The pathogenesis of the primary spontaneous pneumothorax is the rupture of subpleural bleb or bullae and subsequent sudden collapse of the affected lung. Mostly, the bullae or blebs are present bilaterally, but detecting the number, size, and location of the causating foci by plain chest film is quite difficult . We have performed chest CT scans for detecting the bullous lesions in 33 cases of primary spontaneous pneumothorax, and compared the results with surgical findings.1. Seventy-four blebs were identified in the chest CT scan, and 100 blebs or bullae were detected surgically [ Sensitivity was 0.74 ].2. Diagnosis rate was 80% [40/50] at right upper lobe, 75.7% [28/37] at left upper lobe, 55.6% at right lower lobe, and 25.0% at left lower lobe, respectively.3. Blebs or bullae smaller than 1 cm of its diameter were detected by 57.1% [24/42] of sensitivity, and in the cases of size larger than 1 cm, it revealed 86.2% [50/58] of sensitivity respectively.4. Of the 45 cases, 7 cases were false negative [15.6%], most of these were ruptured or small size [< 0.5 cm]. 5. One case was false positive, which was irregular adhesion at the apex of the lung.6. We could detect blebs or bullae with preoperative CT scans in 84.4% [38/45] of total patients. In conclusion, chest CT scan is a very advantageous diagnostic tool for proper management and preventing recurrence of primary spontaneous pneumothorax patient.
Bileaflet Mechanical Valve의 임상적 고찰

Experience with bileaflet mechanical valve replacement at the Inha Hospital in 192 patients, operated on from June 1986 until April 1993. Two hundred fourty-one prostheses [51 Duromedics, 79 St.Jude Medical, and 111 CafboMedics]were implanted during the total 195 operations. Mitral valve replacment[MVR]was done in 113 cases, aortic valve replacement[AVR]in 34, tricuspid valve replacement[TVR]in 2, and double valve replacement[DVR]in 46 cases.Of the total patients, 63.0% were women and 37.0% were men. The mean age of the patients was 40.8 years, ranged from 14 to 67years. Overall early mortality was 9.2\ulcorner%[18 out of 195]; 9.7%[11 out of 113]for MVR, 14,7% [5 out of 34]for AVR, and 4.3%[2 out of 46]for DVR. All of the operative survors were followed over a period of one to 83 months with a mean of 37 months, for total 543 patient-years. So far, eleven patients[6.7% of the long-term survivors]were lost to follov-up after a mean postoperative follow-up of 22.8 months. There were nine late deaths; three deaths due to prostetic valve endocarditis, two due to persistent heart failure, one due to cerebral hemorrhage, one due to aortic dissection after Bentall oreration, and two sudden deaths. Actuarial survival rate at 6.9 years was 94.8%, There were seventeen valve-related complications; three prosthetic valve thromboses, three thrombembolisms, three instances of prosthetic valve endocarditis, two paravalvular leakages, and six hemorrhagic complications related to anticoagulation. The actuarial rate of freedom from all valve-related complications at 6.9years was 91.3%. There were significant decreases in the heart size postoperatively that can be demonstrated by comparison of cardio-thoracic ratios on simple chest X-ray and left ventricle dimensions on echocardiography. We conclude that this midterm follow-up shows good results in terms of hemodynamics and durability although further long-term evaluations are mandatory.

We experienced a rare case of traumatic ventricular septal defect by penetrating stab injury The patient was 26-year-old women who got stab wound at the left anterior third intercostal space and left sternal border with a knife. seven hours after admission, the patient was undertaken an emergency thoracotomy due to hypovolemic shock caused by massive bleeding from transected left internal mammary artery, vein, and right ventricular outflow tract. On postoperative second day, the patient was suffered from moderate dyspnea, and arterial blood gas analysis and chest X-ray revealed hypoxemia and pulmonary edema. Right heart cardiac catheterization with Swan-Ganz Cathater showed oxygen step-up between right atrium and main pulmonary artery and a 1.6:1 ratio of pulmonary to systemic blood flow. At operation, harsh systolic thrill was palpable along right ventricular outflow tract. Through small vertical right ventriculotomy, the linear ventricular septal laceration on infundibular septum was noticed, and its size was 1.5cm with sharp margin This defeat was repaired by three interrupted matress sutures using Prolene 4-O with pledget. Her postoperative course was uneventful, and she discharged with good physical condition.
