Accurnulation of keratinized cells within the infundibulum of pilosebaceous canal is important in the pathogenesis of acne. Light and electron microscopic study of experimentally induced comedones, from the rabbit's external ear canal, was performed to define the earIy morphologic changes within and around the epithelial lining of the comedones. In light microscopic observation, early comedone was composed of loose cohesive horny cells and late comedone was composed of a mixture of loose and tight cohcsive horny cells. Cohesion between horny cells occured, in eIectron microscopic study, in two different ways: initially, by the persistence of desmosomes; later, and to a lesser extent, by tight junctions, which tightly bound the horny cells together. Multiple lipid droplets within the horny ceIls and a gradual decrease in the numbrane coating granules were observed.

Background:Porokeratosis is characterized by cornoid lamellae histologically and evolve cutaneous malignant tumors in about 7% of the patients. Actinic keratosis is the most common premalignant lesion and progressed into squamous cell carcinoma in 20% of the patients. It has been reported that the expression of p53 increased within or beneath the cornoid lamellae of porokeratosis and actinic keratosis. Objective:The purpose of this study was to understand the role of apoptosis in pathogenesis and progression into malignant tumors and to investigate whether a positive correlation occurs between apoptosis index and expression of p53, bcl-2, PCNA and iNOS in porokeratosis and actinic keratosis. Methods:TUNEL staining and immunohistochemical staining with p53, bcl-2, PCNA and iNOS antibody were done in paraffin-embedded tissue sections of 10 cases of porokeratosis and 12 cases of actinic keratosis. Results:The results are summarized as follow: 1. The mean of apoptosis index(%) was 27.3±7.82 in porokeratosis, 36.3±10.32 in cornoid lamellae of porokeratosis. 2. The mean of apoptosis index(%) of actinic keratosis was 41.5±8.98 and statistically significant higher than that of porokeratosis(p=0.002). (Korean J Dermatol 2000;38(2):207~212)

수장족저부 농포성 건선 및 한포진에서 국소 Bath-PUVA 요법과 스테로이드 요법의 효과에 대한 비교

Background:Local bath-PUVA(LB-PUVA) therapy has been recently tried for the treatment of palmoplantar pustular psoriasis and chronic eczema with successful results. However it has not been reported that the effectiveness of LB-PUVA was compared with that of steroid treatment which is the most available treatment. Objectives:The purpose of this study was to assess the effectiveness, symptom-free interval, recurrence rate and side effects of local bath-PUVA(LB-PUVA) therapy in palmoplantar pustular psoriasis and dyshidrotic eczema which had been resistant to conventional treatments and to compare these results with those of the steroid treatment. Methods:Twenty four patients with palmoplantar pustular psoriasis, dyshidrotic eczema and palmoplantar keratoderma were treated with local bath-PUVA and the control group of twenty patients with systemic and topical steroid. 1. Local bath-PUVA therapy After a 15-minute bath in 1mg/L of methoxsalen solution(0.0001%), the palms or soles or both were exposed immediately to UVA radiation by UV/PUVA200. The initial standard dose was 1.5 J/cm2, and the patients were treated three times a week. 2. Systemic and topical steroid therapy The patients treated with 10-15mg of oral prednisolone once a day and high-potency steroid(Dermovate ointment) to the lesion topically for 4-8 weeks. The efficacy, symptom-free interval and side effects were assessed for 6-12 months after termination of the treatments. Results:The following results were obtained from this study. 1. In the objective study, severity scores before and after treatment significantly differed in palmoplantar pustular psoriasis treated with local bath-PUVA (p<0.001). 2. In the subjective study, steroid treatment was more effective than LB-PUVA treatment for dyshidrotic eczema and palmoplantar pustular psoriasis. 3. Remission period was significantly longer (mean 9.1±2.8 months vs. 5.2±3.1 months) and recurrence rate, lower (26.7% vs. 68.9%) after LB-PUVA treatment than after steroid treatment in palmoplantar pustular psorisis and dyshidrotic eczema. 4. One patient with palmoplantar keratoderma was effectively treated by LB-PUVA. Conclusion:Local bath-PUVA therapy can be used as an effective and safe treatment method of palmoplantar pustular psoriasis and possibly palmoplantar keratoderma. A 73-year-old male was presented with a walnut-sized erosive plaque on the lower lip and several skin colored nodules on the neck, the left shoulder and the arm. The other case was a 74-year-old male who had been diagnosed as metastatic lung cancer of unknown origin was consulted for several ulcerative nodules on the face and the left lateral neck. Both patients were finally diagnosed as metastatic adenocarcinoma each from the lung and stomach by histopathologic finding and immunohistochemical studies. (Korean J Dermatol 1999: 37(7): 941-943)

Oak moss is a lichen extract obtained from Evernia prunastri and Pseudovernia furfuracea. It is the potent,ial allergen in contact dermatitis to perfumes. Sensitivity to oak moss has been atributed mainly to atranorin, evernic acid, usnic acid and fumarprotocetraric acid. We report a 44-year-old female patient presenting with features of contact dermatitis due to oak moss in perfumes. Patch test results revealed positive reactions to fragrance mix, oak moss, atranorin and evernic acid. (Kor J Dermatol 1992; 30(3): 377-380)

We present two cases of angiosarcoma occuring on the scalp of aged person. The case 1 was a 74 year-old male who have had purplish colored, slightly raised irregular surfaced plsque on frontal scalp. The other case was a 69-year-old man who have had purplish nodule with satellite lesions on the right frontal area. The cases were confirmed by clinical picture, histologic findings. Treatment was not done because of refuse by patient.

We report 2 cases who have been diagnosed as lipodystrophia centrifugalis abdominalis juvenilis. The case 1 is l2 year-old girl. She had well-defined depression of skin on both inguinal area, lower portion of chest and both flanks which was noticed at the age of 7. The case 2 is 16 year-old boy with depressed lesions on Rt. inguinal area and abdomen which were noticed at the age of 6. Biopsy specimen showed absence of subcutaneous fat tissue in both cases.

Porokeratosis is a familial, chronic, progressive disorder which demonstrates a characteristic clinical and histologic picture. Since Mibelli described three cases in 1893, several clinical variants have been identified, all of which are delineated by a diagnostic histopathologic pattern. We reported a case of atypical linear porokeratosis that had three types of skin lesions. One was the plaque type, as originally described by Mibelli. And others were atypical lesions, as papules and verrucoid hyperkeratotic plaques.
