        Epstein-Barr virus-transformation of B-cell lines in ovarian cancer patients: feasibility of genomic storage for unlimited use

        Objective: The aim of the current study is to test whether immortalized B-lymphocyte cell line via Ebstein-Barr virus (EBV) transformation is feasible and can be an unlimited source of genome wide study. Methods: We obtained peripheral whole blood from 5 ovarian cancer patients and immortalized the B-cell lines using EBV transformation. The success rate was analyzed and the bio-identity of the genome was performed using human leukocyte antigen (HLA) identity test. Results: EBV transformation was successful in all 5 cases (95% confidence interval, 46.3% to 100%). After cryopreservation of EBV-transformed B-cell lines and subsequent thawing, we observed that all cell lines were viable and proliferative. To check bio-identity, HLA-A, B, and DR were tested between the genome of the original samples and the transformed samples. The HLA typing revealed that all observed HLA-A, B, and DR type was identical in 5 cases before and after EBV-transformation. Conclusion: The current results suggest that EBV-transformation of peripheral blood is an efficient tool in genome banking. The EBV-transformed B-cell lines may be a valuable resource of genome in multi-center translational research by the Korean Gynecologic Oncology Group.         안면 기형이 뚜렷하지 않은 Miller-Dieker 증후군 1예

        Miller-Dieker syndrome is a multiple malformation syndrome characterized by severe lissencephaly and characteristic facial abnormalities at birth. It is associated with visible or submicroscopic deletions within chromosome 17p13.3 including PAFAH1B1 (LIS1) gene. We report a six-month-old boy who presented with spasm and generalized myoclonic seizures. The patient was born at 40 weeks’ gestation to a 36-year-old woman and showed developmental delay without microcephaly or prominent facial abnormality. Magnetic resonance imaging of the brain showed a few gyrus (lissencephaly). High resolution cytogenetic analysis from peripheral blood showed a normal karyotype. However, fluorescence in situ hybridization (FISH) of the metaphase chromosome using Miller-Dieker/ILS probe (Oncor, Gaithersburg, Maryland, USA) revealed only one signal of probe, indicating a microdeletion of 17pl3.3 region including PAFAH1B1 (LIS1) gene. We suggest that FISH 17p13.3 studies should be performed in addition to a standard metaphase analysis in patients with lissencephaly even if facial anomaly is not noted. A confirmatory diagnosis using FISH would be helpful in terms of leading to allow genetic counseling and availability prenatal diagnosis to the family. (Korean J Lab Med 2004; 24:194-7) Miller-Dieker 증후군은 뇌 표면 주름이 없어져 평평해지는 활택뇌증과 특이적인 두개 안면 기형이 특징인 다장기 기형 증후군이다. 이 질환은 17번 염색체 단완 말단의 17p13.3 부위에 있는PAFAH1B1 (LIS1) 유전자와 그 주변 유전자들의 소실과 관련된 것으로 알려져 있다. 이 증례의 환아는 6개월된 남아로 발육 부진과 전신 간대성근경련발작을 주소로 내원하였다. 환아는 36세산모에서 제태연령 40주 3일에 2.9 kg로 출생하였는데 출생 당시소두증이나 특이한 안면기형은 관찰되지 않았다. 뇌자기공명영상결과 대뇌 뇌이랑의 숫자가 적으면서 넓어진 활택뇌증에 합당한 소견을 보였다. 말초혈액으로 시행한 고해상도 염색체 검사 결과에서정상 남성 염색체가 관찰되었으나, Miller-Dieker 증후군에 특이한 17p13.3 위치의 Miller-Dieker/ILS probe (Oncor, Gaithers-burg, Maryland, USA) 소식자를 사용하여 중기세포에 형광동소교잡법을 시행한 결과 한쪽 17번 염색체에서 결실이 관찰되었다. 활택뇌증이 관찰되어 Miller-Dieker 증후군이 의심되는 경우특이한 안면기형이 관찰되지 않더라도 염색체검사와 함께 17p13.3위치의 결실 검출을 위한 형광동소교잡법이 필수적이다. 형광동소교잡법은 Miller-Dieker 증후군 확진을 가능하게 하므로 환자와가족의 유전 상담 및 산전 진단 시에 유용하게 이용될 수 있다.

        De novo CD5 Positive Diffuse Large B-cell Lymphomas with Bone Marrow Involvement in Korean

        In CD5 positive (CD5+) mature B-cell lymphomas, newly recognized CD5+ diffuse large B-cell lymphoma (DLBCL) has been characterized by aggressive features. We studied twenty-five cases with CD5+ lymphomas involving bone marrow. Eleven cases were diagnosed as chronic lymphocytic leukemia, six cases were diagnosed as mantle cell lymphoma (MCL), and three cases with morphologic characteristics of MCL and without both the cyclin D1 expression and IGH/CCND1 rearrangement were unclassifiable. The remaining five cases, showing large to medium-sized lym-phoid cells with prominent nucleoli and a moderate amount of cytoplasm, were diag-nosed as DLBCL. Five DLBCL cases were positive for CD5, CD20, surface immuno-globulin, but negative for CD23. Patients with CD5+ DLBCL showed a high age of onset (median, 68 yr) and two patients expired one month after the diagnosis. Since CD5+ DLBCL forms a distinct subgroup of DLBCL, a study of CD5 expression in DLBCL would be helpful to predict prognosis and to determine future therapeutic strategy. To the best of our knowledge, this is the first report on de novo CD5+ DLBCL in Koreans.

