 14.8$\pm$ 17.8개월)이었으며 복막투석의 적응증으로는 수분과부하상태가 10예, 혈장칼릅치가 5. Smmo111 이상인 경우가 3예, 그리고 충분한 양의 이뇨제를 투여함에도 소변양이 Imllkglhr 이하가 시간 이상 지속된 경우 9예에서 시행하였다. 체외순환이 90분 이상인 경우와 그 이하인 경우를 비교할 때 90분 이상 시행한 경우에 의미있게 많이 복막투석을 필요로 하였다(P<0.05). 1개월 미만 환아 13예중 5예(38%)에서 시행하였으나 그 이상의 나이와 비교할 때 의미있게 높지는 않았다(p : 0.08). 16예에서 신기능이 회복되었으나 4명은 호흡부전 또는 폐혈증으로 사망하였고 신기능이 회독되지 않 은 6예는 저심박출증이나 폐부종 등으로 모두 사망하였다. 아직도 사망률은 높으나 체 외순환 후 빈뇨, 수분과부하, 또는 고칼륨증이 있을 때에는 조기에 복막투석을 시행하는 것이 환자 치료에 도움이 되리라 사료된다. During the 2 years period, we hav performed acute peritoneal dialysis in twenty two children after cardiopulmonary bypass among 198 patients who underwent surgery due to complicated congenital heart diseAses . were fluid over- load (10 cases), oliguria (9 cases) and hyperkalemia (3 cases). Bypass time greAter than 90 minutes was a predictor of the need for dialysis(p< 0.05). Five neonates (38 %) operated on instituted peritoneal dialysis but it was insignificant determinant factor (p 08). Sixteen patients recovered their renal suction, but 4 patients died due to respirAtory fAilure or sepsis. All the patients which could not recover their renal function, expired although the renal failure was not re- sponsible for the mortality. We think that early peritoneal dialysis is helpful in cases of oliguia, fluid overload or hyperkalemia after cradiopulmonary bypass.

Unsual Cytotoxic Phenethylamides form Xenerhabdus nemetophilus

Three simple carboxamides incorporating the phenethylamine moiety have been isolated from strain XR-NC of a symbiotic bacterium Xenorhabdus nematophilus. Their structures were identified by spectroscopic data and synthesis. The compounds exhibited significant cytotoxicities against human cancer-cell line, viz. the gastric adenocarcinoma, colon adenocarcinoma and lung adenocarcinoma.
