        A Case of Medullomyoblastoma of Cerebellopontine Angle Mimicking Acoustic Neuroma

        Medulloblastoma is a common malignant central nervous system neoplasm found mainly in children. One the contrary, medulloblastoma of the cerebellopontine angle, the location of the tumor is very unusual. This is the the first case of the medullomyoblastoma, a rare form of medulloblastoma, occurring in the cerebellopontine angle. A 15-year-old boy experienced a sudden hearing loss in the left ear. Conservative medical treatment failed, and temporal MR imaging revealed a heterogeneously enhancing mass at the left cerebellopontine angle cistern and in the internal auditory canal; therefore, the lesion was regarded as a typical acoustic neuroma. Few days before surgery, an ipsilateral facial palsy developed, and a follow-up MR imaging showed a rapid growth of the previous lesion. The extended translabyrinthine approach permitted surgical removal. And under pathological diagnosis of malignancy, radiation therapy and series of chemotherapy was performed.

        외이도에 발생한 다발성 골종 1예

        Osteoma is a benign neoplasm that can occur and extend beyond the nearest region in the external auditory canal (EAC); however, a multiple growth of the tumor in the ipsilateral ear has never been reported. A 41-year-old male patient having a past history of uncertain chronic otitis media complained of recent otalgia and progressive hearing loss in the same ear. Physical examination showed that two hard masses arising from both anterior and posterior aspect of the right EAC were contacting each other and obstructing most of the lumen. High resolution computed tomography scan revealed not only two growing exophytic bone lesions causing EAC stenosis but also inflammation of the middle ear and sclerotic mastoid cavity. We performed excision and middle ear surgery at the same time. Both specimens were pathologically confirmed as osteomas, so we report this unusual case with a review of literature.         Herpes zoster oticus, traditionally known as Ramsay Hunt syndrome, is nowadays described also as acute peripheral facial neuropathy by varicella zoster virus infection. Besides facial palsy, auricular vesicular eruptions accompanied by neuralgic pain, adjacent cranial nerve structures are involved and various audiovestibular symptoms are frequently combined.         측두골 골절의 합리적 분류법에 대한 고찰

        Background and Objectives:definition of transverse or longitudinal one, but it is often obscure and has problems of discrepancy in corelation with otologic symptoms. So we investigated the practicality of other noticeable reporting schemes for reasonable adoption. Subjects and Method:We thoroughly reviewed the medical records of the temporal bone fracture patients in past two years. And high resolution CT images were also re-evaluated by the senior radiologist using traditional and newer (capsule violating, petrous vs. nonpetrous) classification schemes. Results:A total of 61 cases were identified as temporal bone fracture. Among various symptoms, only sensorineural hearing los was significantly higher in the transverse type of traditional scheme and in otic capsule violating fracture. Symptoms such as subjective and verified hearing los including conductive hear-ing los and ear fulness were higher in nonpetrous fracture (p<0.05). Conclusion:Traditional and otic capsule scheme correl-ated poorly with clinical presentation, whereas the petrous classification was better matched with main symptoms. Therefore, the petrous scheme was sugested as the more discriminative clasification plan of temporal bone fracture to predict its sequela. (Korean J Otolaryngol 2007 ;50 :491-5)

        전정와우신경 형성부전의 방사선학적 소견:CT와 MRI

        Background and Objectives:Sensorineural hearing loss (SNHL) could be caused by dysfunction of the cochlea, abnormality of vestibulocochlear nerve (VCN), or disorder of central auditory pathway. Among these causes, VCN deficiency is one of the ciency by reviewing temporal bone computed tomography (TBCT) and magnetic resonance image (MRI) findings of patients with profound unilateral SNHL, diagnosed as VCN deficiency and also, we compared the width of the bony canal of the cochlear nerve (BCCN) between the normal and afected side on TBCT and tried to clarify the diagnositc value of BCCN. Materials and Method:We reviewed TBCT and temporal submillimetric axial and parasagittal 3D turbo spin echo (3D-TSE) T2-weigh-ted MRI of 9 patients who were diagnosed as unilateral VCN deficiency. We evaluated MRI findings in terms of the presence and comparative size of the component nerves (facial, cochlear, superior vestibular, inferior vestibular nerve) then we classified the type of VCN deficiency, acording to the Casselmans proposal. We compared the diference of width of IAC and BCN betwen normal and afected side on TBCT. Results:We could find the detail anatomic feature of VCN in IAC by parasagittal of the classification, 8 patients were type 2A and the other one was type 1. In the TBCT study, the size of IAC and BCCN of the affected 8 ears were smaller than that of side and one patient had normal IAC but narrow BCCN of the affected ear. Conclusion:In this study, we conclude that TBCT and MRI could be useful methods for diagnosis of VCN deficiency. Also, the hypoplastic bony canal of the cochlear nerve on TBCT is finding to overlook easily and may be another important indicator for evaluating VCN deficiency.         도산 안창호의 민족운동과 대공주의

