Background: Pathologic examination is a very important diagnostic procedure. It is the most important method to decide the therapeutic plan and to predict the prognosis of cancer patients. The Resource-Based Relative Value Scale (RBRVS) is a schema used to determine how much money medical providers should be paid. In Korea, a modified RBRVS has been used since the year 2000. Methods: We researched the July 2010 RBRVS for Korea and the US medicare. The individual Relative Evaluation Index (REI) is defined as the ratio of an individual RBRVS to the mean RBRVS. The REIs of pathologic examination in Korea and America were compared. Results: For an endoscopic biopsy specimen, the pathologic examination REI in Korea was 55.4% of the American REI. The Korean REI of a prostate biopsy (8 sites) was only 5.7% of the American REI. The Korean REI was 28.1% of the American REI for the hysterectomy for uterine myoma, and the Korean REI was 67.6% of the American REI for resection of stomach or colon cancer. Conclusions: The RBRVS of pathologic examination in Korea remains undervalued. Considering the importance of pathologic examination in medicine, the RBRVS in Korea should be increased.

The purpose of this study is to investigate whether paraspinal muscle wasting occurs in association with chronic low back pain(LBP) and to know whether low back muscle exercise is effective for abating chronic low back pain Cross sectional areas(CSA) of lumbar vertebral bodies and paraspinal muscles were measured using CT at the level of the fouth lumbar vertebral body in 37 patients(23 males. 14 females) with acute low back pain and in 44 patients(2O males, 24 females) with chronic low back pain. The results were as follows; 1) The mean age of the patients with chronic LBP were higher than those with acute LBP 2) The CSAs of L4 vertebral bodies were correlated significantly with body weight and the CSAs of L4 vertebral bedies. erector spinae and psoas muscles were larger in males than in females. 3) The volumes of the paraspinal muscles were significantly reduced in patients with chronic LBP compared to acute LBP. These changes can cause erector spinae and psoas muscle weakness and disuse atrophy and thereby predispose to spinal instability and progressive dysfunction.
Expression of MUC1 and MUC4 and Its Prognostic Significance in Non-Small Cell Lung Carcinoma

Background : Mucin (MUC)1 and MUC4 (MUC1, 4) are high molecular weight glycoproteins expressed in normal and malignant epithelial cells, and these expressions are related to the prognosis of some carcinomas. In non-small cell lung carcinoma (NSCLC), the relationship between MUC1, 4 expressions and their prognostic significance is not well known. We evaluated these relationships in a series of NSCLC: 1) between MUC1, 4 expression levels and histologic subtypes, and 2) between high expression of MUC1, 4 and their prognostic significance. Methods : We performed immunohistochemical staining for MUC1, 4 in paraffin-embedded tissues from 165 NSCLC cases arranged in a tissue microarray. Results : We found a significant correlation between MUC1, 4 expressions and NSCLC histologic subtypes (p < 0.05). High MUC1 expression was characteristic of adenocarcinoma. Low MUC1, 4 expressions were characteristic of squamous cell carcinoma. In adenocarcinoma, we found significant association between diffuse MUC1 expression and short patient survival (p = 0.005). In squamous cell carcinoma, diffuse MUC4 expression showed long patient survival trend (p = 0.128). Conclusions : MUC1, 4 expression levels were significantly correlated with NSCLC histologic subtypes. Diffuse MUC1 expression was significantly associated with shortened survival in NSCLC patients, especially in adenocarcinoma.

5% of ovarian neoplasms consist of granulosa cell tumors. 10% of cases coexist with pregnancy2. We report on delivery of normal infant in young woman with granulosa cell tumor diagnosed and treated during pregnancy. At laparotomy a large right ovarian granulosa cell tumor was found and right salpingo- oophorectomy was performed. A normal infant was delivered by cesarean section at full term.

In order to understand the relationship between the distribution of epilithic diatoms and the habitual environments, land-use, water qualities, and epilithic diatoms were studied at 141 sampling sites in the midwestern stream of Korean peninsula (Geum river, Mangyeong river, Dongjin river, and Sapgyo river). The total 183 diatom taxa was appeared in the study, while the dominant species were found to be Nitzschia palea (10.9%) and Achnanthes convergens (8.4%). Based on the abundance of epilithic diatoms, a cluster analysis results indicate that the sampling sites divided the sampling sites into 4 groups (G) at the 25% level. In term of geographic and aquatic environments, G-I and -II accounted for the upper and mid streams of the Geum river, and had large forest areas and good in water quality. G-III accounted for farmland and urban, and high concentration nutrient levels (TN and TP) and electric conductivity. G-IV accounted for mostly farmland, and high levels in turbidity, BOD, nutrient and electric conductivity. CCA results showed that the saproxenous taxa Meridion circulare was the indicator species of G-I, which strongly influenced by altitude and forests. In G-II, the indifferent taxa Navicula cryptocephala was influenced by Chl-a, AFDM, and DO. In G-III and -IV, the indifferent taxa Fragilaria elliptica and saprophilous taxa Aulacoseira ambigua were influenced by electric conductivity, turbidity, and nutrient counts. Meanwhile, random forest results showed that the predicting factor of indicator species appearance in G-I, -II, and -III was found to be electric conductivity whereas in G-IV it was found to be turbidity. Collectively, the distribution of diatoms in the midwestern of Korean peninsula was found to depend more on the land-use and its subsequent water qualities than the inherent characteristics of the aquatic environment.
