        피부신경펩티드에 의한 비만세포 활성화에 대한 연구

        Neuropeptides are widely distributed throughout skin, gastrointestinal tracts, and nervous and immune systems. Neuropeptides act to mediate the vasodilation and induce mast cell activation in humans and rats in vitro. However, the mechanism of the cutaneous neuropeptides-induced mast cell activation and the extent of the vascular permeability by cutaneous neuropeptides are not fully understood. This issue was investigated by the injecting six cutaneous neuropeptides-atrial natriuretic peptide, calcitonin gene-related peptide, neuropeptide Y, somatostatin, substance P and vasoactive intestinal peptide-into the skin of rats and by treatment of six cutaneous neuropeptides in rat peritoneal mast cell (RPMC) in vitro. All of the cutaneous neuropeptides increased the significant vascular permeability by the injection into the back skin of normal rats. All of the cutaneous neuropeptides also induced the mast cell degranulation and the histamine release from RPMCs, in a dose-dependent pattern, and increased the calcium uptake and decreased the level of cAMP of RPMCs in vitro. The effects of cutaneous neuropeptides on the vascular permeability and mast cell activation were inhibited by mast cell stabilization agent, disodium cromoglycate. These findings show that cutaneous neuropeptides can induce the mast cell activation by not only increasing the calcium uptake and decreasing the level of cAMP in RPMCs, but also the increment of vascular permeability in the skin of rats. 여섯 종류의 피부신경펩티드들 ANP, CGRP, NY, SOM, SP, VIP를 사용하여 피부에서 이들이 혈관투과성 증가 유 무와 비만세포의 활성화에 미치는 영향을 밝히고자 연구를 실시하여 다음과 같은 결과를 얻었다. 첫째, 여섯 종류의 피 부신경펩티드 모두 피부에서 혈관 투과성을 증가시켰다. 둘 째, 6종의 피부신경펩티드 모두 비만세포의 탈과립을 유발 하였다. 셋째, 6종의 피부신경펩티드 모두 비만세포로부터히스타민을 농도 의존적으로 유리시켰다. 넷째, 피부신경펩 티드 모두 비만세포의 칼슘 유입을 증가시켰다. 다섯째, 피 부신경펩티드 모두 비만세포의 cAMP 수준을 감소시켰다. 여섯째, 상기한 6종의 피부신경펩티드에 의한 비만세포의 활성화와 피부 혈관투과성 증가가 DSCG 전처리로 억제되 었다. 이상의 연구결과로 이들 피부신경펩티드들에 의한 피부 의 혈관투과성의 증가는 비만세포의 활성화에 기인할 것으 로 생각된다. 또한 피부신경펩티드는 비만세포내로의 칼슘 유입을 증가시키고, 세포내 cAMP 수준을 감소시켜 비만세 포의 활성화를 유발할 것으로 생각한다. 비만세포 안정화 물질(DSCG)이 피부신경펩티드에 의한 혈관 투과성 증가와 비만세포의 활성화를 억제하는 것으로 미루어, 신경펩티드 들과 비만세포들의 상호작용으로 유발되는 만성피부질환 등의 치료제로 비만세포 안정화 물질들을 사용할 수 있을 것으로 생각한다.

      • 흡수성 골고정장치(BioSorbFx®)를 이용한 안면부 골절 치료 1례

        저자는 안면부 골절환자에서 흡수성 골고정장치인 BioSorbFx®를 사용하여 6개월이 경과한 후에도 외부에서 만져져 제거한 예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. At the present day, the metal fixation devices are widely used in facial bone fracture surgery. The known disadvantages of these metal fixation devices are growth disturbances in the developing skull, atropy of the bone, corrosion, and artifacts in CT and MRI. To overcome these problems, there had been a continuous research on the development of a bioabsorbable skeletal fixation devices using polymers of polylactic and polyglycolic acid. And increasing the use of these devices in craniomaxillofacial surgery. We have used the BioSorbFx® in fracture of zygomaticofrontal suture line. But it was palpated by patient 6 months later. In operation, we identified plate was not absorbed at all. In conclusion, BioSorbFx® should be used carefully in thin skin and muscle area such as eyebrow, frontal area, and infraorbital rim.

        기부비동 확장증(Pneumosinus dilatans) 2례

        Pneumosinus Dilatans consists of an abnormal dilatation of the paranasal sinuses which contain air only and lined by normal mucosa. It is a rare condition, the etiology of which is unclear. A 16 year old male complained frontal bossing which developed slowly. Simple X-ray and CT showed abnormal distension of frontal sinus. Another 19 year old male complained slowly growing left cheek mass. Simple X-ray and CT showed abnormal distension of anteromedial wall of maxillary sinus. In view of the cosmetic appearance, operations were performed. Distended sinuses walls were removed and reconstructed using MedporeⓇ. I report 2 cases of pneumosinus dilatans which developed in frontal and maxillary sinuses and the literature reviewed.

