        강직성 척추염 환자마취 1예

        During surgical repair of a descending thoracic aorts aneurysm, one-lung ventiation improves visualization of the surgical field, facilitates surgical resection, and reduces lung tranuma. Trauma to the left lung during surgical repair of the descending thoracic aorta is very common, and intrapulmonary hemorrhage can be occurred if both lungs are ventilated. A doubitus position, hypoxemia can frequently occur and hypoxic damage can result. Selective nondependent lung high frequency jet ventilation was administered using a MERA JET VENTILATOR with FiO_2 1.0, driving gas pressure 1 kg/㎠, respiration rate 120/min., and an I:E ratio of 1:2. Oxygenation (PaO_2 readings were all above 200 torr) was well maintanined and PaCO_2 values were all within normal limits thoroughout the operation. Under the left atrial-femoral artery bypass with the cardiopulmonary bypass machine without oxygenator, an internal bypass graft with Woven dacron was performed. The patient tolerated the surgery well, and the postoperative course was uneventful except for left for left vocal cord paralysis.

        접합점 선택에 의한 표면 이완 모형

        A random deposition model with height relaxation between bonds is studied. The surface width W increases with time t as t^β at early time and becomes saturated at L^ α for t >>L^z,where L is the system size. In 1+1 dimension, the growth exponent β=0.247±0.007, the roughness exponent =0.510±0.015 and the dynamic exponent z =approx 2.06. In 2+1 dimension, the surface width W grows as b log t and follows a log L in the saturated region. The dynamic exponent z=2.01 is obtained from the relation z = a/b. The exponents belong to the Edwards-Wilkinson universality class. 격자점 사이의 접합점 (bond)을 선택하고, 입자를 낮은 쪽으로 보내는증착 모형에 대한 경계면 거칠기를 연구하였다. 표면 거칠기 W는초기에는 t^β로 성장하지만, 시간이 t >> L^z인 구간에서는L^α로 포화된다. 1+1 차원에서 거칠기 지수 α=0.510 ±0.015, 성장 지수 β=0.247 ±0.007, 동역학 지수 z= 2.06 를 얻었다. 2+1 차원에서는 W는 처음에는 b log t로성장하고, 이후 a log L로 포화된다. 계수 a와 b의 비율로써z =2.01 를 얻었다. 이 결과는 Edwards-Wilkinson 방정식과 잘일치한다.

        Rapid Start of Robust Radiation Helped by a Non-competitive Neighbor Mode in a Gigawatts-class Magnetically Insulated Line Oscillator

        A rapid start of robust radiation helped by a temporary excitation in an axial mode adjacent to the desired pi-mode is shown in a gigawatt-class magnetically insulated line oscillator by using a particle-in-cell simulation. A time-frequency analysis of the microwave output signal demonstrates that an oscillator with four interaction cavities experiences a delayed start of oscillation due to a severe mode competition whereas an oscillator with eight interaction cavities rapidly radiates strong microwaves in the non-competitive neighbor mode, then the operating mode softly moves to the pi-mode without microwave power loss because of the relatively small difference between the phase velocities of the modes.

