Myotonic dystrophy 1 (DM1) is a multi-system disorder characterized by endocrine defects that include testicular and tubular atrophy, oligospermia and azoospermia, and increased follicle-stimulating hormone levels. We describe a rare case of DM1 presenting as infertility in a 29-year-old man.

Purpose: To evaluate differentiation of clinical course and outcomes between patients with acute prostatitis only and acute prostatitis with prostatic abscess. Materials and Methods: This retrospective study examined the records of 68 patients with acute prostatitis from January 2006 to June 2010. These patients were divided into two groups according to the presence of the prostate abscess: group 1 (prostate abcess; 18 patients, mean age 59.1±13.3 years) and group 2 (acute prostatitis without prostatic abscess; 50 patients, mean age 57.7±14.6 years)-. We evaluated clinical parameters including degree and duration of fever, admission period, transrectal ultrasonographic findings, and laboratory test including prostate specific antigen (PSA), white blood cells (WBC), erythrocyte sedimentation rate (ESR), C-reactive protein (CRP), urinalysis, urine culture, and antibiotic sensitivity. Results: The degree of fever in group 1 (38.4±<1.3℃) was higher than group 2 (37.7±0.9℃) (p=0.024), and duration of fever and admission period (3.2±3.0 days vs 1.5±1.6 days, respectively; p=0.006) (22.2±12.6 days vs 6.7±2.9 days, respectively; p<0.001) were longer in group 1. Total prostate volume in group 1 (46.6±720.3ml) was larger than group 2 (32.9±13.9ml) (p=0.002). The results of laboratory tests indicating inflammation in group 1 were higher than group 2 (WBC; 20,592±13258/uL vs 14,577±9097/uL, p=0.040, ESR; 65.1±22.3mm/hr vs 34.3±11.9mm/hr, p=0.001, CRP; 19.4±10.7mg/dL vs 12.1±9.4mg/dL, p=0.023). The causative organisms in group 1 were more resistant to quinolone than group 2 (p=0.019). Conclusions: The clinical course of prostate abscess is more severe and longer than acute prostiaitis. We propose that early imaging study for diagnosis of prostatic abscess in acute prostatitis patients with low response to initial empirical conservative treatment for 2 days. Because antibiotic resistant rate was higher, careful choice of antibiotics and different therapeutic plans including abscess drainage will be needed in patients with prostatic abscess.

Purpose: The aim of this study was to determine the prevalence and disease-specific antimicrobial susceptibility of Escherichia coli in urinary tract infections (UTIs).Materials and Methods: A total of 862 patients older than 18 years of age, who were diagnosed with UTI between January 2013 and December 2015, were included. The results of urine culture, prevalence of extended-spectrum beta lactamase (ESBL)-producing E. coli, and antimicrobial susceptibility by disease were also examined. Results: A total of 862 uropathogens were isolated. Among then, E. coli accounted for 756 (87.7%) isolates. The susceptibility rates of E. coli to the following antimicrobial agents were as follows: ampicillin 29.4%, cefazolin 70.5%, ceftazidime 75.1%, cefotaxime 75.0%, cefepime 76.2%, cefoxitin 88.8%, amoxicillin-clavulanic acid 63.6%, trimethoprim-sulfamethoxazole 60.6%, gentamicin 71.4%, ciprofloxacin 73.0%, piperacillin/tazobactam 93.9%, amikacin 99.2%, imipenem 99.1%, and ertapenem 99.3%. The frequency of ESBL-producing E. coli strains was 24.6%. The antimicrobial susceptibility of UTI varied by each disease, but without statistical significance.Conclusions: It is necessary to regularly examine the disease-specific resistance rates to determine the appropriate empiric antibiotic treatment, and the national antibiotic usage policies must be reorganized according to the data obtained from these studies.

Objective: Emergency medical services (EMS) personnel are at a high risk of spreading infections. Compliance with the EMS handwashing regulations is difficult due to frequent dispatches and on-site treatment. This study sought to investigate the education status and awareness about handwashing of EMS personnel, their performance of the recommended handwashing pattern, and their compliance with infection control protocols. Methods: A total of 677 paramedics were educated on handwashing methods and performance and 76 of them were surveyed on handwashing awareness and performance. The survey tool comprised a total of 15 questions, including three on general characteristics, six recognition questions, and six performance questions. The data were analyzed using the SPSS version 17.0 program. Results: Sixty-five percent of the EMS personnel were educated on handwashing methods. The higher the work experience, the higher the performance of handwashing after personal hygiene activities. Hence, there were significant differences in performance among the personnel (P<0.001). Overall, it was found that EMS personnel with a high awareness of handwashing methods, regardless of their occupation, had a good knowledge of the appropriate methods of cleaning and disinfecting ambulances and were able to treat and control infection well. The higher their awareness regarding handwashing, the higher the performance of the same (P<0.001). Conclusion: It is necessary to develop annual evaluation programs, a customized education curriculum for each job, and conduct regular education for paramedics.

The Bosniak renal cyst classification has been accepted by urologists and radiologists as a way of diagnosing cystic renal masses and determining the management approach. We report two cases of a renal cystic mass that showed a category change from category II on the basis of enhanced computed tomography to category IV after further gadolinium-enhanced magnetic resonance imaging. In both cases, the cysts were later confirmed as kidney cancer by pathology.

The Korean Association of Urogenital Tract Infection and Inflammation and the Korea Disease Control and Prevention Agency regularly update, revise, and develop new content for the Korean sexually transmitted infection (STI) guidelines. These professional bodies respond to changing epidemiological trends and evolving scientific evidence, and consider advances in laboratory diagnostics and research. The principal recommendations of the 2023 Korean STI guidelines in terms of viral infection follow: 1) If genital herpes recurs more than 4–6 times annually, suppressive therapy with acyclovir 400 mg orally 2 times/day or famciclovir 250 mg orally 2 times/day or valacyclovir 500 mg orally once a day (for patients with <10 episodes/year) or valacyclovir 1 g orally once daily (for patients with ≥10 episodes/year) is recommended to prevent recurrence; 2) molecular human papillomavirus (HPV) testing is not recommended as a routine test for STI status, nor for determination of HPV vaccination status; and 3) patients should inform their current sexual partners about anogenital warts because the types of HPV that cause such warts can be passed to partners. These guidelines will be updated every 5 years and will be revised when new knowledge on STIs becomes available and there is a reasonable need to improve the guidelines. Physicians and other healthcare providers can use the guidelines to assist in the prevention and treatment of STIs.

Spontaneously ruptured renal cell carcinoma (RCC) in end-stage kidney disease is very rare. Preoperative diagnosis is difficult because of the relatively small tumor size, associated hematoma, and surrounding acquired cysts. Two middle-aged men who were maintained on hemodialysis (HD) for over 10 years suddenly developed flank pain during HD. Computed tomography scans revealed an enhancing ruptured renal mass in one patient, and no obvious tumor lesion except for a hematoma in the other, both of which were later confirmed as RCCs by pathologic specimens.
