Balance can be defined as the ability to maintain the body's center of gravity within the base of support with minimal sway. Falls occur frequently in the elderly persons by the physiological change and dysfunction with age. Injuries resulting from falls include soft tissue damage and fractures of the radius, humerus, and femoral neck other consequences of falls include decreased mobility, reduced confidence, long lies (which can give rise to hypothermia, dehydration and pneumonia), and death Risk factors for falls have been classified as intrinsic (those related to the individual) and extrinsic(those associated with environmental features). Intrinsic factors include decreased strength, visual deficits, vestibular dysfunction, and decreased vibratory sensation in the feet. Improvement of the balance related to the increased probability for falls in the elderly persons and is important for fall preventions and improvement of the living quality of the elderly persons
임상검체에서의 Haemophilus aphrophilus 분리 5예

Haemophilus aphrophilus is a fastidious gram-negative bacillus found in mouth of normal individuals. Though H. aphrophilus infection is quite rate, it includes such serious ones as endocarditis and brain abscess. The authom isolated H. aphrophilus from five patients with the diagnosis of lung abscess, conjunctivitis, brain abscess and facial masticator space abscaa Two of the patients died. Three of the patients also yielded other species of bacteria from the same specimens. One of the isolate was intermediately susceptible to amikacin and resistant to tobramycin, indicating the necessity of a routine susceptibility test in order to select the proper antimicrobial agents. Since FS. Aphrophilus can be diffetentiated from other similar organisms by morphological and biochemical chamcteristics, one should determie the possibility of this organisrn when faptidious gram-negative bacilli are isolated from blood or from sites adjacent to upper respiratory tract.

The purpose of this study was to investigate the ethical values of senior students in nursing (N=111) and in medicine(N=82). The data were collected through self-reported questionnaires in April, 1999. Categories for classifying used in the content analysis of the responses were : (1) human life (2) patient relationship (3) task relationship and (4) co-worker relationship. The results of the study are as follows ; 1) The ethical values of students in nursing and medicine were in the direction of utilitarian in the area of human life. The ethical values of students in nursing and medicine were in the direction of a deontological position in the area of patient relationship. The mean score for students in nursing was significantly higher than for those in medicine(P<0.01) The ethical values of students in nursing and medicine was in the direction of utilitarian in the area of task relationship. The mean score for students in medicine was significantly lower than that of students in nursing(P<0.01). The ethical values of students in nursing and medicine was definitely in the direction of a deontological position in co-worker. The mean score for students in nursing was significantly higher than that of students medicine (P<0.05). 2) A positive correlation was found between the area of human life and patient relationship(P<0.05), patient and task relationship(P<0.01), and task and co-worker relationship(P<0.05) for students in nursing. On the other hand, the area of human life was positively correlated with patient, task and co-worker relationship area(P<0.05) for students in medicine. The area of patient relationship was also positively correlated with co -worker relationship for the students in medicine(P<0.01). 3) The ethical values of students in nursing were related to demographic characteristics ; degree of participation in religion(P<0.01)and degree of recognition of an ethical codes(P<0.05), on the other hand, the ethical values of students in medicine showed no significant differences according to demogrephic variables.
전압 장애성 원위부 신세뇨관성 산증과 신성 요붕증으로 발현한 Sjogren 증후군 1예

Distal renal tubular acidosis is a condition characterized by an inability of the distal nephron to acidify urine, causing hyperchloremic metabolic acidosis. Distal renal tubular acidosis is classified as proton secretory defect, permeability defect and voltage defect based on its pathophysiology. In the former two, serum level of potassium decreases due to increased excretion of potassium. But in the latter (voltage defect), hyperkalemia is characteristic by impaired the generation of an optimal electrical gradient for hydrogen ion and potassium secretion. We experienced a case of Sjogren`s syndrome associated with both voltage defect distal renal tubular acidosis and nephrogenic diabetes insipidus. The patient was a 58- year-old woman who complained of general weakness, nausea and xerostomia. Laboratory analysis showed metabolic acidosis with alkaline urine and hyperkalemia. Anti-nuclear antibody and anti-ds DNA antibody were positive. She presented with polyuria, low urine osmolarity and inadequate response to DDAVP. The response to Shirmer test was decreased. Salivary scintigraphy showed decrease of uptake in the parotid and submandibular salivary glands. We believe this is the first case report in which Sjogren`s syndrome is associated with both voltage defect distal renal tubular acidosis and nephrogenic diabetes insipidus.(Korean J Med 60:485-489, 2001)