        Effects of Multiple Drilling on the Ischemic Capital Femoral Epiphysis of Immature Piglets

        Purpose This study investigated the effects of multiple drilling on the immature capital femoral epiphysis following ischemic injury in a piglet model. Materials and Methods Ischemic necrosis of capital femoral epiphysis was induced bilaterally in 12 piglets using a cervical ligation method. Three weeks later, medial, central, and lateral 3 drill holes were made on the left femoral head using 0.062" K-wire. At 3, 6, 9, and 12 weeks following the multiple drilling, femoral heads were harvested from each three piglets. On histologic examination, percent of revascularization, percent of osteoblast surface, capital femoral epiphyseal quotient and proximal femoral growth plate height were evaluated. Untreated right femoral heads served as control. Results While percent of revascularization of left capital femoral epiphysis with multiple drilling was significantly higher than untreated control side (p<0.001), percent of osteoblast surface, capital femoral epiphyseal quotient and proximal femoral growth plate height showed no significant difference. Conclusion This study indicates that multiple drilling could promote revascularization of ischemic capital femoral epiphysis, and multiple drilling does not appear to produce bony physeal bars at short-term, if using small diameter drill. However, multiple drilling alone does not seem to prevent femoral head deformity or to promote new bone formation.

        콜센터 상담사의 감정노동과 직장 내 괴롭힘이 우울과 주관적 건강인식에 미치는 영향

        여성집중 수준이 높은 콜센터 상담사들은 안전하지 않은 노동환경에서 심각한 수준의 감정노동과 직장 내 괴롭힘을 겪고 있다. 우리나라에서는 2018년 산업안전보건법의 개정을 통해 콜센터 상담사를 포함한 고객응대 노동자의 감정노동으로 인한 피해보호조치의 근거가 마련되었고, 2019년 근로기준법 개정을 통해 직장 내 괴롭힘 피해에 대한 보호조치 의무가 사업주에게 부과되었다. 그동안 콜센터 상담사에 대한 연구는 주로 인적자원 관리 관점에 기초하고 있거나, 감정노동 실태와 현상을 파악하기 위한 것에 초점을 둔 경향이 있다. 이에 본 연구는 국내 콜센터 상담사들의 감정노동과 직장 내 괴롭힘이 우울과 주관적 건강인식에 미치는 영향을 파악하고 이를 근거로 정책 및 제도적 개선방안에 대해 논의하는 것을 목적으로 한다. 이를 위해 2020년 8월 노동조합과 직장갑질119를 통해 자발적 조사참여자 총 501명을 대상으로 웹 기반 설문조사를 실시하였다. 분석결과 콜센터 상담사들의 감정노동 수준은 높았고, 직장 내 괴롭힘 피해율은 22.8%였다. 감정노동과 직장 내 괴롭힘은 다양한 인구사회적 변인을 통제한 상태에서 유의하게 우울과 주관적 건강인식에 영향을 미치는 것으로 확인되었다. 이를 토대로 콜센터 상담사의 감정노동과 직장 내 괴롭힘 예방 및 해결을 위한 개선방안을 제언하였다.

        Serum HER2 as a Response Indicator to Various Chemotherapeutic Agents in Tissue HER2 Positive Metastatic Breast Cancer

        Purpose: The aim of study was to evaluate the usefulness of serum HER2 as a therapeutic response indicator in patients with HER2 positive metastatic breast cancer (MBC).Materials and Methods: The levels of serum HER2 and CA15.3 were assayed in 148 serial serum samples from 50 HER2 positive MBC patients at both the baseline and follow-ups. The changes in the levels of serum HER2 and CA15.3 in relation to the tumor responses to the various chemotherapy regimens were monitored.Results: The levels of serum HER2 and CA15.3 were elevated in 82% and 62% of tissue HER2 positive patients, respectively, prior to therapies, with the changes in both tumor markers showing statistical significance in relation to the tumor responses (p〈0.01) in patients with elevated baseline serum markers.Conclusion: The level of serum HER2 could be a valuable response indicator, not only for trastuzumab containing therapy, but also for other common MBC chemotherapeutic agents. Also, as it is more frequently elevated, the serum level of HER2 may also be a more useful tumor marker than CA15.3 in HER2 positive MBC. (Cancer Res Treat. 2006;38:35-39)