        Dosan is a great pioneer of national movement who devoted his whole life to the cause of national independence. He carried out diverse national movements in search of the plans for national independence to overcome the reality of the fall of Korea. His national movement developed step by step. At first, he sought a methodology to raise national "power". Then he integrated independence movements with the rise of the March 1st Movement. Finally, his national movement developed from power training through integration to daegongjueui(大公主義). Dosan's national movement is based on "power" training for national independence and set with the national spirit of sacrifice for "the unification" and "daegongjueui" as well as an integrated organization for national independence, irrespective of ideologies. Daegongjueui emphasizes collective power concentration with individuals ready to sacrifice themselves for the struggle of national independence and collective integration for the construction of a sovereign national state. That is to say, daegongjueui shows the construction of a philanthropic and egalitarian society by building an individual national spirit and sovereign state on the basis of innovated ego. 도산은 민족독립을 위해 일생을 바친 민족운동의 위대한 선각자이다. 그의 생애는 국망의 현실을 극복하기 위한 민족독립 방략을 찾아 다양한 민족운동을 전개하였다. 그의 민족운동은 단계적으로 전이되고 있는 것을 알 수 있다. 최초 그는 민족의 ‘힘’을 양성하기 위한 방법론을 찾았으며, 다음으로 3·1운동을 계기로 독립운동의 통합으로 발전하고 있다. 최종적으로 민족운동은 실력양성과 통합운동에서 대공주의로 전이되고 있다. 도산의 민족운동은 독립을 위한 ‘힘’을 바탕으로 한 실력배양과 통합된 독립운동조직-이념과 주의를 넘어선-을 위한 ‘통일’과 ‘대공주의’를 위한 민족의 희생정신을 배경으로 한다. 대공주의는 민족의 독립투쟁을 위해 희생할 수 있는 개체를 바탕으로 집단적 힘의 결집을 강조하고, 민족독립국가를 건설하기 위한 집단적 통합을 강조하고 있다. 즉 대공주의는 혁신된 자아를 바탕으로 한 개체적 민족정신과 독립국가의 건설을 통해 인류애와 세계평등의 사회건설을 보여 주고 있는 것이다.

        2015 개정 과학과 교육과정의 ‘기능’ 기반 탐구 수업에 참여한 고등학생의 과학과 핵심역량에 대한 인식

        In this study, we investigated the change in science core competency perception of high school students and the reason for change when science inquiry classes were conducted using eight ‘skills’ of the 2015 revised science curriculum. Fifteen first-year high school students in Jeollanam-do participated in the science inquiry class of this study, and the class was conducted for 20 hours (5 hours a day for four days). The inquiry activities used in the class consisted of four activity stages (research problems, research methods, research results, and conclusions) and each stage was constructed to include at least one ‘skill (Problem Recognition, Model Development and Use, Inquiry Design and Performance, Data Collection, Analysis and Interpretation, Mathematical Thinking and Computer Application, Conclusion and Evaluation, Evidence-based Discussion and Demonstration, and Communication)’. As a result of the study, students’ perception of the five science core competencies increased statistically significantly at the significance level of 0.01 through inquiry classes and more than 93% of students recognized that their science core competencies improved through the classes. However, since the class of this study was conducted for a small number of students, it is difficult to generalize the effect of the class, and so it is necessary to conduct a quantitative study for many students. 본 연구는 2015 개정 과학과 교육과정의 8가지 ‘기능’을 활용한과학 탐구 수업을 실시했을 때 고등학생들의 과학과 핵심역량에 대한인식 변화와 변화 이유에 대하여 조사하였다. 본 연구의 과학 탐구수업에 전라남도 소재 고등학교 1학년 15명이 참여하였으며, 수업은20시간(하루 5시간 4일) 진행하였다. 수업에서 이용한 탐구 활동은연구 문제, 연구 방법, 연구 결과, 결론의 4개 활동 단계로 구성되어있으며 각 활동 단계에 8가지 ‘기능 (문제 인식, 모형의 개발과 사용, 탐구 설계와 수행, 자료의 수집⋅분석 및 해석, 수학적 사고와 컴퓨터활용, 결론 도출 및 평가, 증거에 기초한 토론과 논증, 의사소통)’이최소 1회 이상 포함되도록 구성하였다. 연구 결과, 탐구 수업을 통해학생들의 5가지 과학과 핵심역량에 대한 인식이 유의수준 0.01 기준으로 통계적으로 유의미하게 증가하였으며, 93% 이상의 학생들이수업을 통해 과학과 핵심역량이 향상하였다고 인식했다. 하지만 본연구의 수업은 소수 학생을 대상으로 실시하였기 때문에 수업의 효과를 일반화하기는 어려우므로, 많은 학생을 대상으로 한 정량적 연구수행이 필요하다.
