호스피스·완화의료 사회복지사의 심리사회적 사정항목에 대한 중요도 인식

목적: 본 연구는 우리나라 호스피스 완화의료의 심리사회적 표준사정항목을 개발하기 위한 예비연구로써, 심리사회적 사정항목의 중요도에 대한 호스피스 완화의료 사회복지사의 인식을 조사하였다. 그리고 의료기관의 유형과 사회복지사의 자격조건에 따라 심리사회적 사정항목의 중요도에 대한 인식이 어떤 차이가 있는지를 분석하였다. 방법: 2013년 5월 30일부터 10월 31일까지 전국 55개 완화의료지원기관에 정부인증 호스피스 완화의료기관 10곳의 사회사업초기면접 기록지와 선행연구결과를 반영하여 8개 범주 80개 항목으로 구조화된 설문지를 이메일로 발송하였다. 이 중 조사 참여에 동의한 31개 기관(56%)의 응답을 분석하였다. 수집된 자료는 SPSS 18.0을 이용하여 분석하였다. 결과: 심리사회적 사정영역의 8개 범주 중 가족사항에 대한 평균치가 4.53으로 가장 높고 경제적 사항이 4.06으로 가장 낮게 조사되었다. 또한 사회복지사 자격증, 의료사회사업수련, 호스피스 완화의료 전담여부등 사회복지사의 전문적 자격특성과 병원의 유형에 따라 중요도에 대한 인식이 다른 것으로 나타났다. 결론: 특히 본 연구결과 심리사회적 사정항목의 수행도가 기관마다 차이가 있는 것으로 조사되었고 사정항목을 중요하게 인식함에도 사정하고 있지 않는 등의 편차가 나타나 표준사정항목을 만드는 것이 필요하다고 할 수 있다. 둘째 법적 제도화를 통해 사회복지사의 자격기준과 호스피스 완화의료를 전담할 수 있는 여건을 마련하고 전문교육을 강화하는 방안이 필요하다. Purpose: This preliminary study is aimed at developing standardized tools for psycho-social assessment of patients in needs for hospice/palliative care. To accomplish the purpose, investigators examined effects of perceptions of social workers on the importance of psycho-social domains of assessment in hospice/palliative care settings. Moreover, investigators paid attention to variances of perceptions of social workers' along with types of institution and credentials of those family settings. Methods: A form of questionnaire was first explored from an initial interview assessment of 10 government-certified hospice care providers and a literature review, second constructed with eight domains and 80 items, and sent by e-mail to 55 institutions and hospitals providing hospice/palliative cares in Korea. Lastly, a total of 31 agencies returned with a completed responses and consent form (56% response rate). SPSS program (version 18.0) was used for data analysis. Results: Study found that social workers perceived patients' family background (m=4.53, 5-point scale) as the most important assessment domain, whereas economic conditions (4.06 point) the least important. Social workers' perception varied by credentials (i.e., license types, training, full-time position, types of care facility). Conclusion: Based upon study findings, investigators can conclude strong needs for developing a assessment tool that measures multiple domains (i.e., psychological, social and ecological aspects) of patients. A standardized assessment tool should be structured with 2 axis (center/core and expanded/peripheral) and tailored for institution type. Second, professional trainings must be provided by strengthening legal institutionalization and fostering qualified social workers with full responsibilities of hospice and palliative care patients.

