Lipomas are among the most common benign tumors elsewhere in the body. In the head and neck area, lipomas mainly arise in the posterior cervical triangle and the forehead. But they are only rarely observed in the parotid gland. Lipoma of the parotid gland is frequently mistaken as a mixed tumor or Warthin's tumor because of its location and rarity. We report one case of a lipoma in the superficial lobe of parotid gland with the brief review of literatures.

Background and Objectives : Dysphonia may be secondary to many different type of benign vocal cord lesions such as vocal polyp, vocal nodule, Reinke's edema, and intracordal cyst. Diagnosis and treatment of intracordal cysts are more difficult than other benign vocal cord lesions. But postoperative voice analysis of intracordal cyst have rarely been reported in the literature. The purpose of this study is to analyze aerodynamic and acoustic results and videostroboscopic findings before and after laryngomicrosurgery. Materials and Methods : We reviewed the pre and post-operative voice analysis results and videostroboscopic findings of 15 surgically treated patients of intracordal cysts at Severance hospital from Jun. 1997 to Nov. 1999 retrospectively. They were diagnosed with videostroboscopic findings, surgical findings, and pathologic reports. Their pre and post-operative speech were analyzed with MDVP(Multi Dimension Voice Analysis Program) of CSL(Computerized Speech Lab) and Aerophone II. Their pre and post-operative mucosal wave of true vocal cord was analyzed with videostroboscopy. In order to compare this results with normal group, 10 of normal persons were evaluated with same methods. Results : After the operation, mucosal wave of true vocal cord was improved in all patients. Postoperative acoustic and aerodynamic results were improved in almost parameters, but they did not reach the normal value. Conclusions : Videostroboscopy was essential in diagnosing intracordal cysts. By comparing the acoustic and aerodynamic results and video-stroboscopic findings before and after the laryngomicrosurgery, postoperative vocal function was defined more accurately and objectively. Almost parameters may be useful in assessing the quantitative changes in vocal quality before and after the laryngomicrosurgery.
편측 갑상선 절제술 후의 갑상선 기능저하증에 대한 임상적, 병리학적 고찰

Background and Objectives:In hemithyroidectomy, most surgeons don’t care about postoperative hypothyroidism because ofthe remaining functioning contralateral thyroid lobe, but the incidence of hypothyroidism after hemithyroidectomy is reported from5 to 35%. The purpose of this study was to assess the incidence, natural history, factors contributing to hypothyroidism after hemithyroidectomy.Subjects and Method:212 consecutive cases of hemithyroidectomized patients diagnosed from Feb. 2001 throughDec. 2004 were retrospectively analyzed. One hundred thirty-six of 212 cases were enrolled in this study after the exclusion criteriaapplied. The definition of postoperative hypothyroidism was based on serum TSH and free T4 level. The relationship betweenhypothyroidism and several factors, such as mean age, sex, preoperative free T4, TSH, microsomal antibody, thyroglobulin antibodylevels and lymphocytic infiltration of the resected gland, were stastically analyzed. Results:The fifty eight of 136 patients(42.6%) became biochemical hypothyroidism postoperatively. Among them, 11 patients (8.1%) showed overt hypothyroidismand 47 patients (34.5%) showed subclinical hypothyroidism. In postoperative hypothyroid group, preoperative TSH, microsomaland thyroglobulin Ab levels and lymphocytic infiltration were significantly higher than in postoperative euthyroid group (p<0.05).The odds ratio of preoperative microsomal Ab, thyroglobulin Ab level and lymphocytic infiltration of resected gland was 17.18,3.01 and 2.26 folds, respectively. Conclusion:In this study, we confirmed that there was unexpectedly high incidence of postoperativehypothyroidism after hemithyroidectomy. We propose that preoperative high TSH level, positive microsomal Ab, thyroglobulinAb and marked lymphocytic infiltration of resected gland could be predicitive factors of postoperative hypothyroidismafter hemithyroidectomy. The elucidation of the signal-transduction pathways that drive neoplastic transformation has led to novel rationally designed cancer therapeutics (targeted therapy). Several compounds directed against different molecular pathways are being tested in patients with advanced thyroid carcinoma. Anticancer drugs that target protein kinases include small molecule inhibitors and monoclonal antibodies. Feedback loops and cross talk between signaling pathways impact significantly on the efficacy of cancer therapeutics, and resistance to targeted agents is a major barrier to effective treatments. Increasingly, therapies are being designed to target multiple kinase pathways. This can be achieved using a single agent that inhibits multiple signaling pathways or a combination of highly selective agents. In this review we discuss the principles of specifically targeting multiple kinase pathways with particular reference to targeted therapy for thyroid cancer. Generally, the gradual onset of symptoms relating to the upper airway tract may be associated with thyroid tumors involving the tracheal lumen or with large masses compressing the trachea. Even though thyroid diseases are quite common in the general population, acute airway distress due to tracheal compression by thyroid masses rarely occur. Upper airway infection and hemorrhage into the thyroid may precipitate acute symptoms that cause stridor. We experienced two cases of rapidly enlarging benign tumor of the thyroid causing the acute onset of upper airway distress symptoms. So we present the cases with a review of the related literatures. 이하선 절제술시 Harmonic Scalpel사용의 유용성

Background and Objectives:The use of the harmonic scalpel recently has been described in various surgery, such as thyroidectomy, tonsillectomy and submandibular gland resection. It appears that the use of harmonic scalpel results in decreased operative time and blood loss compared to conventional techniques of vessel ligation and tissue division without increasing the incidence of nerve injury. The aim of this study was to determine whether the application of harmonic scalpel in parotidectomy was effective and safe compared to conventional parotidectomy techniques. Subjects and Method:From March 2001 through Jun 2005, 21 patients who underwent parotidectomy under aid of Harmonicscalpel were compared with 38 patients of conventional parotidectomy techniques. Intraoperative blood loss, operation time, facial nerve injury, wound complication, postoperative drainage and hospital stay were compared between two groups. Results:There were no significant differences between two groups with postoperative wound complication, facial nerve function and hospital stay. But the use of harmonic scalpel was associated with a significant reduction in operation time and intraoperative blood loss. Conclusion:The application of harmonic scalpel was very useful and relatively safe in parotidectomy. However, further research involving a randomized study is needed..
