        Effects of Self-Care Intervention on Self-Care and Health Outcomes in Patients with Heart Failure: A Systematic Review and Meta-Analysis of Randomized Controlled Trials

        This review aimed to determine the effectiveness of self-care intervention in patients with heart failure. Methods: An electronic database search was conducted with PubMed, OVID-MEDLINE, EMBASE, CENTRAL, Google Scholar, and hand searches from February 1 to 4, 2021. Two reviewers independently selected the studies, assessed their methodological quality, and extracted relevant data. We conducted a meta-analysis of the effect of self-care versus usual care using R 4.1.0 and summarized the results of intervention from the included studies. Results: Of the 6,368 articles identified, 55 randomized control trials met the inclusion criteria for review. Self-care interventions significantly improved self-care (SMD=0.87 [95% CI: 0.70, 1.03]). In the subdomain of self-care, it was effective in improving self-care behavior (SMD=0.80 [95% CI: 0.59, 1.02]), self-care maintenance (SMD=0.99 [95% CI: 0.59, 1.38]), self-care management (SMD=1.03 [95% CI: 0.60, 1.46]), and self-care confidence (SMD=0.63 [95% CI: 0.33, 0.93]). There was a significant improvement in quality of life (SMD=0.53 [95% CI: 0.27, 0.79]) and mortality (OR=0.70 [95% CI: 0.50, 0.95]). However, the pooled effects of readmission rate (OR=0.80 [95% CI: 0.59, 1.09]) were not significant. Conclusion: This review provided evidence that self-care intervention in patients with heart failure effectively improves self-care, reduces mortality, and improves the quality of life.

        간호계와 비간호계 대학생의 장기 및 인체조직 기증에 대한 지식, 태도 및 기증 의향 비교

        Abstract Purpose: This study investigated knowledge, attitude, and Intention for organ and human tissue donation of college students. Methods: The study subjects were 499 students who majored in the nursing and non-nursing in three universities at one city. The data were collected from self-administerd questionnaire scales of knowledge, attitude, and intention toward organ and human tissue donation. The data was analyzed with descriptive analysis, x2 test and t-test. Results: The percentage of knowledge about organ and tissue donation was 51% and 41%, attitude 76% and 73%, and intention 60% and 58%, respectively. There was a significant difference between the nursing and nursing college students in the knowledge of organ donation (t=3.41, p<.010) and human tissue donation knowledge(t=3.41, p<.010). On the other hand, attitudes toward organ donation (t=1.35, p=.180) and intention(t=-1.16, p=.250), (t=-1.72, p=.090), there was no difference between nursing and non-nursing college students. Conclusion: In order to change the knowledge, attitudes and donation intentions of organs and tissue donation, various experiences and programs related to organ and tissue donation should be provided to university students, especially education for nursing students, which should be integrated into the regular curriculum efforts are required.

      • 인지장애노인의 요실금에 대한 고찰

        This research was performed to review on urinary incontinence of the cognitively impaired older adults. There was use of diaper or habits practice for a management of urinary incontinence that is one of major problem in nursing at the long term care facilities, which could lead to a physical and emotional problem of the patients and caregivers. Self-management strategies like pelvic muscle exercise and biofeedback were proved the effectiveness on urinary incontinence who were the cognitively intact and ambulatory adult. However it is difficult to apply those for the cognitively or physically impaired elderly as the strategies need cognitive ability for learning and performing, Because of this limitation, caregiver-dependent strategies like scheduled toileting, habit training and prompted voiding are usually recommended for management of urinary incontinence of the cognitively impaired older adults. Prompted voiding involves asking residents at regular intervals if they need to void and if so, they are helped in toileting, which is to assist them to void using appropriate toileting receptacles before the resident urinate involuntarily on the clothing, bed or diaper. So it has advantage of involving the elderly in decision making of their care, and has merit of keeping dignity of individual as they maintain almost normal voiding. Conclusively, this implies that the individual could maintain a certain level of continence if appropriate toileting opportunity given, although the elderly has impaired cognition and mobility. But, Important factors of successful implementation of prompted voiding were as followed. First, there must be a subject selection process for appropriate for prompted voiding. Second, identify subject's voiding pattern and schedule individually. Lastly, give a positive feedback, when the resident request toileting assistance, or maintain continence status, or use toileting receptacles properly. I hope to develope a practical prompted voiding protocol based on this results and to perform it for cognitively and physically dependent older adults.

        정신간호사의 회복에 대한 인식, 자아존중감, 역할수행이 직무만족에 미치는 영향

        Purpose: The purpose of this study was to identify perception of recovery, self-esteem, role performance, job satisfaction and factors influencing job satisfaction of psychiatric mental heath nurses. Methods: Data were collected from 161 psychiatric nurses from G city & J province who understood the purpose of the study and agreed to participate. Data were analyzed using descriptive statistics, t-test, Pearson correlation coefficient, and multiple regression with SPSS 18.0. Results: There were significant differences in job satisfaction between upper 25% and lower 25% group for perception of recovery, self-esteem, and role performance. Results from the multiple regression analysis revealed difference in the related factors and in the magnitude of the relationship evaluated by standardized beta coefficients. Significant factors, listed by the magnitude of beta coefficients, were role performance, self esteem, roles and responsibilities in recovery, expectations regarding recovery in perception of recovery. These variables accounted for 16% of job satisfaction. Conclusion: Results indicate that to improve psychiatric nurses' job satisfaction, there is a need to develop programs including personal qualities and perception of recovery. Further research is necessary in order to establish consensus on roles of the related factors of job satisfaction.

