We report a case of perivalvular abscess in a 66-year-old man with infectiveendocarditis, diagnosed by late gadolinium-enhanced (LGE) cardiovascular magneticresonance (CMR) imaging. No clinical features suspicious of infective endocarditiswere noted, however, transthoracic echocardiography revealed non-specific echogenicfocal wall thickening at mitral-aortic intervalvular fibrosa. Perivalvular abscess in theaortic valve was demonstrated as focal wall thickening between the anterior mitralleaflet and the non-coronary cusp of the aortic valve with peripheral enhancementand central low signal intensity on LGE CMR imaging. Other features suggestiveof infective endocarditis, such as neither vegetation nor valvular perforation werepresent. The perivalvular abscess did not grow after intensive intravenous antibioticstherapy, and the patient was discharged without surgical treatment. CMR withLGE provided an early accurate diagnosis of perivalvular abscess. The diagnosis ofperivalvular abscess using LGE CMR imaging was not previously reported in Korea.

To characterize the elemental composition of fine particles in urban area, $PM_{2.5}$ was collected by a 3-stage DRUM impactor at Gwangju during spring and summer. Time and size resolved concentrations for 19 trace elements were obtained by synchrotron X-Ray fluorescence analysis. Trace elements in summer were distributed in smaller size range compared to those in spring. Almost trace element concentrations in fine particles were highly increased during the Asian dust. In spring, soil elements such as Si, K, Ca, Ti and Fe had low enrichment factors indicating the dominant influence of soil dust. However, all elements had high enrichment factors in summer implying that these elements could be emitted from the anthropogenic sources. Factor analysis was conducted with the elemental composition data in order to identify anthropogenic sources of aerosols in urban area during spring and summer. Fine particles in spring have several sources such as soil dust originating from China continental region, coal and oil combustion, biomass burning, sea salt, ferrous and nonferrous metal sources. On the other hand, fine particles in summer were influenced by road dust, gasoline vehicle as well as coal and oil combustion, sea salt, ferrous and nonferrous metal sources.
A Case of Rosai-Dorfman Disease with Highly Elevated Serum Ferritin

Sinus histiocytosis with massive lymphadenopathy, also known as Rosai-Dorfman disease is a rare disorder characterized by proliferation of distinctive histiocytes within lymph node sinuses and lymphatics, sometimes involving extranodal sites. However, clinical suspicion is difficult and there is also a lack of useful diagnostic markers for this disorder prior to histological confirmation. High elevation of serum ferritin is known to be a useful diagnostic marker for various hematologic diseases, including hemophagocytic lymphohistiocytosis and lymphoma. Here, we report a case of fever of unknown origin that presented along with highly elevated serum ferritin (5,780 ng/mL), and was finally diagnosed as Rosai-Dorfman disease by lymph node biopsy.
