        소아에서 발생한 대장의 염증성 질환에서 E-cadherin의 발현

        목 적: 소아에서 다양한 원인에 의해 발생한 대장염에서 점막의 형태학적 변화와 세포 접합에 다양한 변화가 있으리라 예상되어 세포 간의 결합을 유지하는 E-cadherin의 변화를 살펴보았다. 방 법: 1998년 1월부터 2003년 8월까지 부산대학교병원 소아청소년과에서 하부 위장관 내시경술과 대장점막 조직 검사를 통해 대장염으로 진단된 39명을 대상으로 하였다. 파라핀 블록에서 면역조직화학염색법을 이용하여 E-cadherin의 세포 내 발현을 조사하였다. 주변의 정상 조직과 비교하여 E-cadherin의 발현이 동일한 강도와 양상을 가진 세포가 50% 이상인 경우를 정상으로 판정하였고, 발현이 정상인 세포가 50% 미만이거나 염색 분포의 이상이 있거나 전혀 염색되지 않은 경우를 이상으로 판정하였다. 결 과: 1) 본 연구에서 비특이성 대장염 15예(38.5%), 크론병 7예(17.9%), 감염성 대장염 5예(12.8%), 음식 단백 과민성 직결장염 5예(12.8%), 궤양성 대장염 3예(7.7%), Henoch-Schonlein purpura 대장염 2예(5.1%), 그외 베체트병, 허혈성 대장염 1예가 포함되었다. 2) 모든 종류의 대장염에서 상피세포 E-cadherin 발현 감소가 관찰되었으며, 77%의 대상 표본에서 E-cadherin 발현감소가 있었다. 3) 활동성 염증이 심한 부위에서 Ecadherin 발현 감소가 현저하였으며 병변부에서 떨어진 상피세포에서는 정상 발현을, 궤양 주위나 재생 상피가 있는 부위는 심한 발현 감소를 보였다. 결 론: 모든 종류의 염증성 대장 질환에서 E-cadherin 발현 감소가 있었다. 이러한 변화는 염증과 궤양이 있는 부위에서 상피세포 접합을 느슨하게 함으로써 상피세포의 재생을 위한 세포의 이동을 용이하게 하기 위한 작용이라고 판단된다. Purpose: Colitis is a condition associated with a spectrum of altered morphologic changes and cellular adhesion. E-cadherin plays a key role in the establishment and maintenance of epithelial tissue structure and cell-cell adhesion. The purpose of this study is to evaluate E-cadherin expression in colonic epithelium of various colitis in children. Methods: The expressions of E-cadherin were examined in 39 cases of colonic mucosal biopsy specimen using immunohistochemical staining. When more than 50 percent of cells exhibited uniformly the same intensity and pattern of immunostaining as the adjacent normal mucosa, the antigen expression was considered normal. Abnormal expression was defined when less than 50 percent of cells stained, when cells showed a heterogeneously weak or altered distribution, or when complete absence of staining was observed. Results: Fifteen cases with non-specific colitis (38.5%), 7 cases of with Crohn's disease (17.9%), 5 cases of infectious colitis and milk protein sensitive proctocolitis (12.8%), 3 cases of ulcerative colitis (7.7%), 2 cases of Henoch-Schonlein purpura colitis (5.1%), one case of Behcet's disease and ischemic colitis (2.6%) were included in this study. E-cadherin expression was decreased in all kinds of colitis. Reduced expression of E-cadherin was observed in 77 percent of cases. E-cadherin was weaker or no expression in reparative epithelium and "ulcer associated cell lineage". Conclusion: Altered expression of E-cadherin occurs during mucosal inflammation in any kinds of colitis. These changes may be involved in promoting cell migration during epithelial restitution of the gastrointestinal mucosa.

        Hyalinizing trabecular adenoma of the thyroid gland is a rare benign neoplasm predominantly diagnosed in middle-aged women. Carney et al. first described this entity that may mimic paraganglioma, medullary carcinoma and papillary carcinoma in 1987. We describe cytologic and histopathologic features of a case of hyalinizing trabecular adenoma combined with occult papillary carcinoma in the opposite lobe. A 55-year-old woman presented with nontender palpable mass of the right neck for 6 months. The aspirate was cellular and contained small clusters and sheets of epithelial cells with abundant filamentous, vacuolated, and ill-defined cytoplasm. The nuclei were slightly pleomorphic and showed nuclear overlapping, nuclear grooves, and intranuclear cytoplasmic inclusions. Histologic examination showed hyalinizing trabecular adenoma in the right lobe and occult papillary carcinoma in the left lobe.