G. E. 무어와 무관심의 신(新)윤리

본 논문은 G. E. 무어의 『윤리학 원리』를 지성사의 관점에서 새롭게 해석하려는 시도이다. 우선 고전 공리주의의 변천 과정 속에서 무어의 문제의식을 진단해 보았다. 벤담의 쾌락주의적 공리주의가 밀의 행복주의적 공리주의로, 그리고 다시금 무어의 이상적 공리주의로 변천하는 과정에서 ‘경제학’과 ‘사회학’이라는 두 사회과학의 발전이 외적 동인으로 작용함을 지적하면서 윤리학의 자율성이 새롭게 문제되는 맥락을 짚어 보았다. 그 다음으로, 『윤리학 원리』 5장의 내용을 중심으로 무어가 윤리학의 자율성을 어떤 의미로 이해했고, 왜 강조할 수밖에 없었는지 그 이유를 따져 보았다. 윤리학의 자율성은 곧 개인의 자율성으로 연결됨을 지적하면서, 무어의 윤리학이 현대의 개인주의적 윤리의 시원임을 밝히는 데 또한 주력했다. 마지막으로 『윤리학 원리』 6장의 내용을 중심으로, 윤리학사 전체에 대한 무어의 비판이 가치 분류 방식에 직결되어 있음을 밝혔다. In this article I considered G. E. Moore’s Principia Ethica as a precursor of contemporary individualistic ethics. I started from reviewing the development of Classical Utilitarianism from Bentham’s hedonistic utilitarianism to Moore’s ideal utilitarianism via Mill’s eudaimonistic utilitarianism, so that we could detect the internal and external causes of this development. It is the incredible success of two social sciences, Economics and Sociology, that functions as main external cause. This success also constitutes the context in which the autonomy of ethics matters to Moore. From the chapter 5 of Principia, I tried to prove it, and unfold Moore’s intention. According to him, the autonomy of ethics matters in so much as it serves as a defense of the autonomy of individual, that is to say, autonomy of individual judgement about what is good in itself. In this sense, this article considered Moore’s ethics as the origin of modern individualistic ethics. The chapter 6 of Principia gives us an occasion to see the basic difference between traditional ethics and modern individualistic ethics in reference to the classification of values.

‘영웅-순교자’ 화두와 밀 사상의 통일성

본 논문은 ‘영웅-순교자’라는 화두를 통해 밀 사상의 통일성을 새롭게 조명해 보고자 했다. 사상적 성숙기에 집필된 『공리주의』와 『자유론』사이에 이념적 불일치가 발생함을 주석가들은 지적해 왔다. 밀의 소극적 자유 개념이 필연적으로 모든 간섭주의적 성격의 법을 배제하는데 반해, 공리주의의 제1원리인 ‘최대 다수의 최대 행복’의 원리는 그러한 법을 요구할 수 있다는 것이다. 공시적 관점에서 제기된 밀 사상의 통일성 문제는 곧장 전⋅후기 사상 사이의 단절이라는 통시적 문제로 이어진다. 아버지 제임스 밀로부터 물려받았던 공리주의를 자유주의자 존 스튜어트 밀은 저버렸던 것일까, 아니면 여전히 계승하고 있는 것일까? 공시적 관점과 통시적 관점 모두에서 밀 사상의 통일성을 논하고자 하면서, 본 논문은 ‘영웅-순교자’ 화두가 지니는 논쟁적 의미와 발견술적 의미에 초점을 맞추었다. In this study, I made a new approach to Mill’s thoughts with the theme ‘hero and martyr’. In general, the modern scholars have presumed that a doctrinal inconsistence takes place between Mill’s two masterpieces, Utilitarianism and On Liberty. According to them, while the latter rejects all kinds of legislative intervention by the force of its ‘self-protection principle’, the former may accept interventional laws if they satisfy the utilitarian principle ‘the greatest happiness of the greatest number’. This remark on the doctrinal inconsistence of Mill’s two works introduces a new debate on Mill’s final position vis-à-vis benthamian utilitarianism. Did the liberalist Mill abandon the utilitarianism inherited from his father James Mill, or take it as maxim in advocating liberal society? The objective of this paper was to reveal the unity of Mill’s thoughts in diachronic and synchronic perspective, and for that objective I treated the theme ‘hero and martyr’ in its polemic and heuristic meaning.

