검사지연, 용혈 및 혼탁이 PT 및 aPTT 검사결과에 미치는 영향

배경 : 검체채취 후 때때로 시간이 지연된 검체가 검사실에 접수될 때도 있으며, 용혈이 되었거나 혼탁한 검체로 검사를 의뢰할 때도 있다. 이러한 경우에 검사를 하지 않고 검체를 다시 채취하여야 하는지 아니면 무시하고 검사를 해도 되는 지를 결정하기위하여 검체채취 후 검사지연시간, 검체의 용혈 및 혼탁이 결과치에 미치는 영향을 관찰하였다.방법 : 검사시간 지연으로 인한 영향을 알아내기 위하여 검체채취 즉시, 2시간 후, 4시간 후, 8시간 후에 검사를 실시하였다.PT 198예, aPTT 192예를 대상으로 하였다. 용혈된 검체가 검사결과에 미치는 영향을 알아내기 위하여 31예를, 약하게, 중등도로, 심하게 용혈된 상태로 만든 다음 검사를 하였다. 혼탁한 검체가 검사결과에 미치는 영향을 알아내기 위하여 30예를, 약하게,중등도로, 심하게 혼탁한 검체를 만들었다.결과 : 검사시간이 지연되면 8시간 후의 PT는 결과에 영향이없었고 aPTT는 결과에 영향을 미쳤다. 용혈된 검체는 PT와aPTT의 결과에 영향을 미쳤다. 혼탁한 검체는 약하게 혼탁한 경우와 중등도로 혼탁한 경우에는 영향이 없었으나, 심하게 혼탁한경우에는 결과치를 얻을 수가 없었다.

Variable clinical manifestations and the unpredictable biological behavior of renal cell carcinomas are well known. About a third of patients have metastasis by the time they are first seen by a physician, most commonly to the lung, frequently to the bone and/or liver and less commonly to a virtually unlimited range of tissues and organs. Vaginal cancer represents approximately 1-3% of all genital tract malignancies, and most cases present as metastasis from the cervix, endometrium or colon. Vaginal metastasis from a renal cell carcinoma can be confused with a primary vaginal clear cell carcinoma, and metastasis of a renal cell carcinoma to the vagina is extremely rare. Here, we report a case of a renal cell carcinoma, detected by a metastatic vaginal mass.

We described the findings of fine needle aspiration cytology of the lung from a patient with Wegener's granulomatosis. Early diagnosis and prompt treatment of the patients with Wegener's granulomatosis is essential for a better prognosis. However, the variety of clinical presentations and nonspecific radiologic infiltrates of Wegener's granulomatosis frequently make the diagnosis difficult. Although an open lung biopsy is required for a firm diagnosis, fine needle aspiration cytology & biopsy preparation can also provide an adequate tissue sample, when the findings of fine needle aspiration are considered with clinical manifestations and ANCA value in the serum. The cytologic smears showed scattered necrotic tissue fragments entrapping many neutrophils and occasional epithelioid cells. Multinucleated giant cells were infrequently observed. Ziehl-Neelsen stain for acid fast bacilli was negative. All the cytologic features recapitulated the histopathologic findings of purulent and necrotizing granulomatous inflammation seen in Wegener's granulomatosis.
