        오버헤드 스쿼트 운동이 초등학생들의 자세교정에 미치는 영향

        In this study, two times of exercises a week for 8 weeks were implemented by selecting a total of 30 students, 10 students of squat group, 10 students of overhead squat group and 10 students of control group from 30 male students in grade 4 to 6 in an elementary school located in Gyeongsangbuk-do, in order to investigate the effect of overhead squat exercise on body shape change of elementary school students. The analysis results before and after exercise were as follows: The cervical angle from the side view: The overhead squat group showed a statistically significant difference after exercise(p<.001). In scapula height from the back side view(p>.05) in the squat group, but there was a statistically significant difference found in the overhead squat group(p<.05). It is considered that this study helps better understand what kind of positive effects the overhead squat exercise will bring in children’s body shape change and it can be provided as a basic data for the development of a body correction exercise program for growing children.

        2012-2016년 경희대학교 한방병원 한방안이비인후과 외래환자에 대한 통계적 분석

        Objectives : The purpose of this study is to research on the characteristics of outpatients and distributions of diseases in ophthalmology, otolaryngology of Korean medicine in 2012-2016 and compare the result with previous studies. Methods : This study is based on the analysis of medical records of total 2,319 outpatients in ophthalmology, otolaryngology of Korean medicine at Kyung Hee Korean medicine hospital from January 1, 2012 to December 31, 2016. We searched on the demographic characteristics of outpatients and the distributions of diseases in order of frequency by age and sex. Then, we compared these results to previous results of papers at same hospital in 2000 and 2007. Results : The common diseases in 2012-2016 were dry eye syndrome in ophthalmology(32.14%), tinnitus and hearing loss in otology(72.69%), rhinitis in rhinology(82.20%) and laryngopharyngitis in laryngology(67.77%). The most increasing diseases in the distribution between 2004-2006 and 2012-2016 were retinopathy in ophthalmology(+366.77%), etc including the eustachian tube disorder in otology(+151.55%), olfactory disorder in rhinology(+167.50%) and laryngopharyngitis in laryngology(+126.20%). Conclusions : If we accumulate these kinds of epidemiologic studies on ophthalmology, otolaryngology of Korean medicine, it will be great help to make Korean medical preparations for the increasing diseases on these fields.

        건선 치료용 한약제제에 대한 임상시험 가이드라인 설명 및 제언

        Objectives : We report on the contents and reviews of the guidelines for clinical trials of herbal medicine for psoriasis. Methods : After reviewing the existing reports, the guidelines were drafted by the writing committee on guidelines for Korean medicine clinical trial center in Kyung Hee university Korean medicine hospital. Since then, the guidelines have been consulted by the experts in diseases, clinical trials and pharmaceuticals. In addition, the guidelines have been certified by the Korean medicine clinical trial center association and Korean Medicine Ophthalmology & Otolaryngology & Dermatology Society. Results : The guidelines for clinical trials are divided into 8 categories: (1) general contents, (2) evaluation standards of effectiveness, (3) outcome valuables, (4) selection of study participants, (5) designs of clinical trials, (6) safety evaluations, (7) combination therapies in psoriasis treatment and (8) Korean medical considerations. Conclusions : There are 3 major contents for discussion: (1) obscurity of Korean medical considerations to differentiate from existing guidelines, (2) the absence of Korean version of PASI and DLQI, which are validity parameters and (3) realistic institutional problem using the herbal medicine.