        ICT 장비 ․ SW 글로벌화 기반구축 사업의 경제성 분석

        The research infrastructure project for the ICT devices and device SW is aimed to support and strengthen the capability of the SMEs when they are trying to enter into the global ICT core market. In other words, this project is to support the IT device and device SW SMEs which have innovative technoloigies with the tangible and/or intangible infrastructures which are needed for them to enter into the global ICT collaboration ecosystems. The purpose of this paper is to assess the economic feasibility of this project by analyzing its cost and benefit of this research infrastructure project for ICT devices and device SW which will be implemented according to the need of these days. The result of the analysis showed that the BC ratio was 1.510, which means this project has the economic feasibility. And this study did the sensitivity analysis on the two factors, the revenue ratio by cost reduction and the discount ratio, and the durability analysis of the effects. ICT장비 및 장비 SW 중소기업이 글로벌 ICT핵심 시장의 생태계에 조기 진출하기 위한 역량 강화를 위한 지원하기 위한 사업이 ICT장비․SW 글로벌화 기반구축 사업이다. 이 사업은 혁신기술을 보유한 IT장비 및 장비SW 중소기업이 글로벌 ICT협업 생태계에 조기 진출하는데 필요한 유·무형의 인프라를 구축하여 지원하고자 하는 것이다. 이 연구는 이러한 시대적 요구에 의해 수행하고자 하는 ICT장비․SW 글로벌화 기반구축 사업에 대하여 투입되는 비용과 그에 따른 편익을 분석함으로써 그 경제적 타당성에 대하여 분석하였다. 그 분석결과 비용-편익 비율은 1.510으로 산출이 되어 이 사업이 경제적 타당성을 가지고 있음을 알 수 있었다. 또한, 비용-편익 분석에서 사용한 비용절감 수익률과 할인율 변동에 대하여 이 요인들의 변화에 따른 민감도 분석을 실시하였고, 이와 더불어 수익의 지속 가능성에 대하여도 분석을 실시하였다.

        Clinicopathological Characteristics of Colorectal Cancer according to Gender Difference

        Background/Aims: Colorectal cancer (CRC) has been one of the major causes of death and has become a major public health concern. The incidence of CRC has been increasing regardless of gender in Korea. Until now, however, the studies on gender-based clinicopathological characteristics of CRC focused on pathology have never been reported. Therefore, we aimed to evaluate the difference in clinicopathological characteristics of CRC according to gender in Korea. Methods: Medical records of 342 patients with advanced CRC who underwent surgical resection at Kyung Hee University Hospital at Gangdong from June 2006 to December 2011 were retrospectively analyzed. The data of the clinicopathological characteristics of CRC by gender difference were compared. Results: Of these 341 patients, 203 (60%) patients were male and 138 (40%) patients were female. The male preponderance was noted in all age groups for total CRC and left-sided CRC. However, for right-sided colon cancer, this male preponderance was significantly decreased with increasing age groups (P=0.025) and was finally reversed in elderly groups (age ≥60 years). The microsatellite instability represented by negative staining for hMLH1 and hMSH2 was more frequently detected in women than men (P=0.037). Conclusions: The male preponderance in right-sided colon cancer decreased with increasing age groups and finally reversed in age groups more than 60 years. Microsatellite instability with immunohistochemical staining was more frequently detected in women. However, further studies with a large number of patients are warranted on this issue.         Electron Kinetic Property of a Magnetized Dual-Frequency Capacitively-Coupled Plasma

        Spatially-resolved electron-energy-distribution functions (EEDFs) of a magnetized dual-frequency (60 and 2 MHz) capacitive discharge are numerically investigated by using a one-dimensional particle-in-cell/Monte Carlo collision simulation. It is shown that the electron kinetic property over the entire energy range changes from a nonlocal to a local regime, as the magnetic eld increases. This transition results from a reduction in the electron diusion in coordinate space and an enhancement of the electron diffusion in energy space due to an increase in the magnetic field. In an asymmetric discharge, it is found that the electron kinetic property near the 2 MHz electrode, which has a smaller electrode with an external capacitor, is more localized than that near the 60 MHz electrode.

        Restricted Height Model with Conserved Noise

        We introduce a restricted height model with conservation of the total number of particles. The surface width W(L, t) of our model follows a scaling formula W2(L, t) = W2 i − L2αf(t/Lz) as a function of time t, where Wi and L are an intrinsic width and the system size, respectively. By subtracting the surface width from the intrinsic width, we successfully obtain the roughness exponent α −ds/2 and the growth exponent β −ds/4 for substrate dimensions ds = 1, 2, and 3. We also obtain the dynamic exponent z 2 in all substrate dimensions. The relation between our model and the conserved Edwards-Wilkinson equation is also discussed.
