        아동을 대상으로 한 정제형 불소함유구강세정제의 치면세균막제거 및 치은염완화효과에 관한 임상적 실험 연구

        The authors have examined the plaque index for 56 primary school children who had used fluoride tablet(Denclean, Suheung Capsule Co, Ltd., Korea), which contained with sodium fluoride and aspartame, instead of fluoride mouth rinsing solution, in order to develope convenient method for school based fluoride mouth rinsing program. 2 weeks clinical tests with fluoride tablets after lunch were performed. PHP index & PMA index were surveyed to check the effects on removal of plaque and reduction of gingivitis and questionnaire methods were analysed to find out their interests for fluoride tablet. The obtained results were as follows. 1. PHP index was significantly reduced from 0.53 to 0.31 immediately after using fluoride tablet(p<0.05) and 0.53 to 0.05 after daily application period for 2 weeks(p<0.05). 2. PMA index was also significantly reduced from 1.78 to 0.28 after daily application period for 2 weeks(p<0.05). 3. Fluoride tablet was recommended to use for cleansing teeth and fluoride application for children instead of fluoride mouth rinsing after toothbrushing as a second choice, because it is simple to use and easy to perform continuously even though it was probed less effective for elimination of dental plaque than toothbrushing, 4. There was no evidence for side effect with the use fluoride tablet and developement of more simple and favorable method for application of fluoride tablet was needed.

        시범학교 불소용액양치사업 3년간의 효과평가 연구

        저자등은 학교구강보건사업의 대표적인 사업인 불소용액양치 사업의 효과를 보기 위하여 목천초등학교 고학년 활동 164명을 대상으로 3년간 0.05% 불화나트륨 용액과 10% Xylitol을 주성분으로 제조된 치카치카 용약을 가지고 불소용액양치 사업을 실시하고 학동들의 구강검사르 통하여 우식경험 영구치에 대한 조사를 실시한 구 대조군 아동들의 구강 검사와 비교하여 우예방효과를 산출한 결과 다음과 같은 결론을 얻었다. 1. 사업대상 아동들의 구강검사결과 영구치 우식경함자율은 9세 아동에서 61.2%, 10세 아동에서 62.3%, 11세 아동에서 72.6%로 나타나 대조군 아동에 비해서 우식경함자율에 있어서는 별차이가 없었다. 2. 우식경험영구치율은 9세 아동에서 13.9%, 10세 아동에서는 11.4%, 11세 아동에서는 8.9%로 대조군 아동에서보다 평균 38.3%로 예방 효과를 나타냈다. 3. 우식경험 영구치지수는 9세 아동에서 1.98±0.82, 10세 아동에서 1.79±0.71, 11세 아동에서는 1.92±1.01로 나타나서 대조군 아동과 비교할 때 9세에서는 51.2%로 평균 36.9%의 우식예방효과를 나타내었다. 4. 0.05% 불화나트륨과 10% Xylitol을 함유한 용액으로 실시한 학교 불소용액 양치 사업은 계속 시행할수록 우식예방이 점차 높아지므로 지속적으로 시행되어야 할 것으로 사료되었다. The authors have studied for caries prevention effects of fluoride mouthrinsing program for 3 years. 164 school children were orally examined after mouthrinsing with 0.05% NaF and 10% xylitol solution everyday for 3 years. And compared the results with these of other school children. The obtained results are followings. 1. DMF rate were examined as 61.2% in 9 years old. 62.3% in 10 years old and 72.6% in 11 years old, and no difference with the control group. 2. DMFT rate was 13.9% in 9 years old, 11.4% in 10 years old and 8.9% in 11 years old, so it is estimated that average 38.3% reduction of caries incidence for DMFT rate, compared with the control group. 3. DMFT index was examined as 1.98±0.82 in 9 years old, 1.79±0.71 in 10 years old and 1.92±1.01 in 11 years old. It is estimated that 23.0% of caries reduction effect in 9 years old, 34.7% in 10 years old and 51.2% in 11 years old and average 36.9% reduction effects were calculated. 4. It is recommended that fluoride mouthrinsing program with 0.05% NaF and 10% xylitol solution was effective to prevent the caries incidence for school children.