        라이프케어 증진을 위한 뇌졸중 환자의 자가간호 이행 영향요인

        본 연구는 라이프케어 증진을 위하여 뇌졸중 환자를 대상으로 이차 발병을 예방하기 위하여 자가간호이행의 정도를 확인하고 영향 요인을 알아보고자 시도되었다. 대상자는 뇌졸중으로 요양병원에 입원 중인 환자 188명을 대상으로 2017년 8월부터 10월까지자기보고식 구조화된 설문지를 이용하여 설문조사를실시하였다. 수집된 자료의 분석은 SPSS 21.0 프로그램을 이용하여 t-test, ANOVA, 상관관계 및 다중회귀분석을 실시하였다. 연구결과에 의하면 대상자의 평균연령은 58.9세이며 유병기간은 2.34년이었다. 대상자들의 자가간호 이행은 일반적인 특성 중 나이, 교육수준, 동거인, 직업, 월평균수입과 유병기간과 유의하였으며, 건강신념, 사회적 지지와는 양의 상관관계를 우울과는 음의 상관관계를 나타내었다. 다중회귀분석의 결과 뇌졸중 대상자의 라이프케어 증진을 위한 자가간호 이행에는 자기효능감이 가장 큰 영향을미쳤으며 다음으로 유병기간과 사회적 지지가 영향을 미쳤다 (F=87.46, p<.01, Adjusted R2=.58). 이상의 연구결과를 바탕으로 뇌졸중 환자의 간호실무에서 라이프케어 증진을 위한 자가간호 이행을높일 수 있는 간호 중재를 계획할 때 유병기간과 관련하여 사회적 지지와 자기효능감을 강화한 총제적인 접근이 필요할 것으로 생각되며 자기간호 이행을높이기 위해 입원 생활과 관련된 중재 프로그램의개발 및 적용이 필요할 것으로 생각된다.

        근거기반실무 과정을 활용한 성인간호학 임상실습 모듈의 시범운영

        본 연구의 목적은 근거기반실무를 활용한 임상실습 모듈이 간호대학생의 근거기반 실무 지식, 태도, 역량, 적용의도 및 비판적 사고성향의 향상에 효과적인지 파악하기 위함이었다. 본 연구는 단일군 전후설계의 실험연구이다. 연구 참여에 동의한 연구대상자는 성인간호학실습에 임할 M대학교 간호학과 3학년 학생 30명이었다. 연구기간은 2016년 5월 2일부터 6월 10일까지였다. 성인간호학실습 모듈은 근거기반실무를 활용하여 간호과정을 수행하는 것을 목표로 2주 동안 진행되었고, 1회기 당 30분에서 3시간씩 총 6회기로 구성되었다. 모듈의 효과는 대응표본 t-test로 분석하였다. 연구결과, 근거기반실무를 활용한 성인간호학 임상실습모듈이 간호대학생들의 근거기반실무에 대한 지식(t=-10.67, p<.001), 태도(t=-2.34, p=.023), 역량(t=-9.03, p<.001), 적용의도(t=-2.40, p=.019)를 유의하게 향상시켰으나 비판적 사고성향의 유의한 차이는 없었다. 근거기반실무를 활용한 임상실습이 학생들의 지식, 태도, 역량은 물론 향후 실무에서의 적용의도를 향상시켰으므로 간호교육과정에 근거기반실무를 다각도로 활용할 필요가 있다. The purpose of this study was to figure out whether Adult Nursing clinical practicum module using evidence-based practice is effective in improving the evidence-based practice knowledge, attitudes, competency, intention to use and critical thinking disposition for Nursing students. This study is a one-group pretest-posttest experimental design. The study participants were third grade nursing students from M University who attended an adult nursing clinical practicum and 30 students agreed to participate in the study. The study period was conducted from May 2, 2016 to June 10, 2016. The Adult Nursing practicum module was conducted for two weeks with the aim of performing the nursing process using the evidence-based practice and consisted of 6 sessions, 30 minutes to 3 hours per session. The effects of the modules were analyzed by paired t-test. Research results showed that Adult Nursing clinical practicum module using evidence-based practice significantly improved student’s knowledge(t=-10.67, p<.001), attitudes(t=-2.34, p=.023), competency(t=-9.03, p<.001), and Intention to use(t=-2.40, p=.019) about evidence- based practice. But, there has not been a significant change in the critical thinking disposition. Since clinical practicum education using evidence-based practice has improved students knowledge, attitude, and competency as well as intention of future use, it is necessary to utilize evidence-based practice in nursing curriculum in various ways

        국내 근거기반간호연구의 동향

        Purpose: The purpose of this study was to analyze the current status of research related to evidence-based nursing and to suggest directions for the future of evidence-based nursing practice, education and research. Methods: A search was done of 20 research publications, including domestic nursing journals, nursing master"s theses and doctoral dissertations before November 2016. Finally, 183 studies were selected. The selected papers were analyzed using descriptive statistics and x<SUP>2</SUP> test with the SPSS/WIN 18.0 program. Results: Most of papers examined in this study were journal articles (80.9%). Meta-analysis (35.0%) was the most common study design. Methodological characteristics were as follows: before 2010 about 5% were documents that suggested Priori’ design, generation of PICO, search strategy, quality assessment and description of quality assessment outcome, but after 2011, these designs increased to 30.8%, 73.1%, 41.0%, 91.0% and 65.4%, respectively. The most frequent topics for evidence- based nursing implementation were evidence-based nursing readiness (16 papers). Highest frequency topics in systematic reviews and meta-analysis were studies that confirmed the intervention effect of exercise programs. The highest frequency topics in guideline were temperature control. Conclusion: Researchers" perceptions to improve research methodological quality and education to strengthen the research capability are necessary.