        대퇴부의 지역 차단 마취후 광범위 피부 이식 공여부의 임상적 고찰

        In general, split thickness skin graft was done under general anesthesia. However, there was a difficulty to do general anesthesia in some cases due to poor general condition. The lateral cutaneous nerve block of the thigh(LCNBT) anesthesia the usual donor site of split-thickness skin graft in the thigh. Using 10 ㎖ of 0.5% bupivacaine, LCNBT was used thigh for harvesting split-thickness skin in 42 patients. Patients age ranged from 18 to 62 years with mean 49 years. The onset of full anesthesia took between 12 and 21 minutes. The area anesthetised ranged from 200 ㎠ to 940 ㎠ with mean 551 ㎠. The duration of full anesthesia was from 6 to 16 hours. In 6 patients, LCNBT was compared with previous lidocaine local anesthesia and all patients preferred to do LCNBT. In our experience, LCNBT is a safe, and simple method for harvesting split-thickness from the thigh and LCNBT provided good postoperative analgesia.

        AlloDerm??을 이용한 하지 연부조직 결손의 재건

        Variable methods have been using for reconstruction of soft tissue defect of feet and legs. Skin graft, local flap, and free flap have been using for this purpose. But, skin graft produced secondary contracture of recipient site and increased in donor site scarring. Local flap was limited its donor site and remained severe deformities of recipient site. Free flap was required prolonged operation time and secondary operation. Acelluar human dermal allograft, AlloDerm, was removed cellular elements of the epidermis and dermis. It was relatively immunologically inert. From August 1997 to April 1998, we have grafted acellular human dermal allograft with thin split-thickness skin graft on soft tissue defect of feet and legs in 7 cases. We concluded that cryopreserved acelluar human dermal allograft has been proposed as the solution to problems of skin graft, local flap, and free flap.

      • 클레임 조기경보 시스템에서 AHP 분석 기반 신경망 등급 결정

        Most companies maintain warranty databases for purposes of financial reporting and warranty expense forecasting. In some cases, there are attempts to extract engineering information from such databases. Another important application is to use warranty data to detect potentially serious field reliability problems as early as possible. When a serious problem arises, the existence of the problem will eventually be obvious. Early detection of serious problems through the use of sensitive statistical methods, allowing early action to mitigate potential reliability problems, could save large amounts of money and product good will. This paper describes a detection procedure that has been designed for this purpose. In addition to the AHP analysis, we suggest an neural network model for illustrating and describing the particular information in the data that caused the potential problem to be flagged. The methods are illustrated using data from an automobile warranty database.

      • FUzzyCLIPS 기반 임베디드 퍼지 전문가 시스템 개발

        대부분의 전문가 시스템은 독립적인 프로그램이라기보다는 매우 큰 응용 프로그램에 삽입되어 사용된다. FuzzyCLIPS는 CLIPS 규칙 기반 시스템의 확장된 버전으로 기존의 CLIPS가 갖는 기능이외에 퍼지 추론이 가능하다. 특히 FuzzyCLIPS 시스템은 큰 애플리케이션에포함될 수 없는 하나의 라이브러리 또는 소규모의 전문가 시스템을 구축할 수 있는 프로그래밍 환경을 제공한다. 본 논문은 퍼지 전문가 시스템을 쉽게 적응할 수 있는 하나의 임베디드 프로그램을 제안하는 데 있다. 적용되는 전문가 시스템은 제품 판매정책에 있어서 최근중요하게 대두되고 있지만, 정량적으로 분석하기가 어려운 고객 서비스의 핵심 요소인 서비스 연장과 품질을 사용한 판매정책을 나타낸다. 이러한 서비스 요인들은 고객관리와 수익이라는 측면에서 상당한 리스크를 수반하고 있기 때문에 의사결정을 위한 보다 정교한시스템의 사용이 요구되고 있다.

        쇄골두개이형성증 환자에서 Porous Polyethylene(Medpor®)를 이용한 두개골 함몰의 교정 1례

        Purpose: Cleidocranial dysostosis is a rare hereditary disorder affecting bones that develop by intramembranous formation. The typical features include excessive growth of transverse diameter of the skull, hypoplastic clavicles, low height and characteristic facial features. Methods: A 28 years old female patient visited by frontal area depression. The diagnosis was performed by computed tomographic study and radiographic imaging. The patient has widely opened anterior fontanelle, partial fused metopic suture, multiple wormian bone and supernumenary impacted teeth. Under the coronal incision, we exposed depressed frontal area and corrected with Medpor block carving. Results: Postoperatively, frontoparietal skull was aestheticlly improved and she was satisfied with the results. Conclusion: Authors report a case of cleiodocranial dysostosis who has been done correction of abnormal skull shape by Medpor® insertion.

        묻힌젖꼭지 교정 후 사용 가능한 간단한 젖꼭지 견인장치

        Purpose: The inverted nipple presents many problems in both cosmetic and functional aspects. The histopathologic features of inverted nipple are that inverted nipple has less fibromuscular tissue than normal nipple, short lactiferous duct and dense fibrous tissue. Many papers have been reported for correction of the inverted nipple, however not many medical literatures has specifically described about traction method. Although traditional traction methods were using half of urine cup or lid of bottle, they were uncomfortable and incapable for wearing brassiere. Methods: We invented a simple and more natural method using a dome-shape plastic disposable lid of take-out cup. We applied new device for three patients after corrective surgery. Results: For long term follow up, all patients satisfied their postoperative status and they were comportable with the lids. Conclusion: This new method is very simple, comfortable, and effective method for traction. It also allows patients to wear the brassiere.
