        치과위생사의 치주병환자 구강보건교육 실태 및전략 수립에 관한 연구

        Objectives : The objective of this study was to analyze cognition and status of oral health education of dental hygienists who manage the patients visiting dental clinics and to utilize the findings as continuous study data required for development of systematic oral health education program for management of peridontal disease of adults. Methods : Survey was carried out for 261 dental hygienists working in dental clinics who are performing oral health education of patients with peridontal disease living. Results : 1. In the socio-dynamic diagnosis, it was analyzed that, the younger the age is and the lower the career is, the more the work is centered around assisting medical treatment job and, the older the age is and the higher the career is, the work is centered around oral health education job. 2. In the behavioral diagnosis, it was analyzed that, as to source of knowledge acquisition, guide of dentists and senior dental hygienists was the highest and, as to education media, visual education and demonstration education were mainly used. 3. In educational diagnosis, as to the degree of knowledge about oral health education and the degree of practice, it showed that the degree of knowledge was lower than the degree of practice in periodontal disease process and oral prophylaxis as well as scaling part. 4. In both the cases of independent practice and group practice, it showed that the most frequent obstacle was insufficient educational materials and tools and it was analyzed that acquisition of new knowledge and technology is required as a matter to be improved. Conclusions : It seems to be required for the dental hygienists to develop detailed knowledge and professional technology in order to induce actions and motive of patients during oral health education of peridontal patients and to develop and produce data for efficient education of peridontal patients.

        치과위생사의 근무환경 요인과 스트레스 및 근골격계 장애의 관련성

        치과위생사의 주요 건강문제인 근골격계 장애를 조사하고, 근무환경 요인과 스트레스 및 근골격계 장애의 관련성을 파악하기 위해 광주지역 치과위생사 483명을 편의 추출하여 자기기입식 설문조사를 시행하고 빈도분석, 교차분석, t-검정, 다중 로지스틱 회귀분석을 실시하여 분석한 결과는 다음과 같다. 연구대상자의 심한 통증을 호소하는 정도인 근골격계 장애는 29.8%였다. 근무환경요인과 스트레스 및 근골격계 장애의 관련성을 파악한 결과, 근무환경 요인에서 휴식시간 빈도였으며, 그 외 스트레스 요인 등이었다. 근무 환경 요인에서 휴식시간 빈도가 없음으로 응답한 군에 비해 2회라고 응답한 군의 OR은 0.29 (95% CI, 0.11∼0.73)로 근골격계 장애가 감소하여 유의한 차이를 보였으나, 휴식시간 빈도가 1회와 3회 이상에서는 유의한 차이가 없었다. 스트레스의 근골격계 장애 OR은 0.44 (95% CI, 0.27∼0.70)로 음의 상관관계를 보였다. 이것은 휴식시간 빈도가 적당 할수록(2회일 때) 근골격계 장애가 낮음을 의미하며, 근골 격계 장애가 있는 경우 스트레스가 낮게 나타났다. The purpose of this study is to examine musculoskeletal disorders and grasp the relationship between working environment factors and stress and musculoskeletal disorders in dental hygienists. The self-administered questionnaire survey was conducted with 483 dental hygienists working in Gwangju who were selected by convenience sampling. Frequency analysis, crosstab analysis, t-test, and multiple logistic regression analysis were made. Subjects’ musculoskeletal disorders that complain of severe pains were 29.8%. The relationship between working environment factors and stress and musculoskeletal disorders were examined. As a result, work break frequency of working environment factors and stress were found to influence musculoskeletal disorders. For working environment factors, the odds ratio of musculoskeletal disorders of the two breaks group was 0.29 (95% confidence interval [CI], 0.11∼0.73), compared with the no break group, indicating less musculoskeletal disorders. However, no significant difference was found in one break and more than three breaks groups. The odds ratio of musculoskeletal disorders of stress was 0.44 (95% CI, 0.27∼0.70), indicating negative correlation. As break frequency was moderate (two breaks), musculoskeletal disorders became decreased. Stress showed negative correlation with musculoskeletal disorders.