스피노자의 도덕심리학

본 논문에서 필자는 목적론에 대한 스피노자의 이중적 평가를 중심으로 그의 도덕심리학을 재구성해 보고자 했다. 『에티카』1부 부록에서 스피노자는 목적론적 사고를 사물의 원인에 대한 무지와 자기보존의 충동에서 기인한 편견으로 평가절하한다. 이는 그 자신이 1부 본문에서 제시한 필연론에 부합하는 평가이다. 하지만 4부 서문에서 스피노자는 목적론적 사고에서 파생된 선·악, 완전함·불완전함 등의 가치개념을 재수용해야 함을 역설한다. 윤리적 평가의 기준이 될 ‘인간 본성의 전형`을 구성하기 위해서는 그러한 개념들이 절대적으로 필요하기 때문이다. 필자는 목적론에 대한 두 평가가 어떻게 일치될 수 있는지를 살펴봄으로써 『에티카』의 연역적 논증 체계 이면에 작동하는 스피노자의 도덕심리학을 수면 위로 드러내 보이고자 했다. 더불어, 규범성의 문제가 스피노자 철학의 핵심 문제임을 지적함으로써 『에티카』전체를 일종의 도덕심리학으로 읽는 필자의 독법을 정당화하고자 했다. In this paper, I tried to construct Spinoza`s moral psychology from his double evaluation about teleology. On the one hand, as in the famous appendice of De Deo the first part of Ethica, Spinoza denounced it as an illusion due to ignorance and desire. This negative evaluation of teleology fits well with the necessitarism logically constructed in the main text of De Deo. On the other hand, as in the preface of the forth part of Ethica, Spinoza insisted on the positive role of the values, like the good and the bad, the perfection and the imperfection, although they originated from teleological thinking. He said that these values are useful to us in order to form a model of human nature, model indispensable for our ethical evaluations. These two seemingly contradictory statements lead us to rethink Spinoza`s logical system and to find a moral psychology working under the surface. In this perspective, I tried to read Spinoza`s Ethica somewhat heretically. However, I think that if we construe that Spinoza`s problematics consist in the problem of normativity, such reading can be justified.

데카르트, 인지주의 감정이론의 기원과 한계

본 논문은 데카르트의 『영혼의 정념들』을 통해 현대 인지주의 감정이론의 기원과 한계를 살펴보려는 의도에서 쓰였다. 데카르트가 왜 인지주의적 입장을 취할 수밖에 없었는지를 보이기 위해, 도덕주의적 감정론과 생리학적 환원주의에 대한 배격이 그의 철학 내부로부터 요청되고 있음을 우선 설명했다. 그 다음으로 인지주의 감정이론의 핵심 개념인 지향성과 명제성이 데카르트의 감정론 내부에서 어떤 역할을 수행하는지 설명했다. 『영혼의 정념들』 1부와 2부의 논증구조를 중심으로 분석하면서, 정념 일반에 대한 데카르트의 정의가 지향성을 전제로 하고 있으며, 감탄ㆍ사랑ㆍ미움ㆍ욕망ㆍ기쁨ㆍ슬픔이라는 여섯 모(母)정념들의 논리적 선후관계가 명제적 태도를 기반으로 하고 있음을 보여주고자 했다. 마지막으로 ‘감정의 양가성’이라는 문제를 통해 분석성과 체계성을 지나치게 강조한 데카르트의 감정론이 봉착할 수밖에 없었던 딜레마를 설명했다. This paper is intended to show the origin of the cognitivist theory of emotions and its limit by showing the structure of Les Passions de l" Ame. At first, it needed to explain why Descartes took the cognitivist position. So the first section of this paper was contributed to Descartes’ critics against traditional theories of emotion, theories eminently based on a moral point of view, and his critics against physiological reductionism that he found in Hobbes’ theory of emotion. That these two-track critics came from Descartes" concept of science is the main thesis of our first section. In the second section of this paper, we tried to prove the role of intentionality and propositionality - two principal concepts of cognitivism - in the Cartesian theory of emotions. By a structural analysis of Les passions de l"Ame, we could find out that Descartes aimed at defining the general meaningof passion in terms of intentionality, and that his selection of six primitive passions such as admiration, love, hate, desire,joy, sadness, was logically related to the propositional attitude. At the third part of this paper, we examined the question of ambivalence with the intention to reveal the dilemma of Cartesian theory, the dilemma to which a theory of emotion inevitably falls insofar as it emphasizes systematicity and analyticity.