        선량보강(Build-up) 영역에서의 광자선 깊이선량률 측정을 위한 검출기의 특성 비교

        표면 선량을 포함한 선량보강(build-up) 영역에 대한 깊이선량률(percentage depth dose)을 측정하는 데 있어서 적절한 측정방법을 결정하기 위해 6 MV 광자선에 대해 Atiix와 Markus 평행평판형 이온함, 원통형이온함, 그리고 다이오드 검출기를 사용하여 측정, 비교하었다. Attix 이온함에 의한 측정을 기준으로 할 때, Markus 이온함의 측정은 민조사면(open field)에서 $2\%$ 내로 일치하였으나, 오염전자가 포함된 광자선의 경우에는 최대 $3.9\%$의 차이를 보였다. 원통형 이온함과 다이오드 선량계의 경우에는 이들 검출기가 물 팬텀에 완전히 잠긴 이후부터는 오염전자가 포함된 광자선에 대해서도 각각 $1.5\%$ 혹은 $1.0\%$ 내의 정확도를 보였다. 따라서 민조사면에서 표면선량을 포함한 깊이선량률을 정확히 측정하기 위해서는 평행평판형 이온함이 추천되나, 표면에서의 정확한 선량에 특별한 관심을 두는 경우가 아니면, 원통형 이온함이나 다이오드 선량계를 이용하여 선량보강영역의 깊이선량률을 측정하는 것은 양호한 결과를 준다고 할 수 있다. To determine the appropriate method out of various available methods to measure build-up doses, the measurements and comparisons of depth doses of build-up region including the surface dose were executed using the Attix parallel-plate ionization chamber, the Markus chamber, a cylindrical ionization chamber, and a diode detector. Based on the measurements using the Attix chamber, discrepancies of the Markus chamber were within $2\%$ for the open field and increased up to $3.9\%$ in the case of photon beam containing the contaminant electrons. The measurements of an cylindrical ionization chamber and a diode detector accord with those of the Attix chamber within $1.5\%\;and\;1.0\%$ and after those detectors were completely immersed in the water phantom. The results suggest that the parallel-plate chamber is the best choice to measure depth doses in the build-up region containing the surface, however, using cylindrical ionization chamber or diode detector would be a reasonable choice if no special care is necessary for the exact surface dose.

        Obesity and Hypertension in Association with Diastolic Dysfunction Could Reduce Exercise Capacity

        Background and Objectives: Empirical evidence is lacking on the cumulative disease burden of obesity and hypertension and its impact oncardiac function and exercise capacity. The purpose of this study was to determine whether the presence of obesity and hypertensiontogether was associated with cardiac dysfunction and exercise capacity. Subjects and Methods: Using a retrospective study design, medical records were reviewed for echocardiographic and treadmill exercisestress test data. Subjects were grouped according to four categories: normal control, obese, hypertensive, or obese and hypertensive. Results: Obese, hypertensive persons showed significantly lower Ea and E/A ratio and greater E/Ea ratio, deceleration time, left ventricular(LV) mass, and LV mass index compared to their counter parts (normal control, obese and/or hypertensive) (all p<0.05), after controllingfor age and sex. After controlling for age and sex, significant differences in exercise capacity indices were found, with the obese grouphaving shorter exercise time, lower metabolic equivalents, and lower maximal oxygen uptake than the normal control, hypertensive, orboth groups (all p<0.05). The hypertensive or obese and hypertensive group had greater maximal blood pressure compared with thenormal control group (all p<0.001). Obese and hypertensive persons were approximately three times more likely to have diastolicdysfunction (odd ratio=2.96, p=0.001), when compared to the reference group (normotensive, non-obese, or hypertensive only persons). Conclusion: Diastolic dysfunction was associated with obesity and/or hypertension. The cumulative risk of obesity and hypertension andtheir impact on diastolic dysfunction which could be modifiable could reduce exercise capacity.

        건선 한약제제 임상시험 가이드라인 개발을 위한 관련 국내외 가이드라인과 기존 건선치료 한약제제 연구와의 비교

        Objectives : The objective of this study is to develop the strategies of the psoriasis clinical trials guideline on Korean medicine by comparison between Clinical guidelines and domestic and foreign clinical trials papers of psoriasis on Korean medicine. Methods : This study is based on analysis of papers on Clinical trials guidelines, Clinical practice guideline and clinical trials about Korean medicine. The papers were searched from Pubmed, Medline, Oasis(Oriental Medicine Advanced Searching Integrated System), Korean Traditional Knowledge Portal and Google portal database. Results : A total 8 Clinical practice guidelines and 2 Clinical trials guidelines were found. Moreover, there were 15 foreign papers about clinical trials and 29 internal articles about case studies. They suggested the diagnostic strategy, classification, effective outcome measure, severity measure, precaution of combination therapy, precaution and treatment period of clinical trials, safety evaluation, patterns of Korean Medicine, clinical specific features on psoriasis.Conclusions : The criteria of every item to provide the clinical trials guideline using Korean medicine on psoriasis were developed by apply the results. If we accumulate the more clinical articles on Korean medicine, it will be great help to develop the reliable standard of that guideline.