        PRECEDE 모형을 적용한 치과위생사의 근골격계 통증 심각도 관련 요인

        Objectives: The purpose of the study is to investigate the musculo-skeletal pain prevalence and severity in the dental hygienists based on PRECEDE model. Methods: A self-reported questionnaire was completed by 483 dental hygienists in Gwangju from September 13 to October 12, 2013. Data were analyzed by frequency analysis, chi-square test, t-test, and multiple logistic regression analysis using SPSS 18.0 program. Musculo-skeletal pain severity was classified from 1 to 5 by PRECEDE model. The questionnaire consisted of six questions of the general characteristics of the subjects, one question of musculo-skeletal pain prevalence, one question of body part musculo-skeletal pain prevalence, one question of subjective health status, three questions of activities of daily living, six questions of working environment, one question of musculoskeletal system diseases knowledge, two questions of social support, two questions of education experience and data use method, and five questions of necessity of health education. Results: The prevalence rate of musculo-skeletal pain within a year was 83.9% and 22.8% of the dental hygienists complained of severe pain. The odds ratio of moderate pain severity was 1.99(95% CI, 1.10-3.60) and the odds ratio of unhealthiness was 3.27 (95% CI, 1.35-7.94). The odds ratio of pain severity in those working for 4-6 years was 0.21(95% CI, 0.08-0.57). The odds ratio of pain severity in those practicing 6-10 scaling cases per day was 0.33(95% CI, 0.17-0.65). The odds ratio of pain severity in wrist turning and bending was 3.56(95% CI, 1.19-10.62). Conclusions: The muscolu-skeletal pain severity in the dental hygienists was closely associated with subjective health condition, work duration, the number of scaling practice activity, and a treatment posture. Regular physical checkup for the dental hygienists will improve the musculo-skeletal pain due to scaling practice.

        치과 의료기관 종사자의 감정노동과 직무스트레스가 직무만족에 미치는 영향

        Objectives : This study was to investigate the level of emotional labor and stress on job satisfaction and the significant factors influencing job satisfaction in oral health professional. Methods : Data were obtained from a cross-sectional survey of 297 oral health professional(mean age=$29.95{\pm}8.10$). Participants in the study were recruited when they attended the dental health form in Gwangju. Emotional labor was classified into two factors: external, inner. Stress consisted of four factors: sleep disorder&anxiety, self-confidence, health&vital, social role work. job satisfaction included growth development, stability chase. The data was analysed using t-test, one-way ANOVA, Pearson correlation and multiple regression. Results : The mean score of emotional labor and score was 3.13 out of a maximum 7 points. job satisfaction was negatively correlated with emotional labor and stress. job satisfaction explained 32.0%. Conclusions : Based on the findings, The emotional labor and stress was correlated with job satisfaction. These results suggest that further development should be continued to develop the effective emotional labor and stress to improve the job satisfaction of oral healthcare professional.

      • 앤더슨모형을 이용한 예방진료 항목의 구강보건교육 개발

        진료실에서 예방진료 항목에 대한 치과종사자의 구강보건교육은 중요하므로 구강보건 진료기관의 예방진료항목에 대한 구강보건교육 실태와 요구도를 분석하여 구강보건교육 모형개발의 기초자료로 활용하고자, 2010년 광주광역시 치과의원근무 치과종사자 324명을 대상으로 설문조사를 통하여 분석한 결과는 다음과 같다. 결과; 1. 소인성 요소에서는 지식(B=.206)과 태도가 높을수록(B=.438) 실천이 높아지는 것으로 분석되었다.[표 2] 2. 필요성 요소에서는 구강병 예방 진료항목 중 치면세마는 진료기관 운영형태에 따라 단독개원(15.3%), 공동개원(14.9%) 모두 높게 분석되었다[표 3]. 3. 가능성 요소에서는 구강보건교육 자료활동에서 악치모형이 19.4%로 가장 많았고, 그 다음으로는 사진과 구강카메라가 16.1%로 분석되었다[표 4]. 4. 구강보건교육의 장애요인에서는 22.6%로 업무과다가 가장 큰 장애요인으로 나타났고, 다음으로 환자의 요구가 없어서가 19.4%로 나타났다[표 6]. 결론; 이상의 결과에서 예방진료항목의 구강보건교육시 동기유발을 위하여 환자의 요구에 맞는 구강보건교육 프로그램 개발이 필요하며, 업무량 조절과 예방진료 항목의 의료보험 급여화가 절실하며, 구강보건교육 자료개발이 필요할 것으로 사료된다.

        치과의료기관 종사자의 조직문화와 셀프리더십 유형이 조직유효성에 미치는 영향

        Objectives: This study was to investigate the level of organizational culture and self-leadership on organizational effectiveness and the significant factors influencing organizational effectiveness in oral health professional. Methods: Data were obtained from a cross-sectional survey of 297 oral health professional(mean age=29.95±8.10). Participants in the study were recruited when they attended the dental health form in Gwangju. Organizational culture was classified into four factors: relation-oriented, innovation-oriented, hierarchy-oriented, task-oriented culture. Self-readership consisted of five factors: rehearsal/self-observation, self-goal establishment, selfesteem, self-criticism, self-reward. Organizational effectiveness included organizational commitment and job satisfaction. The data was analysed using t-test, one-way ANOVA, Pearson correlation and multiple regression. Results: The dominant organizational culture was rank-oriented culture. The mean score of self-readership score was 3.51 out of a maximum 5 points. Organizational commitment was positively correlated with age, relation-oriented culture, self-goal establishment, self-reward explained 41.9% and job satisfaction was positively correlated with relation-oriented culture and self-esteem explained 48.6%. Conclusions: Based on the findings, the organizational culture and self-readership was correlated with organizational effectiveness. These results suggest that further development should be continued to develop the effective self-readership and organizational culture to improve the organizational effectiveness of oral healthcare professional.