        Pseudoangiomatous stromal hyperplasia (PASH) was initially described by Vuitch et al. as a benign breast lesion, consisting of mammary stromal proliferations which simulate vascular lesions, and which might be mistaken for a low-grade angiosarcoma. This condition occasionally presents as a palpable mass in postmenopausal women, but is more frequently encountered as an incidental component in premenopausal women. Clinical, radiological, and fine-needle aspiration (FNA) findings associated with this condition can mimic those observed in conjunction with a phyllodes tumor or a fibroadenoma. The cytological features of PASH are generally nonspecific, and its diagnosis by FNA cytology is fairly difficult. In this study, we report a case on PASH, manifesting as a palpable mass.

        N/A Background/Aims: In recent years, knowledge about the role of lymphocytes-macrophages and characterstics of poliferation and activation of fat-storing cells(FSCs) in DMN induced liver fibrosis have greatly increased. However immunologic characteristics of mononuclear cells in this rnodel are not well defined. Furthermore, bFGF has been identified in liver extracts by Western blot, but its cellular lacalixation remains unknown. We have studied lymphocyte subsets and cellular localixation of bFOF in DMN-indueed liver fibmsis vsing an immunohistochemical method. Methods: Male Sprague-Dawley rats recieved -1% DMN dissolved in saline at a dose of 10 ul/kg, i.p., 3 conscutive days a week for 3 weeks. The animals were sacrificed on days 7, 14, 21, 28 and 35. We camed out immunostain by indirect three-step immunoperoxidase method for lymphocyte subsets on frozen sections and,ABC rnethod for a-smooth muscle actin as an activation marker of FSCs and bFGF on paraffin sections. Results: The early stage of DMN treated rats show multiple foci of centrilobular hemcerhagic necrosis infiltrating lymphoeytes and macrophages. CDS(+) T-cells seemed to be the major camponents of the inflammatory infiltrate, and CD4(+) T-cells were also seen around and periportal areas. bFGF was expressed in centrilobular clusters of macrapbages, later in activated FSCs and marginal hepatocytes of regenerating micronoduks, Condusions: CDS(+) T-cell was the major effector cell in DMN-induced liver injury. bFGF was poduced by macrophage and then expressed by PSCs and hepatocyte. Therefore bFGF prornotes produetion of extracellular matrix by FSCs and regeneration of hepatocytes. (Korean J Gastroenterol 1997 29: 497-505)

        Gastric epithelial neoplasia is a very common disease entity in Korea, encompassing gastric adenoma and adenocarcinoma. There are still discrepancies in pathologic diagnosis of gastric epithelial neoplasia between Western and Japanese pathologists after Vienna consensus classification. With increasing use of endoscopic therapy such as endoscopic mucosal resection and endoscopic submucosal dissection, it is very important to agree on the consensus criteria in the diagnosis of gastric epithelial neoplasia among pathologists in Korea. On this background, the current concepts, and contemporary issues of definition, diagnostic and classification criteria of gastric epithelial neoplasia were reviewed.         대장 절제술 후 발생한 회장 정맥류 출혈을 캡슐 내시경으로 진단한 1예

        We report a case of bleeding ileal varices associated with intra-abdominal adhesions after colectomy which was successfully diagnosed using capsule endoscopy. A 77-year-old woman visited the emergency department for several episodes of melena. She had a medical history of neoadjuvant chemo-radiation therapy and subsequent surgery for rectal cancer 6 years previously. Conventional diagnostic examinations including upper endoscopy, colonoscopy, and abdominal computed tomography could not detect any bleeding focus, however, following capsule endoscopy revealed venous dilatations with some fresh blood in the distal ileum, indicating bleeding ileal varices. The patient underwent exploratory laparotomy and the affected ileum was successfully resected. No further gastrointestinal bleeding occurred during the 6 months follow-up. Small intestinal varices are important differential for obscure gastrointestinal bleeding especially in patients with a history of abdominal surgery in the absence of liver cirrhosis, and capsule endoscopy can be a good option for diagnosing small intestinal varices.         십이지장에서 발생한 염증성 근섬유모세포종 1예

