슬관절 전방십자인대에 발생한 국소형 건막 거대세포종 - 2례 보고 -

국소형 건막 거대세포종이 체중 부하에 관여하는 큰 관절에서 발생하는 경우는 매우 드문 일이며 슬관절의 경우 지금까지 4례만이 보고되고 있다. 슬관절에서 국소형 건막 세포종이 발생할 경우 관절통, 종창, 잠김, 불안정감 또는 신전제한과 같은 증상이 나타날 수 있지만 임상적으로 십자인대파열 혹은 반월상연골 파열이 있는 경우와 감별하기는 어렵다. 저자들은 전방십자인대 재건술 때 국소형 거대세포종 2례를 우연히 관찰하였다. 두 경우 전방십자인대에서 발생하였으며, 관절경하에서 절제한 후 병리학적으로 진단을 확인하였다. 종양의 완전절제후 좋은 결과를 얻었기에 문헌고찰과 함께 보고하고자 한다. The localized form of a giant cell tumor of tendon sheath rarely affected the large weight-bearing joints. Only four cases of localized giant cell tumor arising in the knee joint could be found in the literatures. Physical examination of the affected knee could show pain, swelling, locking, giving way and limitation of extension, but it was difficult to differentiate from meniscal tear or cruciate ligament tear. Two cases of localized giant cell tumor of tendon sheath were incidentally found in the knees during the reconstruction of the anterior cruciate ligaments. Both tumors arising from the anterior cruciate ligament were excised arthroscopically and were confirmed to be localized giant cell tumor of tendon sheath by microscopic examination. The clinical results were good after complete excision, so we reported these cases with a review of the literatures.

Purpose: The purpose of this study is to report the result of endoscopic assisted curettage and bone graft for the treatment of simple bone cyst in the calcaneus. Materials and Methods: Three cases in three patients who had curettage and bone graft of simple bone cyst in the calcaneus under endoscopic view were prospectively reviewed. A minimum follow-up of 1 year was required for entrance into the study. The results of treatment were clinically and radiologically assessed. Results: One case with preoperative heel pain gained complete relief of the pain at postoperative four weeks. All three cases had radiological union of grafted bone at an average of thirteen weeks after the operation. Shortened hospitalization and decreased postoperative pain were remarkable. Complications such as infection, skin necrosis, nerve injury, and, fracture were not seen. There was no evidence of the recurrence of the cyst in all three cases. Conclusion: Endoscopic assisted curettage and bone graft of simple bone cyst in the calcaneus is thought as an effective alternative procedure avoiding the possible complications of the classic method.
