        Esmolol 과 Diltiazem 이 기관내 튜브 발관시 심혈관계 변화에 미치는 영향

        Background: Tracheal extubation causes hypertension and tachycardia. In susceptable patients, even this short period of hypertension and tachycardia can result in myocardial ischemia. The purpose of this study was to evaluate the effect of esmolol and diltiazem in attenuating cardiovascular responses to tracheal extubation. Methods: Changes in heart rate, systolic and diastolic blood pressure were measured during extubation and emergence from anesthesia in 60 ASA physical status I patients to assess the effect of esmolol and diltiazem. The patients were randomly assigned to one of three groups (n=20 for each group): saline 5 ml (as a control), 0.2 mg/kg diltiazem and 1.5 mg/kg esmolol. These medications were given 2 min before tracheal extubation. Results: Both groups of diltiazem and esmolol were greater attenuating effect on changes of heart rate, systolic and diastolic blood pressure than control group. The inhibitory effect on changes of heart rate was greater with esmolol than diltiazem, but the attenuating effect on changes of systolic blood pressure was greater with diltiazem than esmolol. Conclusions: We concluded that a bolus dose of intravenous diltiazem 0.2 mg/kg or esmolol 1.5 mg/kg given at 2 min before extubation was of value in attenuating the cardiovascular changes occuring in association with tracheal extubation and emergence from anesthesia. Esmolol is more effective than diltiazem in attenuating the heart rate changes. Diltiazem is more effective than esmolol in attenuating the systolic blood pressures changes.

        중력모형을 이용한 한국 과실류의 교역형태 분석

        This study uses a single-country gravity model approach to analyze the main factors affecting the trade flows of five fruit products in Korea. The empirical model is applied with the trade data for the period 1991-2007. The study constructs and estimates a random-effect panel tobit model. The results reveal that fruit trade in Korea is influenced by the trade partner`s per capita GDP, distance from Korea, and the number of Korean immigrants. Unlike the existing works, distance is negatively correlated with trade. The number of Korean immigrants is the single most important factor affecting fruit trade of Korea. It is found that fruit trade of Korea is highly concentrated in several countries including the U.S. and Asian countries, especially Taiwan.

        안면 신경 마비로 인한 토안에서 성상신경절 차단의 효과

        Stellate ganglion block(SGB) is frequently performed in pain clinics. Facial palsy produces asymmetry of the face, lagophthalmos and incomplete eyelid closure. Exposure keratitis and eyeball pain can be induced by prolonged lagophthalmos. SGB was performed on a 51-year-old female patient who had exposure of keratitis and eyeball pain due to facial nerve palsy after an operation for chronic otitis media. After 31 stellate ganglion blocks, eyeball pain and keratitis nearly subsided. SGB is an effective method to treat lagophthalmos caused by facial nerve palsy.

        모노레일은 그동안 과좌식과 현수식으로 분류 되어 왔으나 세계 최초로 도입한 중앙 안내 방식의 인천 월미은하레일은 신개념의 Y Beam을 가이드 레일로 사용한 새로운 형식의 모노레일이라 할 수 있다. Y Beam 은 안내륜을 통해 모노레일의 수평 하중을 전달하는 역할을 담당하고 있기 때문에 수직 하중을 타이어를 통해 전달 받는 콘크리트 바닥 및 강재 Beam과 함께 모노레일의 주행 안전에 없어서는 안 될 가장 중요한 구조물이다. 본 연구에서는 수평 하중을 감당하고 있을 뿐만 아니라 모노레일의 안정과 안내를 함께 보장 하면서 급전선을 지지하는 역할도 동시에 담당하고 있는 Y Beam과 Y Beam을 고정하는 클램프의 특성과 안전성에 대해 고찰하였다. Monorail has been classified as straddled type and suspended type. However, Inchon Wolmi Eunha rail, a center guide type first ever introduced in the world, uses a new concept Y-Beam as guide rail. Since Y-Beam takes horizontal loads of monorail through guide wheels, it is an essential structure for the safe operation along with concrete surface and steel beam which take vertical loads through main tires. This study presents the characteristic and reliability of Y-Beam and Y-Beam fixture, clamp, which are not only taking horizontal loads but also guarantying both guiding and stability of monorail and holding power supply line.