        Calcium glycerophosphate와 Flavonoid 및 Vitamin - E 배합세치제의 치면세균막제거효과 및 구취제거효과에 관한 실용실험적 연구

        In order to evaluate the effect of a therapeutic toothpaste containing calcium glycerophosphate flavonoid and vitamin-E on the inhibition of dental plaque formation, the authors had examined the oral status of 96 students aged twentieth, divided into two groups, 48 in one control group were given with cosmetic toothpaste, 48 students in the other experimental group with therapeutic toothpaste containing CGP, Flavonoid, Vitamin-E. At the same time, all students were given with straight handled medium bristled toothbrush and directed to brush teeth with toothpaste and toothbrush offered. The patient hygiene performance were repeatedly examined in the 7th day, and 14th day. On the other hands, to evaluate the malodor reducing efficiency, the authors and analysed the volatile sulfur compounds from the breath after ingestion of garlic powder. 5 adults aged twentieth were examined. To obtained adequate data, expirating breath were collected into sampling loop, after ingestion of garlic powder, the subjects cleansed the oral cavity by one of the designated methods and reevaluated at 2, 7, 12 and 17 minutes after ingestion of garlic powder. Gas chromatographic technic was employed for the evaluation of volatile sulfur compounds in mouth air. The percentage change in VSC concentration of air samples were calculated in relation to values of it after ingestion of garlic powder immediately. Thereafter, the data was compared statistically and discussed. The obtained results were as follows; 1. Calcium glycerophosphate, flavonoid and vitamin-E containing toothpaste inhibited plaque formation more effectively than control cosmetic toothpaste(p<0.05). 2. Methyl mercaptan and allyl mercaptan were detected in the breath after ingestion of garlic powder. 3. Calcium glycerophosphate, flavonoid and vitamin-E containing toothpaste reduced more methyl mercaptan than control cosmetic toothpaste(p<0.05). 4. Calcium glycerophosphate, flavonoid and vitamin-E containing toothpaste reduced more allyl mercaptan than control cosmetic toothpaste(p<0.05) 5. Therapeutic toothpaste containing calcium glycerophosphate, flavonoid and vitamin-E increased the effects of plaque formation inhibition & malodor elimination.

        한국 노인틀니 보험급여를 위한 보험재정 추계에 관한 조사연구

        The use of insurance to help spread the financial burden attendant upon geriatric populations in Korea is of a governmental issue. Much of recent issues is the effort to apply this principle to current dental expenses. Therefore, the purposes of this study were (1) to determine the actuarial budget of national insurance payment for denture services of Korean geriatric patients whose age are more than 70-year and (2) to provide a guideline for the national oral health care policy of the insurance payment system for denture service. An opinion survey form was mailed to an approximately 10% of general dental practitioners (1,164) and 260 returned survey responses were analysed. Also, conducted were a telephone interview with 64 general dental practitioners who were willingly participated out of initial contact of 80 practitioners concerning all expenses incurring for the maintenance of dental clinics and personnel. Preexisting informations concerning the dental technician's fee schedule and the cost of materials needed for the construction a denture were also compiled form the Korean Dental Technician's Association and the Korean Association for Dental Materials and Instruments for the actuarial cost to determine the total budget and insurance payment for the denture services of geriatric population in Korea. The results were as follow. 1. If all geriatric population, who need for denture services and their age is more than 70 years-old in Korea, were provided the necessary services in 1998, the total actuarial cost of 80.2 billion won for the resin-based full dentures and 227.1 billion won for non-precious cast metal partial dentures, respectively. If these cost were estimated by the current insurance reimbursement plans in which 30% of expense-sharing by the patient, the total expenditure to the Korea Health Care Union is a 215 billion won comprising of 56.1 billion won for the resin-based full dentures and 158.9 billion won for nonmetal cast partial dentures. However, the actuarial costs will be decreased as a function of time because the number of patients who need a new denture service will be decreased as their denture services were provided. 2. If all geriatric population, who need for denture services and their age is more than 70 years-old in Korea, were provided the necessary services under the base of 300,000 won per a denture service in 1998, the total expenditure of 150.5 billion won comprising of 40.7 billion won for the resin-based full dentures and 109.8 billion won for non-precious cast metal partial dentures, respectively. It is also expected that the cost will be decreased as function of time. Therefore, at present time, the most reasonable method is to revise the monetary reimbursement system to adjust the current situations.
