        증례 : 급성 췌장염 환자에서 복강동맥 협착과 동반하여 발생한 비장경색의 1예

        비장경색이 급성 췌장염의 합병증으로 발생된 보고는 드물며, 아직까지 복강동맥 협착과 연관되어 비장경색이 발생된 보고는 없다. 췌장염에서 비장 경색의 발생은 비장 혈관의 혈전에 의한 폐색에 의한 것으로 알려졌다. 그러나 급성 췌장염(특히 두부) 환자에서 비장색을 동반한 경우 가능한 원인으로 복강동맥 협착을 고려해 봐야 할 것이다. 본 증례에서 비장경색의 기전으로 협착후방 확장부의 혈전 형성, 혹은 췌장 두부 동맥으로부터 비장 동맥으로의 비정상적 역류에 의한 것으로 생각된다. 저자들은 급성 췌장염의 병력을 가진 환자에서 혈관조영 술상 발견된 무증상 복강동맥 협착이 비장경색과 연관 되는 1예를 경험하였기에 보고하는 바이다. Prevalence of splenic infarction developed during acute pancreatitis is extremely rare. However, we recently experienced a case of 42-year-old woman who developed splenic infarction during acute alcoholic pancreatitis. There were sustained subjective symptoms and no resolution of image despite of conservative management, so we performed angiography to confirm whether vascular lesion existed or not. We found the significant celiac artery stenosis due to compression by median arcuate ligament and no visible thrombus. We report an unusual case of splenic infarction developed during acute recurrent pancreatitis possibly related with celiac artery stenosis.(Korean J Med 71:198-202, 2006)

        잣뽕나무버섯과 곤봉뽕나무버섯의 생태학적 및 생리학적 특성

        Armillaria is a white-rot fungus that decomposes organics; it is also a root pathogen that kills trees. Recently, since the forest tending practice, the incidence of Armillaria infections on tree stumps has been increasing. Herein, we have examined the morphological features of Armillaria solidipes and A. gallica collected from 10 areas in Korea, and investigated the activities of the decomposing enzymes cellulase and laccase of these two Armillaria species. A. solidipes was found in eight host tree species including Pinus densiflora , Quercus serrata , and Castanea crenata at the altitude of 107~1,396 m, while A. gallica was found in three host species including P. densiflora and Fraxinus rhynchophylla at the altitude of 119~556 m. The fruiting bodies of A. solidipes are close with lamellae, and have brown to black hairs on the stipe, while those of A. gallica are distant with lamellae and clavate-shaped, and have yellow hairs on the base. The activites of cellulase of the two species were not different, but the activity of A. solidipes laccase was higher (12.94 ± 0.4 U/mL) than that of A. gallica laccase (9.14 ± 0.3 U/mL).

        유지 혈액투석 환자에서 저나트륨 투석액의 효과

        Purpose: Hypertension is a common problem in maintenance hemodialysis (HD) patients. We assessed the effects of low sodium dialysate on changes of blood pressure in maintenance HD patients. Methods: Forty HD patients were enrolled in this cross-over study. All the patients underwent nine consecutive HD sessions with the dialysate contained 138 mEq/L sodium (conventional sodium HD), then concentrations of sodium were switched to match the patients average pre-HD plasma sodium measured during the conventional sodium phase (135 mEq/L for patients with sodium levels less than 137, 137 for patients with sodium levels over 137). Dry weight and dialysis prescription were not modified during the six weeks of the study. Results: There was a significant decrease in the interdialytic weight gain (2.4±0.9 kg vs. 2.0±0.7 kg, p<0.001) and the interdialytic thirsty in low sodium HD sessions compare to conventional sodium HD sessions. Pre-HD systolic and diastolic blood pressure (BP), post-dialysis systolic BP was similar in both periods of the study. The use of low sodium dialysate is associated with significantly lower systolic BP in patients with uncontrolled hypertension (n=10,157.1±3.6 mmHg vs. 148.0±9.4 mmHg, p= 0.011), but not in those with controlled hypertension. Cardio-thoracic ratio was significant decrease in low sodium dialysate HD (0.53±0.08 vs. 0.51±0.07, p=0.002). The episodes of intradialytic hypotension and related symptoms were not more frequent in low sodium dialysate HD. Conclusion: Low dialysate sodium concentration based on predialysis sodium levels of patients could reduce the pre-HD systolic BP, interdialytic thirsty and interdialytic weight gain in maintenance HD patients.

