Bizarre parosteal osteochondromatous proliferation (BPOP) otherwise known as Nora's lesion, is a benign surface tumor of the small tubular bone of the hands and feet with high probability of local recurrence. The report of BPOP in the foot is very rare in our country. We report a case of BPOP of proximal phalanx of right $3^{rd}$ toe in 44-year-old female, successfully managed by en-bloc marginal excision with a review of the literatures.
외상성 전방 불안정성 환자에서 Bankart 복원술 후 조기 능동 운동의 결과

Purpose: To evaluate prospectively the results of early active exercise after open Bankart repair of traumatic anterior shoulder instability. Materials and Methods: From January, 2001 to June, 2003, 26 patients who were followed up at least 1 year after open Bankart repair for traumatic anterior shoulder instability were evaluated. Average age was 23.9 years old (range, 19~43) with 24 males and 2 females. We evaluated them using the functional shoulder scores (modified Rowe score, ASES score), range of motion, VAS pain scale, patient`s subjective satisfaction and return to unlimited daily living activity. Results: The shoulder functional scores increased significantly. At last follow up, the final range of motion were flexion in average 5˚ deficit in comparison to normal side, external rotation in average 10˚ deficit, and internal rotation in T9. The patient`s subjective satisfaction was good in 2l patients (81%). Return to unlimited daily activity was possible in 23 patients (88.5%), and 19 patients (73%) rejoined to sports activity before injury. There were complications including anterior recurrent subluxation in 1 case, weakness of subscapularis muscle in 1 case. Conclusion: In traumatic anterior shoulder instability, early active range of motion exercise after open Bankart repair does not decrease shoulder stability. Early exercise can be useful for returning to previous level of sports activity in young active patients.

The plica is a remnant of the synovial folds during fetal development. The plica is classified suprapatellar, medial patellar, infrapatellar, and lateral patellar plica according to the anatomic site. The one most likely cause of clinical problem is medial patellar plica. There are many reports of problems caused by medial patellar plica syndrome. But there has been no documented case report of Outerbridge classification Grade III-IV, above $2{\times}1.5\;cm$ sized huge cartilage defect of both medial femoral condyle, due to medial patellar plica. So we report this unusual case with a review of relevant literatures.

For the treatment of deep extensive abscess, the incision and drainage is necessary. But in order to make a thorough incisional drainage we can't avoid the skin incision enlarged. Even if the incision is enlarged, it is often impossible to obtain a satisfactory view of operative field. Also, the additional damage to surrounding normal tissue during operation and the scars made by large incision may be problematic. To solve these problems, we performed the endoscopic treatment for extensive deep abscesses occurred in hematoma after rupture of gastrocnemius muscle and obtained the satisfactory results. It has not been previously described, so we report it with a review of the literature.

During sports activity, rectus femoris muscle can be commonly injured as the quadriceps contracts forcefully to extend the knee and flex the hip, and rectus femoris muscle has a high percentage of type II muscle fibers which enable it to produce rapid forceful activity and the muscle is the only biarticular muscle of the quadriceps. Avulsion fractures by the traction of rectus femoris from the anterior inferior iliac spine in children are often reported. Rarely, complete rupture of proximal musculotendinous junction of rectus femoris muscle in adult may occurs by chronic repetitive stimulation and may mimic soft tissue neoplasm in the case of no history of antecedent trauma. However, there has been no report of acute complete rupture of origins of the rectus femoris muscle during relay race till now. So, we report a case of acute complete rupture of origins of the rectus femoris muscle occurred during relay race in middle-aged man, diagnosed by magnetic resonance image and treated by surgical repair with review of current literature.

Wiring techniques in the fixation of patellar fracture are commonly used. Rigid fixation is essential for early rehabilitation. Breakages of wires after the fixation of patellar fracture are common with extracapsular migration if not removed for long term. But, we experienced a case of patellar fracture with early breakage of wiring and migration of the broken wire fragment into the popliteal fossa through the popliteal hiatus in 64- year-old man and we successfully treated this case, using both arthroscopic and open procedures. So we report this unusual case with a review of current literatures.

Purpose: We evaluated the clinical and radiological results of arthroscopic ankle fusion using 2 medial screws which had advantages of less morbidity, early weight-bearing and high union rate. Material and Methods: From April, 2002 to March, 2004, 8 patients who had ankle osteoarthritis were treated by ankle fusion using 2 medial screws under arthroscopy; five patients with post-traumatic osteoarthritis, two with post-infectious arthritis and one with paralytic foot. There were 5 male and 3 female. Average age was 67 years old ranging from 57 to 71 years. We evaluated them clinically preoperative and postoperative using AOFAS score, VAS pain scale and patient's satisfaction. In regard to radiological fusion, we checked them by simple AP, lateral and mortise view. Follow up period was average 11 months (range, $6{\sim}24$ months). Results: All ankles were successfully fused with 2 medial screws under arthroscopy. The mean time of fusion was 10.5 weeks (range, $8{\sim}14$ weeks). Patient's satisfaction checked at 6 months after operation had favorable results (excellent and good 75%). One case had pain on medial malleolar area because of screw's protrusion. Conclusion: Arthroscopic ankle fusion using 2 medial screws was good modality of ankle fusion with less morbidity and early weight-bearing in some cases of ankle arthritis.

Parosteal lipoma is a rare kind of lipoma that occurring adipous tissue around the periosteum. It has been reported most commonly in the femur, the radius, the tibia and the fibular. Treatment consists of resecting the lipomatous tumor with further exeresis of the bone and periosteal excrescence in cases with hyperostosis. The authors report a rare case of parosteal lipoma occurring at the medial portion of the femur shaft with a review of the relevant literatures.