        방사선 치료에서 치료 표적과 조사 빔의 일치 정도 평가: 팬텀 연구

        CT와 치료계획시스템 및 다엽콜리메이터(Multi Leaf Collimator, MLC)를 이용한 방사선 치료의 전 과정을 팬텀에 대해 재현해 봄으로써, 치료계획상의 표적과 조사빔의 일치 정도를 평가해 보았다. 이를 위해, CT 및 그 레이저 정렬을 위한 팬텀을 제작하였으며, 치료실의 레이저 정렬을 손쉽게 할 수 있는 방법을 고안하였다. 조사빔의 확인에는 필름을 이용하였고, 조사빔과 실제 표적위치 사이의 정량적인 분석은 자체 제작한 프로그램을 이용하였다. 표적과 빔 중심의 불일치 정도는 테이블 각도에 따라 차이가 있었으며 시험에 이용된 치료기의 경우, 테이블 각도 에서 최대 2.0 mm의 차이를 보였다. MLC field에 대해 Winston-Lutz 테스트를 시행하여 $295^{\circ}$얻은 isocenter를 기준으로 다시 시험한 결과는 측정한 모든 테이블 각도에서 1.35 mm 이하였다. 표적과 빔 중심의 차이에 관한 이러한 평가는 실제 환자의 치료 계획 설정에 유용하게 이용될 수 있다. We evaluated the positional accuracy of the delivered beams to the target in a phantom by simulating the whole process of the radiation treatments Including CT scanning, planning and beam exposures with MLCs. For this purpose, a phantom was made to calibrate the alignment between the CT and the attached laser system. A new, convenient method was also devised to align the setup lasers in the treatment room. Film was used for the Identification of the delivered beam and analyzed with a homemade computer program. The positional differences between the target and the beam centers varied with the couch rotations. The accelerator we used showed a maximum discrepancy of 2.0 mm at the table angle of $295^{\circ}$. The same measurements based on the new isocenter from the Winston-Lutz test resulted in the maximum of 1.35 mm for all rotation angles. The evaluation of the differences between the target and the beam centers is useful for the treatment planning.

        제작된 선량 검증용 IMRT 팬텀의 몬테칼로 시뮬레이션: 예비적 연구

        세기조절방사선치료(Intensity modulated radiation therapy, IMRT) 선량의 정확도를 원격으로 점검할 수 있는 시스템 구축을 목적으로, 두경부용 팬텀을 제작하였다. 팬텀은 공기 및 뼈 구조물의 교체 장착을 통해, 균질 혹은 비균질 팬텀으로 이용할 수 있다. 일차적으로 단일 빔 및 세 빔의 조사에 대한 몬테칼로(Monte Carlo, MC) 전산모사를 시행하고, 이온 전리함 및 열형광선량계(thermoluminescent dosimeter, TLD)를 이용한 측정과 비교하였다. TLD의 판독은 독립된 두 기관에서 수행하였다. 단일 빔의 경우, 균질 및 비균질 팬텀에 대한 이온 전리함 측정 결과는 MC전산모사 결과와 대체로 2% 수준에서 일치하였으며, TLD 의 경우에는 기관에 따라 2% 혹은 7%수준의 차이를 보였다. 세 빔을 이용한 비교의 경우에는, 이온 전리함은 -5% 수준, TLD의 경우에는 $+2{\sim}+3%$ 수준의 차이를 보였다. 원격 선량 검증을 위해서는 TLD 판독의 신뢰도 향상이 필요한 것으로 판단되었다. MC 전산모사는 새로운 팬텀 개발 시에, 선량 측정값의 신뢰도 확보에 이용될 수 있다. A head-and-neck phantom was designed in order to evaluate remotely the quality of the delivery dose of intensity modulated radiation therapy (IMRT) in each institution. The phantom is homogeneous or inhomogeneous by interchanging the phantom material with the substructure like an air or bone plug. Monte Carlo simulations were executed for one beam and three beams to the phantom and compared with ion chamber and thermoluminescent dosimeter (TLD) measurements of which readings were from two independent institutions. For single beam, the ion chamber results and the MC simulations agreed to within about 2% TLDs agreed with the MC results to within 2% or 7% according to which institution read the TLDs. For three beams, the ion chamber results showed -5% maximum discrepancy and those of TLDs were $+2{\sim}+3%$. The accuracy of the TLD leadings should be increased for the remote dose monitoring. MC simulations are a valuable tool to acquire the reliability of the measurements in developing a new phantom.