        국제 금융중심지로서 부산의 발전전략

        The objective of this paper is to provide the development strategies of Busan for international regional financial center. Busan was selected as one of the two financial centers together with Seoul. For this purpose, we employed SFA(stochastic frontier approach) to figure out the relative advantages and properties of Busan for possible financial center in the future. In addition, two advanced financial cities, Hong Kong and Singapore are analyzed for case studies to obtain policy implications, and SWOT analysis is done to set right directions for Busan's strategy. Lastly, we suggest more specific policy agendas for the development strategies of Busan.

        뽕나무버섯속 버섯의 분류와 산림생태학적 및 병리학적 중요성

        산림에서 나는 뽕나무버섯속(Armillaria)은 봄부터 가을에 걸쳐 나는 식용버섯으로 유기물을 분해하는 백색부후 부생균(saprophyte)이면서 동시에 나무를 죽이는 뿌리기생 병원균으로 비절대기생체(nonobligate parasite) 또는 임의기생체 (facultative parasite)이다. 이들은 산림생태계에서 자연간벌과 천이에 큰 기여를 한다. 이 속에는 전세계 40여 종이 알려져 있고 기주식물로는 침엽수, 활엽수, 목본덩굴식물과 초본식물도 포함한다. 이 버섯의 유전적 개체(genet)는 산림에서 수명이 길어서, 약 2,000여년을 살면서 약 960 ha의 면적까지도 퍼져 나가기도 한다. 간벌 등 숲가꾸기로 그루터기가 많이 생기는 요즘 산림에서 발생량이 증가하고 있으며, 산촌에서 중요한 소득원으로 인식되고 있다. 따라서 뽕나무버섯에 대하여 정확한 동정이 필요하다. 이 총설에서는 뽕나무버섯속의 분류학적 문제와 국내 7개 분포 종의 형태에 의한 분류검색표, 산림생태학적 및 산림병리학적 의미를 토의하였다. Armillaria species are non-obligate root pathogens, as well as facultative parasites. The genus consists of about 40 species with worldwide distribution that are difficult to identify morphologically. Their hosts include conifers, hardwoods, vines, and even herbs. These fungi contribute to natural thinning and succession of vegetation in forests and decompose wood to develop edible fruiting bodies from spring to autumn. Its genets have a lifespan of up to 2000 years and have been found to occupy as much as 960 ha of forest land. Recently, forest tending work such as thinning of forests has resulted in the creation of tree stumps that support the growth of Armillaria, and these mushrooms have become an income source in mountainous areas. Therefore, identification of these species is needed. We review the difficulties in identification, suggest a species identification key for Korean indigenous species, and discuss the significance of Armillaria in terms of forest ecology and plant pathology.