        An inflammatory myofibroblastic tumor (IMT) is a rare disease that can occur in a variety of locations, including the lung, orbit, parotid, pleura, and stomach. Despite multiple reports in various organs, a duodenal IMT is rare with limited case reports. We encountered a case of a 49-year-old male with a duodenal IMT. The patient underwent a laparoscopic wedge resection under the impression of a duodenal mesenchymal tumor, such as gastrointestinal stromal tumor, but the final diagnosis was a duodenal IMT. The patient was treated successfully with an oral nonsteroidal anti-inflammatory drug for the residual lesions. He was free of recurrence during the 12 month follow-up period.         활성화된 내피세포에서 GRO-${\alpha}$, IL-8 및 ENA-78의 발현양상과 호중구 부착에 미치는 영향

        연구배경 : 혈관내피세포와 호중구의 상호작용은 급성염증반응의 생리 및 병리학적 핵심적인 과정으로서 특별한 분자들에 의해서 매개되고 있다. 적절한 자극이 주어질 때 혈관 내피세포에서는 부착 및 신호인자가 표현되며 이들은 호중구의 수용체에 의해 인지된다. 본 연구에서는 IL-1과 LPS로 혈관내피세포를 자극하였을 때 CXC chemokine family에 속하는 GRO-${\alpha}$, IL-8 및 ENA-78이 내피세포에서 분비되는 양상을 관찰하였고, ENA-78과 GRO-${\alpha}$가 호중구-내피세포의 부착에 미치는 영향을 평가하였다. 방 법 : 인제대혈관 내피세포를 배양하여 다양한 농도로 IL-1과 LPS로 자극하였다. 자극된 내피세로로부터 분비 되어진 GRO-${\alpha}$, IL-8 및 ENA-78의 농도는 ELISA로 측정하였다. recombinant human ENA-78 및 GRO-${\alpha}$로 자극된 내피세포에 동위원소가 부착된 정상백혈구를 이용하여 부착능을 측정하였다. 결 과 : IL-1a과 LPS로 자극된 내피세포에서 GRO-${\alpha}$ IL-8 및 ENA-78의 분비는 자극의 농도와 지속시간에 비례하여 증가되었고, recombinant human ENA-78과 GRO-${\alpha}$의 농도가 증가함에 따라 호중구의 부착율이 더욱 증가하였다. 결 론 : ENA-78과 GRO-${\alpha}$는 CXC chemokine family에 속하며 주로 상피세포에서 발현되어 백혈구의 화학적 주성을 유발하며 염증성 반응을 매개하는 중요한 물질로 알려져 있다. 본 연구에서는 다양한 농도의 IL-1과 LPS로 자극된 내피세포에서 농도에 비례하여 GRO-${\alpha}$, IL-8 및 ENA-78이 분비됨을 확인할 수 있었으며, 또한 ENA-78과 GRO-${\alpha}$를 각각 다른 농도로 배양한 후 호중구 부착능이 농도에 따라 증가함을 알 수 있었다. 따라서 본 연구의 결과는 염증성 자극으로 혈관내피세포에서 분비되는 ENA-78과 GRO-${\alpha}$ 등의 CXC chemokine 이 염증반응의 진행 기전에서 중요한 역할을 담당할 것임을 시사하며, 향후 염증성 질환의 발병기전 규명이나 치료법개발을 위한 기초적 자료를 제공하는데 도움이 될 것으로 생각된다. Background: Inflammation, where vascular endothelial cells are activated by cytokines, recruits circulating leukocytes such as neutrophils into the tissues. Mononuclear phagocytes as well as tissue cells activated by these stimuli produce these chemokines. In this study, thr effects of IL-1 and LPS on the expression of CXC chemokines such as GRO-${\alpha}$, IL-8 and ENA-78 in vascular endothelial cells and the neutrophil adhesion effects of ENA-78 and GRO-${\alpha}$ was investigated. Methods: Human umbilical vein endothelial cells were cultured and stimulated with various concentrations of IL-1 and LPS. The concentrations of the GRO-${\alpha}$, IL-8 and ENA-78 secreted were measured using enzymelinked immunosorbent assay. The effects of ENA-78 and GRO-${\alpha}$ on neutrophil adhesion to the endothelial cells were also investigated. Results: The addition of IL-1 and LPS to the vascular endothelial cells induced GRO-${\alpha}$ IL-8 and ENA-78 secretion in a time- and dose-dependent manner. The neutrophil adhesion was also increased by induction of ENA-78 and GRO-${\alpha}$ to the vascular endothelial cells in a dose-dependent manner. Conclusion: CXC chemokines such as GRO-${\alpha}$, IL-8 and ENA-78 secreted by the vascular endothelial cells play an important role in the acute inflammatory responses by stimulating neutrophil adhesion to the vascular endothelial cells, raising the possibility that the CXC chemokines are one of the targets in the clinical application of acute inflammation.
