Purpose: The purpose of this study was to identify factors affecting job satisfaction of health care service practitioner. Methods: Participants were 241 health care service practitioner in elderly care facility of Busan, Ulsan. Data were collected from January 1st to 31th, 2013 using the scales of measurements of job satisfaction, job characteristic, job stress, and organizational commitment . Results: There was significant difference according to general characteristic in age, marital status, religion, satisfaction to current division, having experience of residence with elderly, actual experience of residence with elderly, and consideration of job change. There was light positive correlation between job characteristic and job satisfaction(r=.35, <i>p</i>=.001). light negative correlation between job stress and job satisfaction(r=-.28, <i>p</i><.001), and strong positive correlation between organizational commitment and job satisfaction(r=.65, <i>p</i><.001). Of factors affecting job satisfaction of subject, organizational commitment was 41.1% of explanation power(<i>ß</i>=.43) which was the most and it combined with improper reward/treatment. Job characteristic, and satisfaction to current division totally has 51.7% of explanation power. Conclusion: Organizational commitment was the most affecting factor to job satisfaction. It is assumed that intervention program to improve organizational commitment and to increase job satisfaction of health care service practician in elderly care dacility.

세침흡인 세포학적 검사로 진단한 폐의 Nocardiosis - 1예 보고 -

Nocardia, aerobic members of the order of Actinomycetaceae, produces infections in human lung. Nocardial infection is associated with underlying diseases of immuno-suppression or treatment with corticosteroid. It is difficult to detect Nocardia by sputum examination or histologic sections and it has rarely been diagnosed by fine needle aspiration of the lung. We describe a case of pulmonary nocardiosis in a 72 year-old man, diagnosed by fine needle aspiration, which was confirmed by culture of aspirates. The aspirates showed neutrophil-predominant inflammatory cells with microorganisms demonstrated by Gomori methenamine silver and Gram stain. The organisms had characteristic long blanching filamentous structures. The lesions on chest X-ray were in resolution with antimicrobial therapy.

가속 열 노화시험을 이용한 침매터널용 고무 씰 소재의 사용수명 예측

This paper considers accelerated thermal degradation tests which are performed for rubber seal materials used for undersea tunnels constructed by immersion method. Three types of rubber seals are tested; rubber expansion seal, omega seal, and shock absorber hose. Main ingredient of rubber expansion seal is EPDM(Ethylene Propylene Diene Monomer) and that of both omega seal and shock absorber hose is SBR(Styrene Butadiene Rubber). The accelerated stress is temperature and an Arrhenius model is introduced to describe the relationship between the lifetime and the stress. From the accelerated degradation tests, dominant failure mode of the rubber seals is found to be the loss of elongation. The lifetime distribution and the service life of the rubber seals at use condition are estimated from the test results. The acceleration factor for three types of rubber seals are also investigated.

Railway rail fastening system is the critical device which gives big influences to not only the vehicle driving stability and the orbit's structural stability against the impulsive load but also the noise vibration and the ride comfort. As a part of the low-carbon green growth the importance of the railroad industry is getting highlights on its excellent energy-efficiency and eco-friendliness. However so far the Korea's domestic rail fastening system technology is not so good and the technical reliance to abroad is very heavy. In this study we conducted comparative analysis on the rail fastening system with new and used one of the same type. And those systems are imported by Seoul Metro and are being used by it. With this basis we developed the components and the materials and then established the durability assessment methods appropriate to the Korean domestic circumstances. And through the reliability qualification test on the 7 parts of the rail fastening system we've improved the reliability and guaranteed the 15 years of service lifetime. (B10life15) Establishment and standardization of Reliability Standard on the parts of the rail fastening system such as anti-vibration pads guide-plate screw spike made it possible to perform the internationally fair assessment. And it is thought that we can satisfy the manufactures' and consumers' needs of cost-cutting and qualification security by shortening of assessment period on rail fastening system.

Since the first description of Kikuchi disease(Subacute necrotizing lymphadenitis) in Japan, 1972, this condition has been reported in a worldwide. The etiology remains unknown. This disease shows a marked predilection for young woman, and a few cases in children has been reported. The usual initial clinical manifestation is localized cervical lymphadenopathy with fever, weight loss and myalgia. We experienced a 6 year-old girl of subacute necrotizing lymphadenitis with ANA, therefore report a brief review with the related literatures.

장대레일 교체주기는 자갈궤도 레일두부 표면요철과 레일 휨피로의 상관관계 분석을 통해 산정되었다. 본 연구에서는 실물 레일 휨 피로시험을 통한 레일의 S-N선도, 국내 도시철도(서울메트로) 콘크리트궤도에서 발생하는 레일 휨응력 예측식, Haibach's rule을 통해 콘크리트궤도 장대레일의 휨 피로수명을 평가하였다. 여기서, 레일용접방법별 S-N선도와 레일표면요철, 열차운행속도가 고려되었다. 또한, S-N선도 파괴확률 1%, 0.1%, 0.01%를 고려한 휨 피로수명을 비교하였다. 따라서 본 연구에서는 콘크리트궤도 장대레일의 휨 피로수명을 제안하였다. It is suggested that the service life of the continuous welded rail(CWR) is estimated by the relationship between the rail surface irregularity according to the accumulated passing tonnage and bending fatigue of welded part in CWR. In this study, based on the results of bending fatigue tests of rail and results of measuring tests in situ of rail bending stress, this study estimated the bending fatigue life of welded rail in concrete track, adopting a Haibach's rule. The bending fatigue life of CWR considered the rail surface irregularity, train speed and the S-N curve by types of rail welding. In addition, this study estimated it for the fracture probability 1%, 0.1%, 0.01%. Therefore, this study proposed bending fatigue life of CWR in concrete track.

A total of 44 patients with extremity soft tissue sarcomas had received conservative surgery and radiation therapy in the Department of Radiation Oncology, Yonsei Cancer Center, Yonsei University College of Medicine between Jan. 1980 and Dec. 1990. Initial surgical treatment consisted of intracapsular excision in 33 patients (75%), marginal excision in 9 patients, and wide excision in two patients. Total radiation doses were between 40Gy and 65Gy (median 60 Gy). Median follow-up period was 47 months. Overall actuarial 5-year survival rate was 76.9% and 5-year recurrence free survival rate was 39.3%. There was no statistically significant prognostic factors but the 5-year survival rates tended to be decreased in the patients with high grade tumors and treated with narrow surgical extent. Sixteen patients recurred during fellow-up (16/31, 51.6%). The incidence of initial local recurrence was 22.6% (7/31) and distant metastasis was 29%(9/31). Of the 7 who did not have metastasis at diagnosis of local recurrence, 5 were submitted to a salvage treatment. All of them achieved local control and distant metastasis appeared in three of them. In conclusion, conservative surgery and radiation therapy in the patients with extremity soft tissue sarcomas was the effective treatment modality.