        산림복지서비스 공간 관리를 위한 가을철 도시숲의 온열환경쾌적성 평가에 관한 연구

        본 연구는 가을철 도시숲 온열환경요소와 인간의 쾌적성과의 관계를 규명하고 숲환경과 실내환경의 공간별 온열환경의 차이를 알아보기 위한 목적으로 수행되었다. 연구 대상지의 온열환경 측정을 위하여 기온, 습도, 풍속, 복사열을 측정하였다. 온열환경쾌적성 평가를 위해 대학생 남녀 67명이 피험자로 참가하였으며, 도시숲 환경과 실내환경에서 앉은자세로 5분간 안정을 취한 후, 심리·생리지표를 측정하였다. 심리지표로는 기분상태검사(POMS-B), 긍정부정척도(PANAS), 주관적 쾌적도를 측정하였다. 생리지표로는 심박변이도(HRV)를 측정하였다. 온열환경과 심리·생리지표 관계분석에서 풍속과 심박변이도의 HF성분 간에 음의 상관관계가 나타났으며 온열환경과 주관적 쾌적성에서는 기온, 풍속, 복사열과 온열민감도(TSV) 간의 양의 상관관계 그리고 습도와 온열민감도 간의 음의 상관관계가 나타났다. 숲환경과 실내환경과의 차이 비교 분석에서는 숲환경에서 기분상태와 심박변이도의 HF 성분이 실내환경보다 유의하게 높다는 사실이 확인되었다. 본 연구를 통하여 가을철 도시숲이 실내환경보다 쾌적하며 가을철 도시숲 내 산림복지서비스 활용 공간 계획에 있어서 온열쾌적성의 중요성을 제시할 수 있었다. This study was conducted to identify the relationship between urban forest thermal comfort factors and human comfort during the fall season and to find out the difference in thermal comfort between a forest and an indoor environment by space. Temperature, humidity, wind speed, and radiant heat were measured to measure the thermal comfort of the experimental site. Sixty-seven university students participated as subjects for the comfort evaluation. We measured psychological and physiological indicators after five minutes of sedation in the urban forest and indoor environments. Psychological indicators measure Profile of Mood State (POMS-B), Positive and Negative Affect Schedule (PANAS), Thermal Sensation Vote (TSV), and Comfort Sensation Vote (CSV) as a subjective comfort. The physiological indicators also measured the heart rate variation (HRV). Analysis of the relationship between thermal comfort and psychophysiological index showed a negative correlation between the HF power of HRV and wind speed, and a positive correlation between HF power of HRV and temperature, wind speed, radiant heat, and Thermal Sensation Vote (TSV). Comparative analysis of differences between the forest environment and the indoor environment confirmed that HF of HRV and mood conditions of humans in the forest environment are significantly higher than in the indoor environment. Through this study, the autumn urban forests are more pleasant than the indoor environment, and the importance of thermal comfort in planning a space for utilizing forest welfare services in the autumn urban forests can be suggested.

        증례 : 신장 ; 면역글로불린과 Leflunomide로 치료한 BK 바이러스 신병증 1예

        저자들은 면역억제제의 감량과 항바이러스제치료에도 진행하는 BK 바이러스 신병증을 면역글로불린으로 치료한 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다. BK virus nephropathy has emerged as an important cause of renal allograft dysfunction. It affects 1-10% of renal transplant patients and results in significant graft dysfunction in more than 50% of cases. A reduction in the amount of immunosuppressants is not an appropriate treatment option for advanced stage BK nephropathy; therefore, other treatment strategies need to be considered such as cidofovir, leflunomide, and intravenous immunoglobulin (IVIG) in combination with reduced immunosuppression. The use of IVIG may be a valuable treatment option in patients with BK virus nephropathy. We report our experience with IVIG rescue therapy in a patient and the progression of BK nephropathy despite leflunomide therapy. (Korean J Med 2011;81:512-516)

      • SM25C의 마찰용접에서 용접면에 가공된 홀의 영향에 관한 연구

        This study deals with friction welding of SM25C. One of test material has a hole in the center, but the other is not. Then,same friction welding condition is applied. Friction welding is conducted with varying hole diameter at one material. The results obtained are as follows. 1. When the diamether of machined hole is 4mm, the highest bending strength of friction wleld was 1569MPa which was about 120% of base metal strength. 2. When the diameter of machined hole is 6mm, the highst shear strength of friction weld was 512MPa which was about 115% of base metal strength. 3. The machined hole at one material can be increased in the bending and shear strength.
